There was a man surnamed Zhang in the neighborhood. He contracted Shanghan (Cold Damage). The physician treated him with diaphoresis (sweating method). Although sweat was produced, his body heat remained as before. Upon examination, I said: After sweating, one should expect the body to cool, the pulse to become calm, and an appetite to return. Now, the pulse is rapid and restless, the body is hot, and he cannot eat or speak coherently. This disease is named Yin-Yang Jiao (Yin-Yang Intertwining), which is incurable. In the *Suwen* (Basic Questions), the Yellow Emperor asked about warm diseases where fever recurs immediately after sweating, accompanied by a rapid pulse that does not subside with sweating, delirium, and anorexia. What is this disease called? Qibo replied: This disease is named Yin-Yang Jiao; "Jiao" implies death. The reason humans sweat is that it originates from grain (food essence), which in turn generates Jing (Essence). When pathogenic factors contend with healthy Qi in the muscles and bones, and sweat is produced, it indicates that the pathogen is retreating and the Essence is victorious. If the Essence is victorious, the patient will be able to eat and fever will not recur. Sweat is transformed from Essence. Now, if sweating is followed by recurrent fever, it means the pathogen is prevailing. An inability to eat means there is no assistance for the Essence. His lifespan will end abruptly. Indeed, he died half a day later.
In the year Jisi, my neighbor Wang Yousheng, whose profession was trading in the capital, kept a maid who suffered from Shanghan. She had been feverish for eight or nine days. I treated her, and she recovered after sweating. Not long after, she fell ill herself. Her body was hot, her head felt heavy and she did not wish to lift it, and she saw sparks in her eyes. I summoned me to examine her. I said: This must have been contracted through sexual intercourse shortly after recovering from Shanghan. It is definitely the syndrome of Yin-Yang Yi (Yin-Yang Transmission). The external genitalia must be swollen, and there must be severe colic pain in the lower abdomen. Wang Yousheng said: Regarding the previously ill maid, I thought she was cured, so I engaged in intercourse with her. The next day, she developed this illness, suffering greatly. I said: If left untreated, she will die with her tongue protruding several inches. I prescribed formulas such as Shufen Powder (Mouse Feces Powder) to promote the discharge of toxic vapors. She recovered within ten days.
In the sixth month of the year Yisi, Merchant Tan from Jishui, residing in the south of the city, contracted Shanghan. After eight or nine days, he felt palpitations and anxiety in the chest, holding his heart with both hands, while his body trembled and shook. Other physicians treated him for Heart Pain without effect. I said: This is caused by excessive sweating. Zhang Zhongjing stated: When taking the pulse, if the patient crosses their hands over their heart, experiencing palpitations in the epigastrium, it is because repeated sweating has led to deficiency. Furthermore, it is stated: Excessive sweating leading to the patient crossing hands over the heart, experiencing palpitations in the epigastrium, and desiring pressure, constitutes the Guizhi Gancao Tang (Cinnamon Twig and Licorice Decoction) syndrome. I administered Huangqi Jianzhong Tang (Astragalus Middle-Jian Replenishing Decoction), Zhenwu Tang (True Warrior Decoction), and a combination of Gancao (Licorice) and Guizhi (Cinnamon Twig). Gradually, he recovered.
