Guan-Ge (Obstruction and Repletion) must be treated with emesis to lift the qi that is horizontally obstructed; it is not necessary to expel phlegm. If there is phlegm suitable for emesis, Er Chen Tang (Two Aged Decoction) should be used to induce vomiting; within the vomiting process, descending action naturally occurs. If it is due to deficiency of Middle Qi failing to transport, ascending and descending herbs should be added to Qi-tonifying medicines. The presentation involves Cold in the Upper Jiao and Heat in the Lower Jiao, with both Cun (inch) pulses on the two hands being four times larger than normal.
Dai Yuanli stated: Guan-Ge refers to a sensation of obstruction in the chest and diaphragm, where one wishes to ascend but cannot, wishes to descend but cannot, wishes to eat but cannot. This is called horizontal obstruction of Qi.
Lin Syndrome (Strangury) has five types, all belonging to Heat. Treatment should involve clearing Heat and promoting urination, using herbs such as Gardenia Fruit (Shan Zhi Zi). One he of peeled Gardenia Fruit should be taken with warm water. Lin Syndrome presents as dribbling urine, wanting to pass but unable, unable yet recurring, all belonging to Heat.
Formula: For elderly patients suffering from Lin due to Qi deficiency.
Ren Shen (Ginseng Radix), Bai Zhu (Atractylodis Rhizoma), Mu Tong (Akebiae Caulis), Shan Zhi Zi (Gardeniae Fructus).
Di Sui Tang (Earth Marrow Decoction): Treats Lin caused by fixed blood (blood stasis), with unbearable pain. This condition can also damage Stomach Qi, leading to loss of appetite.
Du Niu Xi (Achyranthis Bidentatae Radix) (one he).
Decoct the above with five zhong of water until four parts are consumed and one part remains. Strain, add a small amount of Moschus (She Xiang). Take on an empty stomach. It may also be decocted solely with wine, which is effective, and is also known as Ku Zhang San (Bitter Staff Powder). For elderly patients with Deficiency-Cold constitution, Ba Wei Wan (Eight-Ingredient Pill with Rehmannia) or Liu Wei Di Huang Wan (Six-Ingredient Pill with Rehmannia) are essential medicines.
Another formula: For Lin caused by Qi deficiency.
Ba Wu Tang (Eight-Ingredient Decoction) plus Huang Qi (Astragali Radix), Hu Zhang (Polygoni Cuspidati Rhizoma et Radix), Gan Cao (Glycyrrhizae Radix et Rhizoma) decocted in water and taken. Niu Xi (Achyranthis Bidentatae Radix) may be added to these herbs.
[Appendix] The onset of various Lin syndromes is due to Kidney Deficiency and Heat generated in the Bladder. Water and Fire do not interact; Heart and Kidney Qi become stagnant, causing Yin and Yang to deviate from harmony, and clear and turbid elements to mix. Accumulation occurs in the Lower Jiao, causing urgency in the Bladder interior, so semen-like fluid, blood, and sand/stones are discharged through the urinary tract. Thus, there is a state of wanting to pass but unable, with continuous dribbling. In severe cases, blockage causes suffocation and fainting. Generally, if Small Intestine has Qi stagnation, there is urinary distension; if Blood, there is urinary difficulty; if Heat, there is urinary pain. Pain indicates Blood Lin; no pain indicates Hematuria. Congealed essence forms Sand; scattered essence forms Semen-like Lin; mineral stones form Stone Lin; urinary difficulty with residual drops indicates Qi Lin. Tracing back to the origin, each follows its category. The method of prescribing involves circulating stagnant Qi, draining and benefiting urination, and clearing/dissipating pathogenic Heat. Among these, regulating and calming Heart Fire is the guideline. When Heart is clear, urination flows freely; when Heart is calm, blood does not move recklessly. It is most inadvisable to use Qi-tonifying herbs; tonifying Qi worsens distension, tonifying Blood worsens stasis, and tonifying Heat exacerbates it. If the water passages do not flow, compounded by blocked grain passages, survival is unlikely. Although, if Kidney Qi is weak and the scrotum/Bladder receives Cold, there may be Cold-complicated urinary dribbling and difficulty, presenting first with chills followed by urination. This is a struggle between Cold Qi and Healthy Qi; when Cold Qi is strong, chills occur and Lin develops; when Healthy Qi is strong, chills resolve and urination occurs. There is also non-patency due to torsion of the bladder ligaments, which must not be ignored. Sexual intercourse with urine retention causes Water Qi to reverse upward; Qi presses against the Bladder, causing torsion and inability to expand. If the Bladder ruptures, death ensues.
Lin and Bi (Retention), in ancient formulas, are called Long (Stagnation). Long means fatigue/inability. Non-patency is Long; lack of control is Yi (Enuresis); dribbling and painful urination is Lin; urgent fullness and non-patency is Bi. Wuling San (Five-Ingredient Powder with Poria) and Deng Xin Tang (Juncus Decoction) should be taken together. If there is distension below the umbilicus, adding 1 qian of Polychroite (Hu Po) powder is very effective.
For Lin patients who cannot be unblocked by various draining/patency-promoting drugs, some recover using Mu Xiang Liu Qi Yin (Costus Flowing Qi Decoction) or other aromatic Qi-unblocking agents. This is Qi Lin, distinct from Cold and Hot Lin. For Blood Lin, observe the blood color to distinguish Cold and Heat. Bright red indicates Excess Heat in the Heart and Small Intestine; dark/clotted indicates Deficiency-Cold in the Kidney and Bladder. If it is indeed Cold Lin and Lower Yuan Deficiency-Cold with dark blood, Han Jiao Gen (Zanthoxyli Radix) should be chopped, boiled in plain water regardless of quantity, and taken cold after cooling. If extreme Heat causes Lin and medication is ineffective, Jian Gui Wu Ling San (Wuling San without Cinnamon Twig) plus Mu Tong, Hua Shi (Talcum), Deng Xin, Qu Mai (Polygalae Herba) in small amounts, taken with honey-water, is appropriate.
