Zilin (Stranguria during Pregnancy) refers to the symptom of scanty and difficult urination during pregnancy, caused by deficient-heat in the Kidney and Bladder failing to manage water metabolism. However, since the Bao (uterus) is connected to the Kidney in pregnant women, deficient-heat within the Kidney gives rise to this syndrome. In severe cases, it manifests as mental restlessness and oppression. Use the following prescription:
Zilin San (Subling Dispersion)
Treats stranguria with pain and frequency during pregnancy.
Ophiopogonis Radix (Ouyidong, 麦门冬) [removed heart], Poria Cocos (Chifuling, 赤茯苓), Akebiae Caulis (Mutong, 木通), Arecae Pericarpium (Dafupi, 大腹皮) [washed to remove sand, stir-fried with ginger juice], Glycyrrhizae Radix et Rhizoma (Gancao, 甘草), Lophatheri Herba (Danzhuye, 淡竹叶).
The above ingredients are decocted in water and taken orally.
Cheqian San (Plantago Seed Dispersion)
Treats dribbling or obstruction of urine with heat in the Lower Jiao.
Plantaginis Semen (Cheqianzi, 车前子), Angelicae Sinensis Radix (Danggui, 当归), Citri Reticulatae Pericarpium (Chenpi, 陈皮), Paeoniae Radix Rubra (Chishaoyao, 赤芍药), Areca Semen (Binglang, 槟榔), Poria Cocos (Chifuling, 赤茯苓), Talcum (Huashi, 滑石), Akebiae Caulis (Mutong, 木通), Pyrosiae Frond (Shiwei, 石苇) [de-haired and roasted].
The above ingredients are decocted in water and taken orally.
Zhuanbao (Displacement of the Uterus/Bladder) refers to the sudden inability to urinate during pregnancy. This occurs because the growing fetus presses against the Bao (uterus/bladder), causing frequent urges to void without relief. Here, "Bao" refers to the Bladder. Although Zilin and Zhuanbao are similar, they differ as follows: frequent, scanty, and painful urination characterizes Zilin; frequent, scanty, but painless urination characterizes Zhuanbao; even if there is slight pain, it remains distinct from stranguria. Both conditions are suitable for treatment with Wuling San (Five-Ingredient Powder with Poria) plus Ejiao (Donkey-hide Gelatin).
Dongkui Zi San (Malva Seed Dispersion)
Treats urinary obstruction due to Zhuanbao in pregnant women.
Malvae Semen (Dongkuizi, 冬葵子), Talcum (Huashi, 滑石), Gardeniae Fructus (Zhizi, 栀子) [roasted], Akebiae Caulis (Mutong, 木通) [each 5 qian].
One dose is decocted in 1.5 zhong of water until reduced to 1 zhong, and taken warm on an empty stomach. This medicine has a slippery effect that may induce abortion; it can be used near term. It must not be used before the sixth or seventh month.
Another method: Powder Dongkui Zi, Huashi, and Zhizi. Mix with paste made from fresh apple snail meat or raw green onion juice, and apply to the umbilicus. Urination will clear quickly.
Vaginal bleeding during pregnancy is caused by deficiency of Qi in the Chong and Ren vessels, failing to restrain blood. Since the Heart and Small Intestine meridians are exterior-interior partners, fluids transform into milk upwards and menstruation downwards. Therefore, during pregnancy, menstrual blood should be retained to nourish the fetus and stored to become milk later. If vaginal bleeding occurs intermittently, it is called Tailou (Fetal Leakage); if the blood is exhausted, death ensues. This pattern belongs to Qi and Blood deficiency with internal heat. Use the following prescription:
Jiao'ai Siwu Tang (Ejiao and Four Substances Decoction)
Angelicae Sinensis Radix (Danggui, 当归), Scutellariae Radix (Huangqin, 黄芩) [stir-fried with wine], Paeoniae Radix Alba (Baishao, 白芍) [stir-fried with wine], Atractylodis Macrocephalae Rhizoma (Baizhu, 白术) [stir-fried with soil), Rehmanniae Radix Preparata (Shudihuang, 熟地黄) [stir-fried with ginger juice], Artemisiae Argyi Folium (Aiye, 艾叶) [a small amount], Amomi Fructus (Sharen, 砂仁) [roasted], Cyperi Rhizoma (Xiangfu, 香附) [stir-fried black with child's urine], True Ejiao (Donkey-hide Gelatin) [fried with clam powder into pearls].
One dose is decocted together with Oryza Sativa Germ (Jingmi, 粳米) and taken orally.
Xiong Gui Tang (Chuanxiong and Angelica Decoction) [Transmitted by Liu Min'an]
Treats persistent bleeding from Tailou, or abdominal distension and fullness; one dose brings immediate relief.
Angelicae Sinensis Radix Caudex (Dangguiwei, 当归尾), Ligusticum Chuanxiong Rhizoma (Nanchuanxiong, 南川芎) [each 5 qian].
One dose is decocted with yellow wine, taken before sleep, with one zhan of child's urine added; bleeding stops immediately.
