The treatment of hemorrhoids primarily focuses on cooling the blood.
Internal Decoction:
Renshen (Ginseng Radix), Huangqin (Scutellariae Radix), Shengdihuang (Rehmanniae Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma), Danggui (Angelicae Sinensis Radix) [harmonizes blood], Shengma (Cimicifugae Rhizoma), Tiaoqin (Scutellariae Radix, specific variety), Zhike (Aurantii Fructus) [widens intestines and regulates qi], Huaijiao (Sophorae Tonkinensis Fructus) [cools blood and generates blood].
Another prescription does not contain Huanglian (Coptidis Rhizoma).
Fumigation and Washing:
Wubeizi (Galae Chinensis), Puxiao (Natrii Sulfas), Sangjisheng (Taxilli Herba), Lianfang (Nelumbinis Nuciferae Calyx) [Jingjie (Schizonepetae Herba) can also be added].
Decoct in water, fumigate first, then wash. Dongguateng (Benincasae Caulis) is also good. Additionally, for heat-swelling of the large intestine, use Mubiezi (Semen Momordicae) and Wubeizi ground into fine powder, mixed with liquid and applied topically.
If the hemorrhoidal prolapse points upward, it indicates excessive heat in the large intestine causing contraction and upward movement. Treat with Siwu Tang (Four Substances Decoction) to resolve toxicity, adding Zhike (Aurantii Fructus), Baizhu (Atractylodis Macrocephalae Rhizoma), Huaijiao (Sophorae Tonkinensis Fructus), and Qinjiao (Gentianae Macrophyllae Radix).
[Appendix] Hemorrhoids arise from the inherent deficiency of the zang-fu organs, external invasion of wind-dampness, internal accumulation of heat-toxin, sexual intercourse after heavy eating or drinking, and excessive desire damaging the body. Consequently, qi and blood sink downward, knotting at the anus, where stagnation remains unresolved, leading to conflict and the formation of hemorrhoids. Manifestations include: flesh nodules protruding from the anal margin, resembling mouse teats, dripping pus and blood intermittently, called Mu Zhi (Yang Hemorrhoid); sores swelling and painful at the anal margin, protruding one mass, discharging pus and dissipating after a few days, called Pin Zhi (Yin Hemorrhoid); large nodules protruding from the intestinal opening with bleeding discharge, called Mai Zhi (Vascular Hemorrhoid); hard masses inside the intestine with blood, alternating chills and fever, rectal prolapse during defecation, called Chang Zhi (Intestinal Hemorrhoid). If Xue Zhi (Blood Hemorrhoid), clear blood flows continuously with every bowel movement; if Jiu Zhi (Alcohol Hemorrhoid), sores swell and bleed painfully upon drinking alcohol; if Qi Zhi (Qi Hemorrhoid), pain and swelling appear immediately upon worry, fear, depression, anger, or straining during defecation, with difficult bowel movements and irreducible prolapse upon forceful pushing. These are the external signs of various hemorrhoids. The general principle of treatment is to clear heat, regulate blood, and smooth qi first. Heat injures blood; injured blood causes meridian stagnation; stagnant meridians impede qi circulation; when both qi and blood stagnate, they sink into the large intestine due to deficiency, which is why hemorrhoids form. Untreated hemorrhoids for a long time will inevitably perforate and become fistulas.
