When blood stasis fails to dissipate due to falls or sprains, or when congealed blood fails to circulate due to melancholy, or when accumulated blood cannot reach its destination due to wind pathogenic factors, the condition may be neglected over time. The blood remains in the abdomen, eventually leading to Blood Distension (a disease characterized by abdominal distension and internal blood stasis). Food and drink entering the stomach fail to transform into essence and blood; instead, they assist the pathogenic factors. Over time, this leads to bloating, and severe bloating develops into Distension syndrome.
Prescription: Shuizhi (Hirudo, leech) 3 qian (stir-fried until black. Approximately 1 liang of Hirudo is stir-fried until black, then powdered, and 3 qian of the powder is used); Danggui (Angelicae Sinensis Radix) 2 liang; Leiwan (Ovum Ascaris), Honghua (Carthami Flos), Zhishi (Aurantii Fructus Immaturus), Baishao (Paeoniae Radix Alba), and Niuxi (Achyranthis Bidentatae Radix) each 3 qian; Taoren (Persicae Semen) 40 grains (skin and tips removed, crushed). Decoct and take orally. After taking the medicine, more than one dou of blood will be discharged. Take again; once the blood is completely expelled, the disease will heal spontaneously.
Commentary by Gengsheng: The inclusion of Shuizhi (Hirudo) in this prescription is considered too potent and harsh. Moreover, although this creature is ground, it can revive upon contact with water. Patients suffering from Distension have deficient righteous qi and weak zang-fu organs. If this causes harm, would it not be a major disaster? It would be more appropriate to substitute Ye Mingsha (Stercoris Vespertilionis, bat guano). Mosquitoes suck blood just as effectively as leeches, and Ye Mingsha is formed after mosquitoes are consumed. Since Bencao (Materia Medica) states that Ye Mingsha can expel dead fetuses, its ability to attack accumulated blood is evident.
