When pathogenic heat transmits inward, if it has not undergone sweating, vomiting, or purging treatments, "fanre" (restless heat) will occur. This appears similar to fever but is essentially different. The Classics state: "When patients suffer from fanre, relief comes with sweating." If diaphragmatic heat has not yet formed, one should only employ mild harmonizing methods to resolve the slight heat. However, if there is fullness and vexation in the epigastrium, the treatments differ between inducing vomiting and purging; one must distinguish the deficiency or excess of the syndrome to determine the severity of the prescription. Vexation preceding palpitations indicates an Excess pattern; palpitations preceding vexation indicate a Deficiency pattern. Empty vexation manifests as a sensation of depression and nausea in the heart region.
In Taiyang disease, if there is heart vexation, spontaneous sweating, and frequent urination, Guizhi Tang (Cinnamon Twig Decoction) is contraindicated; Shaoyao Gancao Tang (Peony and Licorice Decoction) is appropriate. After taking Guizhi Tang, if vexation and thirst appear following sweating, and the pulse is flooding, Baihu Jia Renshen Tang (White Tiger Plus Ginseng Decoction) should be used.
In Yangming disease, if there is heart vexation, frequent vomiting, alternating high fever and chills, palpitations in the epigastrium, and difficult urination, Xiaochaihu Tang (Minor Bupleurum Decoction) with Fuling (Poria) is indicated. In Cold Damage, if within two to three days the patient experiences palpitations and vexation, Xiao Jianzhong Tang (Minor Construct the Middle Decoction) is prescribed.
After sweating resolves the condition, if vexation recurs after half a day and the pulse is floating and rapid, another sweating treatment may be applied using Guizhi Tang.
If there is nasal bleeding accompanied by vexation and thirst, and drinking water causes vomiting of fluids, Wuling San (Five-Ingredient Powder with Poria) is used; if this fails, Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction) is employed.
If after purging, the patient is restless during the day but quiet at night, without thirst or vomiting, and lacking exterior signs, with a faint and deep pulse, Ganjiang Fuzi Tang (Dried Ginger and Aconite Decoction) is indicated. If there is inner restlessness and discomfort (Ao'nao), Zhizi Chidui Tang (Gardenia and Fermented Soybean Decoction) is used.
If after sweating or purging the disease remains unresolved and is accompanied by vexation and restlessness, Fuling Sini Tang (Poria and Sini Decoction) is used. After severe purging, if for six or seven days there is no bowel movement, the disease is unresolved, and there is abdominal fullness and pain due to dry feces, Dachengqi Tang (Major Order the Qi Decoction) is indicated. Following vomiting, sweating, or purging, if empty vexation occurs with epigastric stuffiness and fullness, qi rushing up to the chest, dizziness, and body tremors, Fuling Guizhi Baizhu Gancao Tang (Poria, Cinnamon Twig, Atractylodes, and Licorice Decoction) is prescribed.
In Kidney Cold Damage, with no heat in the interior or exterior, but only mental confusion and vexation, aversion to light, occasional abdominal pain, and a deep, fine pulse, Sini Tang (Recover the Yielding Decoction) is used.
"Fan" (vexation) refers to mental disturbance, while "Zao" (restlessness) refers to physical agitation arising from anger or distress; Zao develops gradually from Fan. In Cold Damage, Fan-Zao can be distinguished by Yin/Yang and Deficiency/Excess. Heart heat causes Fan, associated with Yang Excess and Yin Deficiency; Kidney heat causes Zao, associated with Yin Excess and Yang Deficiency. Fan represents mild heat, while Zao represents severe heat. When pathogenic factors are in the interior causing Fan-Zao, or when Zao occurs directly without prior Fan, this is termed Yang Excess repelling Yin. The patient may wish to lie in mud, and even water cannot be swallowed.
If the Jie Xiong (Chest Binding) syndrome is complete and accompanied by Fan-Zao, or if vomiting and diarrhea with cold limbs are accompanied by Fan-Zao, or if diarrhea with fever, cold limbs, and inability to sleep due to restlessness occur, or if there is aversion to cold, muscle cramping, absent pulse, and restlessness, these are all untreatable conditions.
In Taiyang Wind Strike, if the pulse is floating and tight, with fever, aversion to cold, body pain, absence of sweat, and restlessness, Da Qinglong Tang (Major Green Dragon Decoction) is used. If two to three days have passed since onset, the pulse is weak, and there are no Taiyang signs, Chaihu Tang (Bupleurum Decoction) is used.
If the syndrome presents with Fan-Zao, hardness in the epigastrium, and ability to eat, Xiao Chengqi Tang (Minor Order the Qi Decoction) is used to gently harmonize. If there are palpitations with Fan-Zao, Xiao Jianzhong Tang is used.
For those with aversion to wind, spontaneous sweating, a soft and weak pulse, and restlessness, the aforementioned methods apply.
If there is only fever for six or seven days and a desire to drink water, Wuling San is used. If there is no fever but only delirium and restlessness, Wuling San is administered with water, followed by induced vomiting.
In Yangming disease, if there is constipation, periumbilical pain, and restlessness, Chengqi Tang is used to purge.
In Shaoyang disease, if there is vomiting, diarrhea, cold limbs, and extreme restlessness, Wuzhuyu Tang (Evodia Decoction) is used.
With cold limbs, deep pulse, spontaneous diarrhea, and restlessness preventing sleep, Huanglian Jizi Tang (Coptis and Egg Yolk Decoction) or Huanglian Ejiao Tang (Coptis and Donkey-Hide Gelatin Decoction) are used.
After purging and subsequent sweating, if the patient is restless and unable to sleep during the day but quiet at night, without vomiting or thirst, lacking exterior signs, with a deep and faint pulse, and no significant fever, Ganjiang Fuzi Tang is used. If after sweating or purging the disease persists and is accompanied by restlessness, Fuling Sini Tang is used.
When Yin reaches its extreme, it may cause restlessness. Or, in cases of Yin toxicity or Fire Evil, if administering hot medicines worsens the condition, cooling medicines must not be used. If restlessness is severe, hot formulations such as Sini Tang should still be administered.
