Mr. Zou Yun Kun of the Chu Guan Warship, aged twenty-eight. Because he chased and retrieved a sampan in the rain during summer, he contracted Wind-Dampness, resulting in abdominal distension and leg swelling. The swelling extended downwards to both feet. Initially treated at a hospital in Shanghai, laxatives were ineffective. In September, when the warship arrived in Zhenjiang, I was invited to treat him. He presented with fever, chest oppression, yellow and greasy tongue coating, uncomfortable abdominal distension, slippery pulse, and red urine. This indicated Dampness accumulating internally with concurrent Phlegm stagnation. Initially, Banxia Xiexin Tang (Pinellia Decoction for Draining the Epigastrium), Xiao Chaihu Tang (Minor Bupleurum Decoction), and Xiao Xianxiong Tang (Minor Chest-Harmonizing Decoction) were used. After the fever subsided and the chest felt relaxed, generalized joint pain remained. Therefore, Qianghuo (Notopterygii Rhizoma et Radix), Qinjiao (Gentianae Macrophyllae Radix), Sangzhi (Mori Ramulus), and Niuxi (Achyranthis Bidentatae Radix) were added to the formula clearing and draining Damp-Heat to treat the pain. Unexpectedly, after taking this medicine, severe continuous panting erupted the next day. Upon urgent examination, indeed, he was restless from panting, exhausted, and unable to sit or rise. His pulses on both hands were fine, weak, and lacking spirit; his tongue color had turned smooth and devoid of coating; his complexion was sallow and pale. This signified that while the pathogen had receded, the Primordial Qi was severely deficient and on the verge of collapse. I urgently prescribed: Lushen Shen (Codonopsis Radix) 3 qian, Xiyang Shen (American Ginseng Radix) 3 qian, Shudi Huang (Rehmanniae Radix Preparata) 4 qian, Huangqi (Astragali Radix), Gouqizi (Lycii Fructus), and Hetaorou (Juglandis Semen) each 3 qian, Ganjiang (Zingiberis Rhizoma) 8 fen, Wuweizi (Schisandrae Chinensis Fructus) and Gancao (Glycyrrhizae Radix et Rhizoma) each 5 fen, decocted in water and taken orally. The next day, his companion Mr. [Name] returned to invite me again, saying, "Sir, you are truly miraculous! After taking yesterday's medicine, the panting stopped immediately, and his spirit calmed down; he slept peacefully through the night." I accompanied him to examine the patient, who was indeed quiet as a normal person, though still feeling shortness of breath upon rising and sitting. I advised continuing the original prescription for another dose. After this dose, the panting stabilized, but suddenly his abdomen became massively distended like a pregnant woman, with obstruction of urination and defecation. Therefore, Zisheng Wan (Vitality-Generating Pill) with Coptidis Rhizoma removed, plus Chenpi (Citri Reticulatae Pericarpium) and Muxiang (Aucklandiae Radix) were made into a decoction. One dose relieved the distension; five consecutive doses completely eliminated it. He could eat over a bowl of rice per meal and stand and walk, only complaining of soreness and weakness in the legs and feet. At that time, the Jiang-Zhe Allied Forces were attacking Nanjing, and the warship was ordered to proceed urgently to Nanjing. Therefore, Shan Zha (Crataegi Fructus) and Shenqu (Massa Medicata Fermentata) were removed from the previous formula, and Chao Shudi Tan (Charred Rehmanniae Radix Preparata), Niuxi, and Duzhong (Eucommiae Cortex) were added for him to take along. Generally, diseases involving complex deficiency and excess inevitably undergo many transformations. Physicians must respond flexibly according to the changing condition, adapting intricately to the disease. If one is slightly rigid or stuck in fixed ideas, failure is rare to avoid. Although so, how can this be explained to those with shallow understanding?
A Mrs. Surname, over forty years old, had a chronic history of leukorrhea. In autumn of the Gengxu year, she fell ill; medication was ineffective, so she sought my treatment. The patient was irritable and restless, sleepless all night, and vomited incessantly even after consuming small amounts of liquid. There was a sensation of qi movement to the left of the umbilicus, frequent leukorrhea flow, and a subjective sensation of abnormal heat, yet her body felt cool to the touch like a healthy person. Her pulse was wiry, small, and not rapid; her tongue was bright red and smooth, entirely devoid of coating. Inquiry revealed her personality was naturally irritable, and she had given birth twelve times. This indicated exhaustion of blood and yin fluids with hyperactive yang-heat. Additionally, the medications she had taken were acrid-dispersing and bitter-cold in nature, further damaging stomach qi and depleting stomach fluids, causing the spirit to lose its residence. I prescribed Huanglian Ejiao Tang (Coptis and Donkey-Hide Gelatin Decoction), adding Shashen (Glehniae Radix), Maidong (Ophiopogonis Radix), Shudi Huang, Zaoren (Ziziphi Spinosae Semen), Fushen (Poria with Hostwood), Muli (Concha Ostreae), Longchi (Os Draconis), Zhenzhumu (Margarita), Cishi (Hematitum), and Louren (Trichosanthis Semen). Huangqin (Scutellariae Radix) and Huanglian were used only in minute amounts (several fen), while Shudi Huang and Ejiao (Colla Corii Asini) were used at 3 qian. One raw egg yolk was beaten in raw and taken with the decoction. After one dose, irritability settled, and she slept peacefully. After two doses, sleep and appetite were stable, though spirit remained fatigued. Therefore, Huangqin and Huanglian were removed from the previous formula, and Congrong (Cynomorii Herba) and Gouqizi were added to tonify essence and blood. Taking several more doses resulted in recovery.
A Mrs. Wang suffered from fever, headache, and incessant vomiting. Medications containing Jingjie (Schizonepetae Herba), Fangfeng (Saposhnikoviae Radix), and Suye (Perillae Folium) were ineffective. My pulse diagnosis showed rapid pulse, thirst, thin greasy tongue coating, hot urine, and chest oppression. This indicated Summerheat-Dampness and Phlegm stagnation accumulating in the Middle Jiao, preventing the Stomach from transforming and transporting. I prescribed Jupi Zhuru Tang (Tangerine Peel and Bamboo Shavings Decoction), adding Huanglian, Banxia (Pinelliae Rhizoma), Xuanfuhua (Inulae Flos), Peilan (Eupatorii Herba), Pipaye (Loquat Leaf), Fuling (Poria), and Yiren (Coicis Semen). After taking it, she experienced a chills-and-sweat reaction (Zhanhan), after which fever ceased, vomiting stopped, and she could consume thin congee. The original formula was then administered with reduced dosage, adding Shashen and Maidong, leading to complete recovery.
The second son of Wang Shanyu, aged sixteen, suddenly developed abdominal pain and vomiting, aversion to cold, and fever. Sweating occurred when pain was severe. Tongue coating was thin and greasy; pulse was slow and slippery. I prescribed Xiaochaihu Guizhi Tang (Minor Bupleurum and Cinnamon Twig Decoction) with Renshen (Ginseng Radix) removed, adding Kouren (Amomi Fructus Rotundus) and Muxiang. After one dose, pain and vomiting ceased, and cold/fever symptoms resolved. A second consecutive dose led to full recovery.
A Mrs. Wu, in her twenties, suddenly developed vomiting, irritability, chest oppression, dizziness, and dry mouth during summer. Self-medication with Sha medicines and Shidi Water (Ten Drops Liquid) was ineffective. I prescribed Huanglian 5 fen, Wuyu (Evodiae Fructus) 2 fen, Xuanfuhua and Banxia each 1.5 qian, Xiangyuanhua (Citrodis Flower) 5 flowers, Chenpi 8 fen, Liuyisan (Six-One Powder) 2 qian, and Daizheshi (Haematitum) 3 qian. Recovery occurred after one dose.
A sixteen-year-old prostitute suffered from abdominal pain and vomiting. Several days prior, menstruation began, followed by sexual intercourse, immediately triggering abdominal pain, vomiting, diarrhea, chest oppression, and fever. She cried out in agony during pain attacks. Tongue coating was greasy; pulse was slippery. This resulted from careless diet, causing Stomach disease combined with Uterine disease. I prescribed Guizhi Tang (Cinnamon Twig Decoction), adding Houpo (Magnoliae Officinalis Cortex), Muxiang, Sharen (Amomi Fructus), Banxia, and Danggui (Angelicae Sinensis Radix). Abdominal pain lessened, but vomiting and diarrhea persisted. Body heat, thirst, hot urine, and severe lower abdominal pain during uritation appeared. Pulse became wiry, slippery, and rapid. I switched to Huanglian Tang (Coptis Decoction) with Houpo added. Abdominal pain greatly stabilized, and body heat cleared. However, copious sticky phlegm was vomited. Therefore, Guizhi, Houpo, and Banxia were removed from the original formula, and Maidong, Zhuru (Bambusa Textilis Caulis in Media Segmentata), Shenqu, Peilan, Shengjiang juice (Fresh Ginger Juice), and Laifu juice (Radish Juice) were added. After taking it, profuse sweating occurred, leading to recovery.
An eighteen-year-old female servant of Cha Yanghe consumed excessive glutinous rice zongzi during the Dragon Boat Festival, resulting in chest and diaphragmatic fullness, aversion to cold, fever, greasy tongue coating, and large slippery pulse. Initially, Pingwei San (Powder for Maintaining Stability) combined with Zhijie Tang (Immature Bitter Orange and Magnolia Flower Decoction), adding Shenqu and Gualou (Trichosanthis Fructus), was ineffective. Therefore, Untan Wan (Rolling Phlegm Pill) 3 qian was added to the formula. After taking it, two bowel movements occurred, and the chest/diaphragm opened. Subsequently, Untan Wan was removed from the original formula, combined with Xiao Xianxiong Tang, and taken for two consecutive doses, resulting in complete recovery.
Mr. Huang Huanwen suffered from Damp-Warmth disease, which I had already cured. Shortly after, due to overeating chicken, meat noodles, lotus seeds, etc., he developed chest fullness and discomfort, fever, dry mouth, and dry greasy tongue coating. Zhijie Tang combined with Xiao Xianxiong Tang, adding Shenqu, was made into a decoction, and he was instructed to first take Untan Wan 3 qian. After taking it, he first had a bowel movement, followed by profuse sweating soaking his clothes. This indicated residual pathogenic qi from the previous illness had not fully cleared, and food stagnation was not the sole cause. From then on, fever subsided, chest felt relaxed, hunger returned, and appetite improved. Liu Junzi Tang (Six Gentlemen Decoction) with Maidong and Yiren was administered for two consecutive days, leading to recovery.
Mr. Guo, over sixty years old, suffered from hernia disease in December. The root of the external genitalia was swollen and hard like a chicken egg, with extreme pain, aversion to cold without fever, dry mouth, smooth tongue without coating, and non-red complexion. This was Cold Hernia. The hardness like a chicken egg indicated Cold-Evil knotting, which dissipates with warmth. I prescribed Wutou Guizhi Tang (Aconite and Cinnamon Twig Decoction): Honey-fried Wutou (Aconiti Lateralis Radix Preparata) 3 qian, Guizhi (Cinnamomi Ramulus) and Baishao (Paeoniae Radix Alba) each 2 qian, Gancao 1 qian, plus Dangshen (Codonopsis Radix) 2 qian, Ganjiang 8 fen, Xiaohuixiang (Foeniculi Fructus) and Danggui each 3 qian, Muxiang 1 qian, made into a decoction. Pain stabilized by night, swelling and hardness subsided, and dry mouth ceased. The next day, the original formula was decocted using mutton soup, and he was instructed to stew and eat mutton, leading to recovery.
Long Yaonan, over fifty years old, had a chronic hernia disease that came and went. In winter of the Xinhai year, the disease recurred, but differently from before. He felt qi rushing straight from below the umbilicus to the heart, causing splitting heart pain, cold sweat on hands, and inability to support himself. Inhaling opium provided temporary relief but did not help the disease. Initially, episodes occurred every one or two days; later, they occurred daily without end. Spirit was fatigued, appetite significantly decreased, pulses were wiry and small, and there was white coating in the center of the tongue. This was Ben Tun Disease (Running Piglet). It resulted from inherent Kidney Qi deficiency compounded by exogenous Cold. Internal Yang Qi could not overcome the invading Cold, causing upward rush and pain. As ancients said, this is "Kidney Qi assaulting the Heart." I prescribed Guizhi Jia Gui Tang (Cinnamon Twig Decoction with Added Cinnamon Twig), plus Shudi Huang, Lujiaojiao (Colla Cornus Cervi), and Xiaohuixiang. After two doses, pain greatly retreated. Two days later, weather grew colder, and the disease recurred with vomiting. I switched to Lizhong Tang (Middle-Warming Decoction) with Rougui (Cinnamomi Cortex), Wuzhuyu (Evodiae Fructus), Banxia, Lujiaojiao, and Chenxiang (Aquilariae Lignum Resinatum). Three consecutive doses brought complete peace.
A Mrs. Li, about forty years old, had missed menstruation for two months, suffered from vomiting and inability to eat, vomiting everything including tea and water upon entry, with slight aversion to cold and fever. My pulse diagnosis showed slow, slippery, and spirited pulse. I told her, "It is pregnancy." The family was half-believing, half-doubting, anxious to stop vomiting. Repeated medication failed to stop vomiting. Other doctors were consulted, opinions varied, prescriptions differed. Days passed, vomiting continued, emaciation increased. One month later, the family invited me again. Entering the room, the patient was recovering from convulsions/fainting, hands tightly clenched, knees curled, sallow and thin complexion. Family had discussed funeral arrangements. I said, "Though thin and weak, convulsions are alarming, but the pulse is slow, slippery, and vibrant—this is not disease, but pregnancy." I advised against medication, suggesting only rice soup and chicken/duck broth. Pregnancy nausea (E Zu) can last sixty or eighty days before resolving; one should not recklessly use drugs. Over a month later, her nephew visited for consultation; she had gradually improved and could eat slightly. By December, her son-in-law brought consultation fees; she was eating and walking normally. In March of this year, she indeed gave birth to a daughter. The *Jin Gui Yao Lue* warns against treating pregnancy nausea with harsh drugs. I never expected to see this principle validated today.
Seven-year-old son of Mr. Sun suffered from fever, aversion to cold, cough, fatigue, and reduced appetite in December. Pulse was slow and not rapid. Initially, Congchi Tang (Green Onion and Fermented Soybean Decoction) with Bohe (Menthae Haplocalycis Herba), Jiegeng (Platycodonis Radix), Xingren (Armeniacae Semen Amarum), and Gancao was used. After taking it, sand points (sha rash) appeared on neck, chest, and back, faintly visible under the skin, not yet fully erupted. Original formula continued. On the third day, sha rash fully erupted, densely distributed on chest, back, neck, arms, bright red. Nighttime fever intensified with thirst. Formula changed to Sangye (Mori Folium), Jinyinhua (Lonicerae Japonicae Flos), Xingren, Yimucao (Leonuri Herba), Huafen (Trichosanthis Radix), Beimu (Fritillariae Cirrhosae Bulbus), and Gancao. On the fourth day, fever persisted, tongue bright red with prickles, no coating. Formula changed to Digupi (Lycii Cortex), Sheng Dihuang (Rehmanniae Radix) each 3 qian, Maidong 2 qian, Beishashen (Glehniae Radix) 1 qian, Baimaogen (Imperatae Rhizoma) 3 qian, Beimu 1 qian, Pipaye one slice. After one dose, fever subsided, spirit calmed, tongue became pale without prickles. One more dose led to full recovery.
Mr. Yang, nearing thirty, had a chronic history of hematemesis triggered by exertion. In May this year, due to labor and anger, bleeding recurred, vomiting a bowlful of blood, with fever and cough. Initial medical treatment had slight effect, then massive uncontrollable bleeding ensued despite medication. Relative Mr. Wang invited me. Condition: Massive hematemesis daily at 4 AM, cough with phlegm, irritability, thirst desiring cold water, subjective burning in chest/upper abdomen, preference for covering chest with cold wet towel, hunger present, appetite intact, thin greasy slightly yellow tongue coating, rapid but not large pulses on both hands, emaciated appearance. I stated this was latent Summerheat-Heat in Lung and Stomach, forcing blood to bleed recklessly. To stop bleeding, one must first descend heat; once heat descends, blood returns to its vessels and stops spontaneously. I prescribed Yuni Jian (Jade Woman Decoction) combined with Qingzao Jiufei Tang (Clear Dryness and Rescue Lung Decoction): Sheng Shigao (Gypsum Fibrosum) 4 qian, Sangye 1 qian, Sheng Dihuang 4 qian, Ejiao 3 qian, Beimu, Maidong, Shashen each 2 qian, Xingren 1 qian, Pipaye one slice. After taking it, unusual cooling sensation, borborygmus, empty feeling in heart, slight reduction in body heat. No bleeding at 4 AM; bleeding started in afternoon, with much cough and phlegm. Original formula continued, adding Zhuye (Phyllostachys nigra Folium) 3 qian, Gualougen (Trichosanthis Radix) 2 qian, Zaoren and Baiziren (Biota Seed) each 4 qian. Two consecutive doses nearly stopped bleeding. Only spirit fatigue and occasional cold sweat remained; pulse was large and powerless, tongue devoid of coating. Heat had receded, but Yuan Qi was deficient. Moreover, prolonged hematemesis meant significant blood loss; deficiency was inevitable. Formula changed to Shengmai San (Generate Pulse Powder) with Huangqi, Shudi Huang, Gouqizi, Zaoren, and Ejiao. Two consecutive doses reduced sweating, allowed consumption of two large bowls of congee. Only cough with blood-streaked phlegm remained, occasionally vomiting one or two mouthfuls when coughing severely, but vastly different from previous torrential bleeding. Formula adjusted using Baihe Gujin Tang (Lily Metal-Solidifying Decoction) combined with Qianjin Weijing Tang (Thousand Gold Reed Shoot Decoction). Within days, able to get up and walk, appetite greatly improved. Dietary nourishment supplemented with Qiongyu Gao (Jade Essence Paste) led to recovery.
My father suffered from lung disease since youth, with cough and hemoptysis, requiring several doses of Xiebai San (Drain White Powder) plus Beimu, Shanzhi (Gardeniae Fructus), and Maidong to recover. Recurrence followed exertion or anger, though sometimes years passed without episode. In summer of Dingwei year, exposure to summer heat caused fever and recurrence of old disease, worse than before. One night, felt substance overflowing in throat, assumed it was phlegm, spat several mouthfuls, but under lamplight, saw it was all blood. Hemoptysis continued, pure blood or mixed with phlegm. Spirit fatigued, bedridden. Medications including Ejiao, Dihuang, Maidong, Beimu, Pipaye had slight effect; appetite slightly improved, complexion normal, no fever, night sweats, or thirst, pulse calm. Continued previous formula. Unexpectedly, next night, blood surged up, vomiting several mouthfuls, followed by fainting, inability to speak, near death. Urgently administered Lu Shen Shen 5 qian and Xiyang Shen 5 qian decocted in water. Minutes after taking ginseng soup, speech resumed, describing palpitations, whole-body tremors, very low voice. This indicated severe Yuan Qi deficiency impending collapse. Continued ginseng soup combined with Ejiao, Shudi Huang, Zaoren, Gouqizi decocted, plus pork trotter soup. Nourishing care for ten-plus days gradually improved condition. Appetite became excellent, needing six to seven meals daily; hunger if delayed. Preferred pork trotters, sea cucumber, eggs, congee, rice; could only tolerate these nourishing items. Vegetables, radishes, tofu milk caused irritability and hunger. After blood loss, stomach fluids were depleted; only animal fats/gelatin could replenish them. After one month, spirit recovered, able to diagnose patients, but mental effort caused hand tremors and blurred vision. Walking limited to half a li; farther impossible. This is the summary of my notes attending my father's illness upon returning home from Suzhou in Dingwei year. For three years thereafter, disease did not flare severely, energy healthier than before. In July of Xinhai year, intense heat, combined with diagnostic work fatigue, caused recurrence: cough, hemoptysis, fever, dry mouth. Clear-nourishing medicines provided slight improvement, but spirit remained very weak. In September, during the Wuchang Revolution, residents of our Yang area fled en masse, leaving nine out of ten houses empty. Daily news alarmed and worried my father, causing disease to flare severely again. Taking medication for ten days, bleeding decreased, sleep slightly improved. Henceforth, Liangyi Gao (Two Polarity Paste) and Jiling Gao (Auspicious Spirit Paste) were combined into paste form, taken for over a month, hemoptysis completely stopped, spirit largely recovered, only slight cough remained. Total consumption: approx. 1 jin of Dangshen, several taels of Xiyang Shen, approx. 1 jin of Gouqizi, 2 jin of Shudi Huang, several taels each of Sheng Dihuang, Maidong, and Ejiao. Nearly three years have passed since then; disease has not recurred, spirit vigorous, running day and night to treat others. Alas, is the merit of medicine not great? Compiling this book, I record these outlines here, awaiting correction from experts.
Mr. Cao Yunsheng's younger brother, Mr. Ru, over thirty, chronically suffered from lung disease and headaches. Episodes triggered by exertion, anger, or consuming sticky, rich, sweet foods. Symptoms included headache, blurred vision, cough with sound like a water fowl in throat, chest oppression, lack of appetite, thin greasy tongue coating, slippery pulse at Cun and Guan positions. Due to multiple childbirths, brain/nerves weakened, plus opium smoking and preference for rich/sticky foods, causing Phlegm stagnation obstruction. Each time, Sangye, Hangju (Chrysanthemi Flos), Bohe, Xingren, Beimu, Jiegeng, Qianhu (Peucedani Radix), and Chenpi proved effective. This formula worked for years. Now aware of harm from rich/sticky foods, dietary changes led to milder attacks, no longer as severe as before.
Son of Mr. Yao, six years old. In June, suffered from dysentery: tenesmus, ceaseless day and night, fever, thirst, yellow greasy tongue coating, slippery rapid pulses. Xianglian Wan (Incense-Coptis Pill) with Huangqin, Zhike (Aurantii Fructus Immaturus), Binglang (Arecae Semen), Yiren, and Liuyisan made into decoction. Huanglian used only 4 fen, other herbs 1-2 qian. One dose halved dysentery; two doses brought complete peace.
Mr. Zhang Xiaofen suffered from dysentery: diarrhea, abdominal pain, severe tenesmis, distress. Pulse slippery rapid. Xiao Chengqi Tang (Minor Order the Qi Decoction) combined with Xianglian Wan, adding Binglang and Muxiang. Pain and dysentery ceased after taking, but transformed into fever and chest oppression, indicating interior qi unblocked and residual pathogen resolving via exterior. Formula changed to Xiao Xianxiong Tang combined with Xiao Chaihu Tang, removing Renshen and Gancao, adding Zhike, Jiegeng, and Houpo. Resolved with sweating. Abdominal distension and anorexia remained, white greasy tongue coating appeared. Formula changed to Pingwei San with Huangqin, Sugeng (Perillae Stem), Kouren, Peilan, and Yiren. Two doses led to recovery.
Son of Mr. Xia, four years old. Dysentery with red and white stool, abdominal distension, no body heat, no thirst, pale tongue without coating, slow slippery pulse. Pingwei San with Baizhu (Atractylodis Macrocephalae Rhizoma), Muxiang, Sharen, and Bidou (Dolichoris Semen) administered. Two consecutive doses cured. Caused by excessive consumption of cold/raw foods, impairing intestinal and stomach transformation.
Son of Mr. Yin, four years old. Dysentery for many days, cold extremities, occasional cold sweat, small weak pulse, fatigued spirit, no thirst, no tongue coating. This was not dysentery, but Yang deficiency impending collapse. Without warming, death would ensue. Lizhong Tang with Renshen and Baizhu each 3 qian, Ganjiang 8 fen, Gancao 8 fen, plus Fresh Ginger and Red Dates decocted. One dose stopped sweating, warmed hands, reduced diarrhea.
