七十二症辨治方法

1.发热
翕翕发热为表热，风寒甫客皮肤烈；此为表热里无恙，无汗解发功方捷。
蒸蒸发热为里热，阳邪下陷入阴辙；譬之蒸笼气达外，有渴下之休跋涉。
又有表邪未罢症，欲传入里兴妖孽；里来作热难容受，半表半里发为热。
治法不外和为贵，伤寒重大不论舌；无汗麻黄汗桂枝，太阳分野别须知。
2.潮热
潮热有常如潮水，阳明荆棘使之然；潮于寅卯属肝木，其余仿此细思量。
消导疏通为治法，温能除热忌寒凉；又有劳伤潮热症，不比伤寒一样腔。
3.恶寒恶热 寒热往来 似疟非疟
寒热如何似往来，邪气分争胆受灾；恶寒以卜邪居表，恶热先知邪内栽。
若是邪居半里表，所以寒热有往来；日夜发作无定数，传过别经即不来。
又有似疟非疟症，有时作止亦安排；通用柴胡汤和解，误投下吐亦衰哉。
4.烦热 烦躁
感冒风寒或失表，变为烦热心中扰；又有烦热不同科，阳虚阴实所当晓。
阳实错乱心烦热，阴虚愤怒肾烦躁；烦则热轻燥热甚，烦躁先后分燥烦。
5.恶风 背恶风
伤风恶风寒恶寒，腠理不密致邪干；恶风悉属阳和气，不比阴阳有恶寒。
又有伤寒背恶风，背为阳会腹为阴；令背恶寒阳不足，姜活冲和所必宗。
6.头眩 头痛 项强
伤寒头痛与头眩，不比寻常部位召；邪在太阳头必痛，房劳只把补为君。
若是瘥家身体痛，发汗亡阳变作风；又有四肢不自便，病前病后补泻分。
7.痞满 结胸 胸膈满
无热恶寒发于阴，反不招来痞满形；按之不痛伤寒血，胸膈满分降无升。
发热恶寒发于阳，误下开门损寇然；按之方痛结胸小，不按自痛大堪伤。
热结胸兮烦且渴，寒结胸兮闷怕凉；水结胸中肿且汗，不轻误下别相商。
8.腹痛 腹满 小腹满
脾为中央戊巳土，太阴疾病腹受苦；阳毒为邪手足温，痛有停止言语纷。
满无稍长如斯已，阴毒生端又不同；腹满欲便无休息，满兼积瘀可消分。
9.无汗 盗汗 自汗 头汗 手足汗
寒邪伤中足三阳，腠理固密外作殃；津液内渗无汗雨，麻黄汤表最为良。
寒邪伤中足三阴，腠理松疏汗自淋；流而不止亡阳症，桂枝汤里可搜寻。
盗汗偷流睡梦里，醒来即止影无踪；半表半里邪盘踞，胆热柴胡汤一钟。
邪在诸阳境界搏，自然头汗至项还；久雨不晴为阳脱，黄芪汤和桂枝攒。
手足脾胃之枝叶，寒从火化中央热；津液滂流四肢汗，清脾饮去功方捷。
10.下痢便脓血 大便难 小便难 小便不通 小便自利
下痢阴阳要认真，水来乘土病根因；伤寒误下多如此，自便自利一般情。
三阳下痢身浑热，手足温和属太阴；少阴数体水如冰，若是心凉救厥阴。
伤寒又有便脓血，肠胃阴寒邪所客；清浊不分带经湿，不比寻常之失血。
其余大小便诸症，可就本门寻法则；伤寒传变似风雷，不象杂症易通寒。
11.伤寒霍乱 呕吐 干呕 哕晼 咳逆 喘急 短气之逆
短气即气难布息，邪气业集胸中窒；气逆粗华气上冲，天下之事未可必。
霍乱呕吐并干呕，哕逆咳嗽及喘急，此章重出反三隅，活法治之无一失。
12.发黄 发斑 发狂
伤寒黄病可心惊，脾胃湿热两相蒸；温气居多抽皮包，热气侵淫残藕形。
小便不利为黄症，小便自利蓄血名；举体成黄唇四黑，热深厥深黄亦深。
甚至伤血发黄斑，乘虚拥出皮肤端；轻若疹麻蚊迹样，重则槟榔钱一般。
蚊迹只见于手足发斑，腹皮不堪观；病人狂乱见大点，鲜红后赤断为斑。
病人静定见小点。先红后黄斑迹观；总是邪气不消散，先以汗下致相搏。
最忌血斑如果实，命门龙火肾经燔；发斑速去投门类，升麻汤且此中宽。
13.痼冷 逆冷 四逆
中寒方书即痼冷，痼者固也是一等，真元触景内外因，腹痛肢冷脂唇轻。
腰腿痛重坐如冰，自汗亡阳流冷精；又名脱阳阴的萎，泄阳反本灸方灵。
厥字方中亦甚多，认真道理不差毛；厥乃尽兮逆乃冷，望闻问切看如何。
阳厥先热后然厥，经云热深厥亦深；阳厥仙厥然后热，经云直中入阴经。
他如厥冷肢体厥，总是变爻不足凭；附子理中汤酌用，再灸关元气海神。
四逆即是四肢冷，须知深浅用刀圭；邪在表时手足热，半表半里略温和。
四肢逆冷邪入里，传经节次甚分明；紫热而温由温冷，四逆汤散速搜寻。
14.谵语 掷声
谵语即是胡乱语，掷声掷冲不正声；邪气甚兮则谵语，精气奔兮则掷声。
轻则睡中呢喃句，重则醒后亦妖言；独言向壁一二语，热邪轻重已分明。
15.不眠 多睡 （气属阳则昼行，血属阴则夜行，阴阳偏乘之生不眠多眠）
卫气昼行阳似寤，阳盛阴虚主不眠；卫气夜行阴似寤，阴盛阳衰目恣瞑。
所云不眠夜不寐，所云多眠昼亦眠；伤寒得此为反候，枣仁生地把来煎。
16.渴 口干 舌胎
邪热消水变为渴，邪热聚胃口应干；杯水难救车薪火，煎耗津液岂等闲。
舌乃心苗色本红，白胎黄滑强无纹；最忌肾水克心火，舌生血刺脉尤洪。
17.鼻衄 吐血
鼻衄病根热迫血，妄行于鼻体汗塞；经云奔血者无汗，治法奔汗即无血。
若是七孔俱流血，伤寒红汗为准则；吐血热盛不须疑，引血归经真一得。
18.痓 瘈疭 战憟 肉瞤 筋惕
原夫痓症属膀胱，头项强直背反张；此是伤风感寒湿，宗筋牵引致拘挛。
目开无汗为刚痓，属阴身热葛根汤；目闭有汗为柔痓，属阴肢冷桂枝汤。
瘈疭之症热生风，风邪主动起纷飞；瘈则缩急疭伸缓，补血荣筋药带温。
战憟阴阳两相斗，乾坤摇动不聊生；邪气交争则为战，憟是邪气内争冲。
肉瞤筋惕不常有，惕者动者瞤者跳；过亡津液血枯干，筋肉失养滋为妙。
19.瘥后昏沉 食复 劳复 色复
病后昏昏非醉痴，言语颠倒脉跷蹊；似疟非疟忘饥饱，余邪尚在心胞迷。
饮食过度为食复，土弱难胜谷气时；劳动再发为劳复，缘其血气未根基。
阴阳交易为色复，邪毒授受最难医；玉茎搅痛头难举，眼目昏花事不知。
妇人玉户里急重，痛连腰腹脉弦微；伤寒瘥后为危症，理中加减酌支持。
20.热入血室
妇人伤感中风寒，头痛发热也恶寒；月信适来冲此症，身凉腰脊痛难堪。
昼则精明夜谵语，似疟非疟不一观；热入血室为名号，莫把杭州作汴看。
21.蛔蚘 狐惑
蛔蚘即入三尸虫，肠胃虚寒求食因；甚至吃人之脏腑，四肢倦怠目常眩。
闻食即吐蛔蚘症，治法酸咸药性寻。狐惑犹豫所不决，刺入肠胃刺入心。
22.拂郁懊憹
拂郁行止不如意，肢体积气由火气；懊憹喜怒不宽舒，散闷淘情求益智。
23.不仁 百合
不仁有呼为尸厥，痛痒针灸浑不知；肢体皮肉何顽梗，经络痼冷恐难医。
百合伤寒为破症，百骸受病药难聘；无复经络节次传，汗吐下法不足信。
似寒无寒不怕寒，似热无热不怕热；微食不食卧不卧，阳春一曲和皆难。
24.痰气 脚气 虚烦 伤食
痰气类寒汗恶风，憎寒吐热气冲攻；头项身体不强痛，几希界限此中分。
脚气如何亦类寒，头痛心热大便难，脚膝屈弱难移动，风寒湿热发其间。
虚烦暴燥热为魔，汗下功夫岂敢劳；不恶寒兮头不痛，伤寒真似判丝毫。
伤食似寒何以知，头眩身热恶寒时；胸满腹痛兼呕吐，莫作伤寒一样医。
25.风门方法
诸风肝胆木招来，六部浮弦疾病胎；风痱癔脾偏枯四，千枝万药此中栽。
血气中风难舞蹈，半身不遂卧形骸；瘫痪瘈疭搐搦作，遍身疼痛可哀哉。
经络中风忘痛痒，多因麻木肉皮顽；风中府兮神恍惚，昏迷不醒倒尘埃。
至于中脏肠堪断，痰迷心窍眼歪斜；牙关噤闭声音杳，痰涎壅盛喷唾花。
假道伐虢先锋将，乌药顺气扣关和；藿香正气无拘泥，续命汤成发表证。
万一余党未心服，防风通信和增减；最忌头摇并发直，汗雨如珠面若妆。
鼻舌喉拽锯鱼咽，水道不进命难存；中气形状象中风，脉状身冷不相同。
且无涎沫何难认，误作风医命难殂；更有许多相类处，须作风吹列调医。
中气者口角无涎沫，中风者口角有涎沫；中气脉沉缓，轻者用藿香正气散。
中风脉浮洪，重者用顺风匀气散。通关散 皂角一味为末吹少许入鼻内，有嚏可救，无嚏不治。盖肺气绝也。若口不开则筋先绝矣。亦不治。
乌药顺气汤
乌药顺气僵蚕姜，枳壳陈皮芎芷详；甘草麻黄和桔梗，中风先服此为良。
夫偏身麻痹表气不顺也，故治以麻黄川芎。语言涩，里气不顺也。故治以乌药陈皮枳壳。口眼歪斜面部之气不顺也。故治以白芷僵蚕，喉中气急甘草可缓。肺气上逆桔梗可也。痰之为物，寒则结滞，热则流行，佐以干姜行其滞也。此治标之剂也。
藿香正气散
藿香正气紫苏臣，大腹陈皮芷术苓；桔梗半夏甘草厚，风寒暑湿各相应。
凡治风必先理气，气顺则痰自消。故用藿香正气则清阳上升浊阴下降。天翻地覆之分定，何邪作梗耶。
续瞑汤下参附子，防风已桂麻黄使；甘草杏仁芎芍芩，不复四生反起死。
古人以此方混治中风，未详其症。予谓麻黄 杏仁汤也。仲景以之治太阳之伤寒。桂枝芍药桂枝汤也。仲景治之太阳之中风。如是言之则中风而有头痛，身热脊强者皆在所必用也。人参 甘草四君之二也。局方用之补气，芍药四物之二也。局方用以养血。如此言之。则中风而有气虚血虚也。皆在所必用者。风淫木疾故佐以防风，湿淫腹疾故佐以防己。阴淫寒疾故佐以附子。阳淫热疾故佐以黄芩。盖疾不单来。襟襟操而至，故其用药亦兼该也。
防风通圣归荆荷，国老将军翘石膏。桔梗栀芎芩芍术，麻黄芷滑起沉疴。
夫防风麻黄解表药也。风热之在皮肤也，得之由汗而泄。荆艾清上药也，风热之在巅顶也，得之由鼻而泄也。大黄芷消通利药也，风热之在阳胃也。得之由后而泄也。风热之在决渎者得之由溺而泄。风淫手鬲肺肤受邪。滑石桔梗清肺腑也。而连翘黄芩 又所以祛诸经之游火。风之为患肝木主之。川芎 当归 和肝血也。而甘草白术 又所以和胃气而健脾胃者也。
定风丹秦艽 羌活 甘草 防风 川芎 白芷 当归 白术 白芍 独活 白芍 独活 熟地 黄芩 生地 石膏 细辛。许学士云中风虚邪也。留而不去其病则实，故用驱风养血之剂，以治之。用秦艽为君者。以其主宰一身之风。石膏去胸中总司之火。羌活去太阳百节之风疼，防风为诸风为药中之军卒。三阳数变之风则有白芷 风热干乎气，清以黄芩。风热干乎血凉以生地独活。留风湿在足少阳甘草缓风邪。上逆于肺。乃当归 熟地黄者，所以养血于疏风之后。一以济风药之燥。一使手得血而能握，足得血而能步也。
26.暑门方法
暑伤于气脉应虚，伤暑伤寒脉各殊；外症与寒虽仿佛，身热无痛不拘挛。
奔劳无烈无暍热，避冷幽清中暑馀；人心包络与胃应，邪从牙齿入心居。
烦喝饮水或吐泻，香薷饮子可先驱；五苓散用兮邪正，六和汤去莫踌躇。
香薷饮
香薷饮子参陈皮，白芩厚朴术黄芪；甘草木瓜白扁豆，清心驱暑此为奇。
五苓散
五苓汤内猪茯苓，白术泽泻桂心求；燮理阴阳清暑湿，精神恍惚定安然。
六合鹰爪藿香薷，砂参甘草术苓居；扁豆木瓜杏厚朴，半夏驱暑任驰驱。
夫六和也六腑和者，脾胃六腑之总司。故凡六腑不和之症。先于脾胃而调之，此妙诀也。香能治胃窍，故用藿砂，辛能散脾胃气，故用半杏。淡能利湿热，故用茯苓。甘能调脾胃，故用扁术。补脾胃弱则脾胃复而诸疾平。盖脾一治，水精四布，五经并行，虽百骸九窍解太和也。况六腑乎。
27.湿门方法
问君何以知中湿，沾染中间不自识；飞专雨水居处卑，岂都生冷汗气积。
令人下体肉皮疮，遍身浮肿脾胃失；脉来三至半奇寒，羌活除湿汤当食。
羌活除湿也防风，苍术藁本妙无伦。升麻柴胡相鼓舞，周天风湿痛无踪。
夫湿性下流，泥泞经络，今以风药治湿，譬之卑湿之地，得太阳即干。得和风则洁。亦治之一奇。
28.火门方法
心为君火配五行，尊君离位水相生；辅弼龙火名为相，元气之贼实可憎。
乘机煽祸易忘动，侮水煎熬肾命争；实火则泻导赤散，虚火宜补撤薪丸。
各经邪火认脉症，清汤凉饮减和增；更有阴虚火动症，地黄六味各精填。
导赤散内药三般，生地木通一处欑；甘草尾稍相佐使，心惊实火刻时宽。
清凉汤用生地黄，栀子玄参翘麦门；甘草桔梗黄连入，一杯之水扑疗源。
撤薪丸：黄连 黄柏 黄芩 栀子 木通 犀角 生地 甘草 人参 五味 麦冬 柴胡。
七十二症辨治方法

1. 发热
翕翕发热（形容发热如羽毛覆盖般温和）属于表热，是风寒刚刚侵袭皮肤所致；这是表热而体内无恙，若无汗则用发汗法解表，功效迅速。
蒸蒸发热（形容发热如蒸汽外腾）属于里热，是阳邪下陷进入阴分所致；就像蒸笼热气向外散发，若有口渴则用攻下法，切勿犹豫跋涉。
又有表邪未解的症状，病邪欲传入里引发变证；若体内产生热邪难以忍受，在半表半里之间发作即为热证。
治疗方法不外乎以“和”为贵，伤寒重症不论舌象如何；无汗者用麻黄汤，有汗者用桂枝汤，必须分清太阳经的界限。
2. 潮热
潮热像潮水一样定时发作，多因阳明经病变所致；若在寅时、卯时发作属肝木，其余时辰依此类推仔细思量。
治疗采用消食导滞、疏通气机的方法，温性药物能除热，忌用寒凉药；又有劳伤引起的潮热，不同于伤寒的机制。
3. 恶寒恶热 寒热往来 似疟非疟
寒热如何表现为往来交替？这是邪气在胆经受争所致；恶寒说明邪气在表，恶热说明邪气在内。
若是邪气居于半表半里，所以寒热往来交替；日夜发作没有固定次数，传过其他经络就不再发作。
又有类似疟疾但非疟疾的症状，有时发作有时停止也有规律可循；通用小柴胡汤和解少阳，误用攻下或催吐也会使病情衰退。
4. 烦热 烦躁
感冒风寒或失于解表，变为烦热扰乱心胸；又有不同类型的烦热，需知晓阳虚与阴实的区别。
阳实导致错乱心烦热，阴虚导致愤怒肾烦躁；烦则热轻燥热甚，烦躁先后分别判断燥与烦。
5. 恶风 背恶风
伤风表现为恶风，伤寒表现为恶寒，因腠理不密导致邪气干扰；恶风多属阳气不和，不同于阴阳俱有的恶寒。
又有伤寒导致的背恶风，背部为阳会聚之处腹部为阴；令背部恶寒是阳气不足，生姜、羌活、冲和汤是必选之方。
6. 头眩 头痛 项强
伤寒头痛与头眩，不比寻常部位召唤；邪在太阳经头必痛，房劳过度只把补益作为君药。
若是病后身体疼痛，发汗亡阳变作风证；又有四肢活动不便，病前病后分别采用补泻法。
7. 痞满 结胸 胸膈满
无热恶寒发于阴经，反而招来痞满症状；按压不痛是伤寒血瘀，胸膈满闷降而无升。
发热恶寒发于阳经，误用攻下开门揖盗损伤正气；按压才痛是小结胸，不按自痛是大结胸令人堪忧。
热结胸伴有烦渴，寒结胸伴有胸闷怕冷；水饮结胸中肿胀且出汗，不要轻易误用攻下法，需另行商议。
8. 腹痛 腹满 小腹满
脾为中央戊己土，太阴疾病腹部受苦；阳毒为邪手足温暖，疼痛有停止言语纷乱。
腹满稍长如此而已，阴毒生端又不同；腹满想排便无休息，腹满兼积瘀可消分。
9. 无汗 盗汗 自汗 头汗 手足汗
寒邪伤中足三阳经，腠理固密外作殃祸；津液内渗无汗如雨，麻黄汤解表最为良效。
寒邪伤中足三阴经，腠理松疏汗自淋漓；流而不止是亡阳症，桂枝汤中可寻治法。

痰气类寒汗恶风，憎寒吐热气冲攻；头项身体不强痛，几希界限此中分。
脚气如何亦类寒，头痛心热大便难，脚膝屈弱难移动，风寒湿热发其间。
虚烦暴燥热为魔，汗下功夫岂敢劳；不恶寒兮头不痛，伤寒真似判丝毫。
伤食似寒何以知，头眩身热恶寒时；胸满腹痛兼呕吐，莫作伤寒一样医。
25. 风门方法
诸风肝胆木招来，六部浮弦疾病胎；风痱癔脾偏枯四，千枝万药此中栽。
血气中风难舞蹈，半身不遂卧形骸；瘫痪瘈疭搐搦作，遍身疼痛可哀哉。
经络中风忘痛痒，多因麻木肉皮顽；风中府兮神恍惚，昏迷不醒倒尘埃。
至于中脏肠堪断，痰迷心窍眼歪斜；牙关噤闭声音杳，痰涎壅盛喷唾花。
假道伐虢先锋将，乌药顺气扣关和；藿香正气无拘泥，续命汤成发表证。
万一余党未心服，防风通信和增减；最忌头摇并发直，汗雨如珠面若妆。
鼻舌喉拽锯鱼咽，水道不进命难存；中气形状象中风，脉状身冷不相同。
且无涎沫何难认，误作风医命难殂；更有许多相类处，须作风吹列调医。
中气者口角无涎沫，中风者口角有涎沫；中气脉沉缓，轻者用藿香正气散。
中风脉浮洪，重者用顺风匀气散。通关散 皂角一味为末吹少许入鼻内，有嚏可救，无嚏不治。盖肺气绝也。若口不开则筋先绝矣。亦不治。
乌药顺气汤
乌药顺气僵蚕姜，枳壳陈皮芎芷详；甘草麻黄和桔梗，中风先服此为良。
夫偏身麻痹表气不顺也，故治以麻黄川芎。语言涩，里气不顺也。故治以乌药陈皮枳壳。口眼歪斜面部之气不顺也。故治以白芷僵蚕，喉中气急甘草可缓。肺气上逆桔梗可也。痰之为物，寒则结滞，热则流行，佐以干姜行其滞也。此治标之剂也。
藿香正气散
藿香正气紫苏臣，大腹陈皮芷术苓；桔梗半夏甘草厚，风寒暑湿各相应。
凡治风必先理气，气顺则痰自消。故用藿香正气则清阳上升浊阴下降。天翻地覆之分定，何邪作梗耶。
续瞑汤下参附子，防风已桂麻黄使；甘草杏仁芎芍芩，不复四生反起死。
古人以此方混治中风，未详其症。予谓麻黄 杏仁汤也。仲景以之治太阳之伤寒。桂枝芍药桂枝汤也。仲景治之太阳之中风。如是言之则中风而有头痛，身热脊强者皆在所必用也。人参 甘草四君之二也。局方用之补气，芍药四物之二也。局方用以养血。如此言之。则中风而有气虚血虚也。皆在所必用者。风淫木疾故佐以防风，湿淫腹疾故佐以防己。阴淫寒疾故佐以附子。阳淫热疾故佐以黄芩。盖疾不单来。襟襟操而至，故其用药亦兼该也。
防风通圣归荆荷，国老将军翘石膏。桔梗栀芎芩芍术，麻黄芷滑起沉疴。
夫防风麻黄解表药也。风热之在皮肤也，得之由汗而泄。荆艾清上药也，风热之在巅顶也，得之由鼻而泄也。大黄芷消通利药也，风热之在阳胃也。得之由后而泄也。风热之在决渎者得之由溺而泄。风淫手鬲肺肤受邪。滑石桔梗清肺腑也。而连翘黄芩 又所以祛诸经之游火。风之为患肝木主之。川芎 当归 和肝血也。而甘草白术 又所以和胃气而健脾胃者也。
定风丹秦艽 羌活 甘草 防风 川芎 白芷 当归 白术 白芍 独活 白芍 独活 熟地 黄芩 生地 石膏 细辛。许学士云中风虚邪也。留而不去其病则实，故用驱风养血之剂，以治之。用秦艽为君者。以其主宰一身之风。石膏去胸中总司之火。羌活去太阳百节之风疼，防风为诸风为药中之军卒。三阳数变之风则有白芷 风热干乎气，清以黄芩。风热干乎血凉以生地独活。留风湿在足少阳甘草缓风邪。上逆于肺。乃当归 熟地黄者，所以养血于疏风之后。一以济风药之燥。一使手得血而能握，足得血而能步也。
26. 暑门方法
暑伤于气脉应虚，伤暑伤寒脉各殊；外症与寒虽仿佛，身热无痛不拘挛。
奔劳无烈无暍热，避冷幽清中暑馀；人心包络与胃应，邪从牙齿入心居。
烦喝饮水或吐泻，香薷饮子可先驱；五苓散用兮邪正，六和汤去莫踌躇。
香薷饮
香薷饮子参陈皮，白芩厚朴术黄芪；甘草木瓜白扁豆，清心驱暑此为奇。
五苓散
五苓汤内猪茯苓，白术泽泻桂心求；燮理阴阳清暑湿，精神恍惚定安然。
六合鹰爪藿香薷，砂参甘草术苓居；扁豆木瓜杏厚朴，半夏驱暑任驰驱。
夫六和也六腑和者，脾胃六腑之总司。故凡六腑不和之症。先于脾胃而调之，此妙诀也。香能治胃窍，故用藿砂，辛能散脾胃气，故用半杏。淡能利湿热，故用茯苓。甘能调脾胃，故用扁术。补脾胃弱则脾胃复而诸疾平。盖脾一治，水精四布，五经并行，虽百骸九窍解太和也。况六腑乎。
27. 湿门方法
问君何以知中湿，沾染中间不自识；飞专雨水居处卑，岂都生冷汗气积。
令人下体肉皮疮，遍身浮肿脾胃失；脉来三至半奇寒，羌活除湿汤当食。
羌活除湿也防风，苍术藁本妙无伦。升麻柴胡相鼓舞，周天风湿痛无踪。
夫湿性下流，泥泞经络，今以风药治湿，譬之卑湿之地，得太阳即干。得和风则洁。亦治之一奇。
28. 火门方法
心为君火配五行，尊君离位水相生；辅弼龙火名为相，元气之贼实可憎。
乘机煽祸易忘动，侮水煎熬肾命争；实火则泻导赤散，虚火宜补撤薪丸。
各经邪火认脉症，清汤凉饮减和增；更有阴虚火动症，地黄六味各精填。
导赤散内药三般，生地木通一处欑；甘草尾稍相佐使，心惊实火刻时宽。
清凉汤用生地黄，栀子玄参翘麦门；甘草桔梗黄连入，一杯之水扑疗源。
撤薪丸：黄连 黄柏 黄芩 栀子 木通 犀角 生地 甘草 人参 五味 麦冬 柴胡。
Methods for Differentiating and Treating Seventy-Two Syndromes

1. Fever
"Xi Xi" fever (fever feeling like feathers covering, mild warmth) indicates exterior heat, caused by wind-cold just invading the skin; this is exterior heat with interior health intact, if there is no sweating, using diaphoresis to release the exterior yields rapid results.
"Zheng Zheng" fever (fever rising like steam from a steamer) indicates interior heat, where yang pathogenic factors have sunk into the yin levels; similar to steam escaping from a steamer, if there is thirst, purgation should be used without hesitation.
There are also cases where exterior pathogens remain unresolved, and the disease intends to transmit inward causing complications; if internal heat arises that is intolerable, it manifests as heat in the half-exterior half-interior stage.
Treatment methods essentially revolve around "harmony"; in severe Shanghan (Cold Damage), tongue appearance is secondary; for no sweating, use Mahuang Tang (Ephedra Decoction); for sweating, use Guizhi Tang (Cinnamon Twig Decoction); one must clearly distinguish the Taiyang (Greater Yang) domain.
2. Tide-like Fever
Tide-like fever occurs regularly like the tide, often due to Yangming (Yang Brightness) meridian pathology; if it occurs during Yin and Mao hours (3-7 AM), it belongs to Liver Wood, analyze other times similarly.
Treatment employs digestion promotion and unblocking methods; warm herbs can remove heat, avoid cold/cool herbs; there is also tide-like fever caused by labor injury, which differs from Shanghan mechanisms.
3. Aversion to Cold and Heat, Alternating Chills and Fever, Malaria-like but not Malaria
How do chills and fever alternate? This is due to pathogenic struggle in the Gallbladder meridian; aversion to cold indicates pathogen in the exterior, aversion to heat indicates pathogen in the interior.
If the pathogen resides in the half-exterior half-interior, chills and fever alternate; attacks occur day and night without fixed frequency, ceasing after transmitting to other meridians.
There are also malaria-like syndromes that are not true malaria, with regular intervals of onset and cessation; Xiaochaihu Tang (Minor Bupleurum Decoction) is generally used to harmonize Shaoyang (Lesser Yang); misusing purgation or emesis will also weaken the condition.
4. Irritable Heat, Restlessness
Contracting wind-cold or failing to release the exterior transforms into irritable heat disturbing the heart/mind; different types of irritable heat require understanding the distinction between Yang deficiency and Yin excess.
Yang excess causes chaotic irritable heat, Yin deficiency causes angry restlessness in the Kidney; "Fan" (irritability) implies milder heat, "Zao" (dryness/agitation) implies severe heat; distinguish dryness and irritability by their sequence.
5. Aversion to Wind, Back Aversion to Wind
Wind-strike presents with aversion to wind, Cold-strike presents with aversion to cold, due to loose interstices allowing pathogen intrusion; aversion to wind mostly relates to disharmony of Yang Qi, differing from the aversion to cold seen in both Yin and Yang disorders.
In Cold-strike, back aversion to wind occurs; the back is the gathering place of Yang, the abdomen is Yin; back cold indicates Yang insufficiency, Ginger, Qiang Huo (Notopterygium Rhizome), and Chonghe Tang are essential choices.
6. Dizziness, Headache, Neck Stiffness
Shanghan headache and dizziness differ from ordinary locations; pathogen in the Taiyang meridian causes definite headache, while sexual exhaustion requires tonification as the primary treatment.
If body pain persists after recovery, excessive sweating leading to Yang collapse may transform into wind syndromes; for limb immobility, differentiate supplementation and draining based on pre- or post-disease state.
7. Epigastric Fullness, Chest Binding, Chest Diaphragm Fullness
No fever, aversion to cold originates from Yin meridians, yet epigastric fullness appears; non-tender upon palpation indicates Shanghan blood stasis, chest fullness with descending but not ascending Qi.
Fever and aversion to cold originate from Yang meridians, mistaken purgation opens the door to damage; tender only upon palpation is minor chest binding, spontaneous pain indicates major chest binding, highly damaging.
Heat-binding chest involves irritability and thirst, cold-binding chest involves stifling sensation and fear of cold; water-binding chest involves swelling and sweating, do not lightly misuse purgation, consult further.
8. Abdominal Pain, Abdominal Fullness, Lower Abdomen Fullness
Spleen is Central Earth, Taiyin diseases suffer the abdomen; Yang poison pathogens present with warm hands and feet, pain stops with confused speech.
Abdominal fullness lasts slightly longer thus far, Yin poison onset is different; abdominal fullness with urge to defecate without rest, fullness combined with accumulation/stasis can be resolved.
9. Anhidrosis, Night Sweats, Spontaneous Sweating, Head Sweating, Hand/Foot Sweating
Cold pathogen injuring Foot Three Yang meridians, tight interstices cause external disaster; fluids seeping internally result in no sweat, Mahuang Tang for releasing exterior is best.
Cold pathogen injuring Foot Three Yin meridians, loose interstices cause spontaneous dripping sweat; flowing endlessly indicates Yang collapse, seek cure within Guizhi Tang (Cinnamon Twig Decoction).
Night sweaks steal flow during sleep, stopping immediately upon waking leaving no trace; half-exterior half-interior pathogen lodges, Gallbladder heat treated with one cup of Xiaochaihu Tang (Minor Bupleurum Decoction).
Pathogen struggling in all Yang domains naturally causes head sweat reaching the neck; prolonged rain symbolizes Yang collapse, Huangqi Tang (Astragalus Decoction) combined with Guizhi Tang (Cinnamon Twig Decoction) gathers efficacy.
Hands and feet are branches of Spleen/Stomach, cold transforming to fire creates central heat; profuse fluid flow causes limb sweating, Qingpi Yin (Clear Spleen Drink) yields swift merit.
10. Dysentery with Pus/Blood, Difficult Defecation, Difficult Urination, Urinary Retention, Spontaneous Urination
Dysentery requires careful Yin/Yang differentiation, Water overcoming Earth is the root cause; Shanghan mistaken purgation often leads here, spontaneous passage resembles normalcy.
Three-Yang dysentery presents with whole-body heat, warm limbs belong to Taiyin; Shaoyin pulse rapid, body cold as ice, if heart is cold, rescue Jueyin.
Shanghan also includes pus/blood in stool, intestinal/stomach cold pathogens invade; turbid/clear separation fails, dampness follows meridians, unlike ordinary hemorrhage.
For other urination/defecation symptoms, seek rules within this school; Shanghan transmission is like wind/thunder, unlike miscellaneous diseases easily treating cold.
11. Shanghan Huoluan, Vomiting, Dry Heaving, Hiccups/Cough, Reverse Cough, Panting, Shortness of Breath
Shortness of breath means difficult Qi distribution, pathogenic accumulation obstructs the chest; reverse Qi rushes upward thickly, worldly matters cannot be guaranteed.
Huoluan (Stomach Intestine Turmoil), vomiting, dry heaving, hiccups, cough, and panting; this chapter repeats to trigger insight, flexible treatment ensures no error.
Shanghan jaundice is alarming, Spleen/Stomach damp-heat steams mutually; warm Qi dominates drawing skin tight, hot Qi invades resembling damaged lotus roots.
Urinary difficulty indicates Jaundice syndrome, spontaneous urination indicates Blood Stasis; whole body yellow with black lips, deep heat causes deep cold and deep jaundice.
Even blood injury causes yellow macules, emerging through skin via deficiency; light resembles rashes/mosquito bites, heavy resembles betel nut coins.
Mosquito-bite marks appear only on limbs/macules, abdominal skin is unsightly; patient manic seeing large spots, fresh red turning dark red determines macules.
Patient calm seeing small spots, first red then yellow macule traces; always pathogen not dissipating, initially sweating/purging causes struggle.
Most taboo is blood macules like fruit, Mingmen Dragon Fire burns Kidney meridian; quickly remove macules entering categories, Shengma Tang (Cimicifuga Decoction) provides relief here.
13. Chronic Cold, Reverse Cold, Four Reversals
Zhonghan (Mid-Cold) in texts is chronic cold, chronic is indeed a grade, True Yuan touches scenery/internal/external causes, abdominal pain, cold limbs, pale lips.
Waist/leg pain heavy sitting like ice, spontaneous sweating Yang collapse flows cold essence; also called Yang collapse Yin atrophy, leaking Yang returning to origin moxibustion is effective.
Jue (Reversal) characters are many in formulas, serious principles leave no hair's breadth error; Jue means extremity, Ni means cold, observe via four diagnostic methods.
Yang Jue first heat then reversal, classics say deep heat causes deep reversal; Yang Jue Xian Jue then heat, classics say direct invasion enters Yin meridians.
Others like cold limbs reversal, always changing lines insufficient reliance; Fuzi Lizhong Tang (Aconite Regulate Middle Decoction) used moderately, then moxa Guanyuan/Qihai spirits.
Four Reversals mean four limbs cold, know depth using knife/guide; pathogen in exterior hands/feet warm, half-exterior half-interior slightly mild.
Four limbs reverse cold pathogen enters interior, meridian transmission steps clear; purple heat then warm then cold, Sini Tang (Counterflow Cold Decoction) powder quickly sought.
14. Delirium, Throwing Sounds
Delirium is nonsense speech, throwing sounds are irregular noises; severe pathogen causes delirium, rushing essence causes throwing sounds.
Lightly muttering in sleep, heavily speaking妖言 (demonic words) after waking; alone speaking to wall one-two words, heat pathogen severity clear.
15. Insomnia, Hypersomnia (Qi is Yang moving by day, Blood is Yin moving by night, Yin/Yang imbalance causes insomnia/hypersomnia)
Wei Qi moves Yang by day like wakefulness, Yang excess Yin deficiency causes insomnia; Wei Qi moves Yin by night like wakefulness, Yin excess Yang deficiency causes eyes closing freely.
Insomnia means night sleeplessness, hypersomnia means day sleeping too; Shanghan obtaining this is contrary sign, Zao Ren (Semen Ziziphus Spinosae) raw decocted.
16. Thirst, Dry Mouth, Tongue Coating
Pathogenic heat consumes water becoming thirst, pathogenic heat gathers stomach mouth becomes dry; cup of water cannot save burning cartload, boiling consuming fluids is no trivial matter.
Tongue is Heart sprout color originally red, white coating yellow slippery strong no wrinkles; most taboo Kidney Water overcoming Heart Fire, tongue blood thorns pulse especially surging.
17. Epistaxis, Hematemesis
Epistaxis root is heat forcing blood, running wildly in nose body sweat blocked; classics say running blood has no sweat, treatment method running sweat means no blood.
If seven orifices all bleed, Shanghan red sweat is standard; hematemesis heat excess need not doubt, guiding blood back to meridians truly obtains.
18. Spasm, Clonic Convulsions, Shivering, Muscle Twitching, Sinew Trembling
Originally Spasm belongs to Bladder, head/neck stiff back hyperextended; this is wind-strike contracting cold-damp, tendons pulling causing contracture.
Eyes open no sweat is Gang Spasm (Rigid Spasm), belongs to Yin body heat Gegen Tang (Kudzu Decoction); eyes closed sweating is Rou Spasm (Soft Spasm), belongs to Yin limb cold Guizhi Tang (Cinnamon Twig Decoction).
Clonic convulsion syndrome heat generates wind, wind pathogen active rises flying; Ji (clenching) contracts urgent, Zong (extending) extends slow, nourishing blood enriching sinews herbs carry warmth.
Shivering Yin/Yang two fighting, Heaven/Earth shaking unable to live; pathogen struggle is shivering, Dou is pathogen internal struggle rush.
Muscle twitching/sinew trembling not common, trembling is moving, twitching is jumping; excessive loss of fluids blood dry, sinews muscles lose nourishment moistening is wonderful.
19. Post-Recovery Drowsiness, Food Relapse, Labor Relapse, Sexual Relapse
Post-illness drowsiness not drunk/confused, speech inverted pulse strange; malaria-like forgetting hunger/fullness, residual pathogen still confusing Pericardium.
Overeating is food relapse, Earth weak hard to overcome grain Qi time; labor re-onset is labor relapse, because Qi/Blood not rooted.
Yin/Yang exchange is sexual relapse, pathogen toxin transmission hardest to treat; penis stirring pain head hard to lift, eyes blurry matters unknown.
Women vulva internal urgency/heaviness, pain connecting waist/abdomen pulse string/tiny; Shanghan post-recovery is critical syndrome, Lizhong Tang modified酌情 support.
20. Heat Entering Blood Chamber
Women contracting wind-cold, headache fever also aversion to cold; menstruation just arrived clashes with this syndrome, body cool waist/spine pain unbearable.
Day bright mind night delirium, malaria-like not uniform view; heat entering blood chamber named title, do not mistake Hangzhou for Bian.
21. Roundworms, Fox-Hu Syndrome
Roundworms enter Three Corpse worms, intestinal/stomach deficiency cold seeking food cause; even eating person's organs, four limbs fatigue eyes often dizzy.
Hearing food vomiting roundworm syndrome, treatment sour/salty herb nature seek. Fox-Hu hesitation undecided, piercing intestines/stomach piercing heart.
22. Oppression and Agitation
Oppression walking/stopping unsatisfactory, limb accumulating Qi by fire Qi; agitation anger/joy not comfortable, scattering depression washing emotion seeking wisdom.
23. Numbness, Lily Disease
Numbness called Corpse Reversal, pain/itch acupuncture completely unknown; limb skin/flesh how stubborn, meridian chronic cold fear hard to treat.
Lily Shanghan is broken syndrome, hundred bones diseased medicine hard to hire; no more meridian step transmission, sweating/vomiting/purging methods insufficient trust.
Like cold no cold not fearing cold, like heat no heat not fearing heat; slight eat not eat lie not lie, Spring Song one tune harmony all hard.
24. Phlegm-Qi, Beriberi, Empty Irritability, Food Injury
Phlegm-Qi like cold sweat aversion to wind, loathing cold vomiting heat Qi attacking; head/neck body not stiff painful, barely boundary distinguished herein.
Beriberi how also like cold, headache heart heat difficult defecation, foot/knee flex weak hard move, wind/cold/damp/heat develop among them.
Empty irritability sudden dryness heat as demon, sweating/purging effort dare not labor; not aversion to cold head not painful, Shanghan truly like judging hair.
Food injury like cold how know, dizziness body heat aversion to cold time; chest full abdominal pain plus vomiting, do not treat Shanghan same way.
25. Wind Door Methods
All winds Liver/Gallbladder Wood attract, six positions floating string disease embryo; Feng Fei Yi Pi Pian Ku four, thousand branches ten thousand medicines planted herein.
Blood/Qi wind stroke hard dance, half-body paralysis lying corpse; paralysis clonic convulsions spasms, whole body pain sorrowful.
Meridian wind stroke forget pain/itch, mostly due to numbness skin stubborn; wind striking Fu organs spirit恍惚, coma not waking fall dust.
Regarding striking Zang intestines can judge, phlegm confusing heart orifice eye slanting; jaw locked voice gone, phlegm saliva abundant spitting flowers.
Borrowing road attacking Guo vanguard general, Wuyao Shunqi (Tso-Woo Regulate Qi) knocking gate harmonizing; Huoxiang Zhengqi (Agastache Rectifying Qi) no拘泥, Xuming Tang (Continuing Life) forming releasing exterior signs.
If remaining bandits not heart-subdued, Fangfeng Tongsheng (Windproof Communicating Victory) harmonize increase/decrease; most taboo head shaking hair straight, sweat rain like pearls face like makeup.
Nose/tongue throat sawing fish throat, water channel not entering life hard exist; Zhong Qi shape resembles wind stroke, pulse shape body cold different.
Moreover no saliva how hard recognize, mistakenly treating as wind medicine life hard perish; moreover many similar places, must treat as wind blowing list adjust doctor.
Zhong Qi corner of mouth no saliva, wind stroke corner of mouth has saliva; Zhong Qi pulse deep slow, light use Huoxiang Zhengqi San (Agastache Powder Rectifying Qi).
Wind stroke pulse floating surge, heavy use Shunfeng Yunqi San (Following Wind Harmonizing Qi Powder). Tongguan San (Passing Through Gate Powder) Gleditsia Sinensis (Zaojiao, 皂角) single ingredient ground blow少许 into nostril, sneezing can save, no sneezing not treat. Because Lung Qi exhausted. If mouth not opening then sinews first exhausted. Also not treat.
Wuyao Shunqi Tang (Tso-Woo Regulate Qi Decoction)
Wuyao Shunqi Jiangcan Ginger, Zhike Chenpi Xiong Zhi detailed; Gancao Mahuang and Jiegeng, wind stroke first take this good.
Half-body numbness exterior Qi not smooth, so treat with Mahuang Chuanxiong. Speech涩 interior Qi not smooth, so treat with Wuyao Chenpi Zhike. Mouth/eye slant facial Qi not smooth, so treat with Baizhi Jiangcan, throat Qi urgent Gancao can relieve. Lung Qi reversing Jiegeng can. Phlegm as substance, cold then stagnation, heat then flow, assist with Ganjiang moving stagnation. This treats branch syndrome agent.
Huoxiang Zhengqi San (Agastache Powder Rectifying Qi)
Huoxiang Zhengqi Zisu Minister, Dafu Chenpi Zhi Zhu Ling; Jiegeng Banxia Gancao Hou, wind/cold/summer/damp each correspond.
Generally treating wind first regulate Qi, Qi smooth then phlegm self-dissipates. So using Huoxiang Zhengqi then Clear Yang rises Turbid Yin descends. Heaven/earth overturning division determined, what pathogen obstructs?
Xuming Tang adds Renshen Fuzi, Fangfeng Yi Gui Mahuang envoy; Gancao Xingren Xiong Shao Qin, not recovering four births instead raising dead.
Ancients used this formula mixed treating wind stroke, not detailing syndrome. I say Mahuang Xingren Tang. Zhongjing uses it treating Taiyang Shanghan. Guizhi Shaoyao Guizhi Tang.
