(Fan [irritability] refers to the disturbance of the Heart spirit, hence the character Fan contains the radical Fire; Zao [restlessness] refers to the agitation of Kidney Qi, hence the character Zao contains the radical Foot. There is a distinction between exterior and interior presentations, as well as differences caused by heat-fire coercion or Yang deficiency.)
Taiyang disease (disease pattern resulting from pathogenic invasion of the Taiyang channel) presents with fever, aversion to cold, body pain, absence of sweating, and a floating and rapid pulse. (Da Qing Long Tang [Major Blue Green Dragon Decoction] is indicated.) Following purgation in Taiyang disease, if the Stomach becomes dry, presenting with irritability, insomnia, desire for water but difficulty in urination, and hard stools, purgation may be applied. (Da Cheng Qi Tang [Major Order the Qi Decoction] is indicated.) After both purgation and diaphoresis, if irritability and insomnia occur, but the patient is quiet at night, without vomiting, thirst, or exterior signs, and with a deep and faint pulse and no significant heat on the body, Gan Jiang Fu Zi Tang [Dried Ginger and Aconite Decoction] is indicated. If the condition remains unresolved after diaphoresis and purgation, presenting with irritability but no thirst, and a deep and faint pulse, Fu Ling Si Ni Tang [Poria and Reversing Cold Decoction] is indicated. In Yangming disease (disease pattern resulting from pathogenic invasion of the Yangming channel), if there has been no bowel movement for five or six days, accompanied by periumbilical pain, irritability, and dry feces, Cheng Qi Tang [Order the Qi Decoction] is indicated. In Shaoyin disease (disease pattern resulting from pathogenic invasion of the Shaoyin channel), if vomiting, diarrhea, and cold extremities are accompanied by irritability, the prognosis is fatal. (Wu Zhu Yu Tang [Evodia Decoction] is indicated.) In Jueyin disease (disease pattern resulting from pathogenic invasion of the Jueyin channel), if the pulse is deep with cold extremities, the skin is cold and restless with no temporary relief, this constitutes Zang Jie [Organ Binding], which is fatal.
(Ao Nao [profound vexation] is synonymous with Nao [annoyance], commonly used in ancient texts. It describes a state of depression, discomfort, and helpless inner turmoil, more severe than simple irritability. This occurs because improper purgation allows exterior pathogens to penetrate the deficiency and lodge in the chest and diaphragm, causing Ao Nao.)
In Taiyang disease, if the pulse is floating and rapid, accompanied by headache, fever, slight sweating, and aversion to cold, the exterior syndrome remains unresolved. Shortness of breath and Ao Nao are present. (Zhi Zi Bai Pi Tang [Gardenia and Phellodendron Bark Decoction] is indicated.) After diaphoresis, emesis, or purgation, if insomnia and Ao Nao occur, Zhi Zi Chi Tang [Gardenia and Fermented Soybean Decoction] is indicated. In Yangming disease, after purgation, if there is external heat, warm hands and feet, Ao Nao, and sweating only on the head, Zhi Zi Chi Tang [Gardenia and Fermented Soybean Decoction] is indicated. In Yangming disease, if purgation leads to Ao Nao and dry feces remain in the intestines, Da Cheng Qi Tang [Major Order the Qi Decoction] is indicated. In Yangming disease, if there is no sweating, difficult urination, and Ao Nao, jaundice will inevitably develop. (Yin Chen Tang [Artemisia Capillaris Decoction] is indicated.)
(Pain at the vertex and back of the head belongs to the Taiyang channel; pain in the forehead belongs to the Yangming channel; pain at the temporal angle belongs to the Shaoyang channel. The meridians of the Three Yin channels return at the neck, so they typically do not cause headache. Only the Jueyin channel connects to the vertex, so it can also cause headache. However, in Feng Wen [Wind-Warmth] disease affecting the Shaoyin channel, or Wen Bing [Warm Diseases] affecting the Taiyin channel, headache may paradoxically occur. Similarly, in Yin Du [Yin Toxin] conditions, headache occurs due to phlegm and rebellious Qi obstructing upward, preventing Qi from descending. Such cases cannot be rigidly confined to meridian differentiation.)
For Taiyang disease, if there is vertex headache, sweating, and aversion to wind, Gui Zhi Tang [Cinnamon Twig Decoction] is indicated. If there is no sweating and aversion to cold, Ma Huang Tang [Ephedra Decoction] is indicated. For Yangming disease, if there is frontal headache, eye pain, nasal dryness, no aversion to cold but aversion to heat, and solid stools, Tiao Wei Cheng Qi Tang [Regulate the Stomach Order the Qi Decoction] is indicated. If there is tidal fever, delirium, confusion, thirst, headache, and a deep, rapid, and forceful pulse, Xiao Cheng Qi Tang [Minor Order the Qi Decoction] is indicated. For Shaoyang disease, if there is temporal headache and a wiry, rapid pulse, Xiao Chai Hu Tang [Minor Bupleurum Decoction] added with Chuan Xiong [Ligusticum Rhizome] is indicated. For Jueyin disease headache accompanied by vomiting of saliva and froth, Wu Zhu Yu Tang [Evodia Decoction] is indicated. For Taiyin disease headache caused by rebellious Qi and phlegm, Er Chen Tang [Two-Cured Decoction] added with Zhi Shi [Immature Bitter Orange], Chuan Xiong [Ligusticum Rhizome], and Xi Xin [Asarum] is indicated. For Shaoyin disease headache caused by cold feet and rebellious Qi, Ma Huang Fu Zi Xi Xin Tang [Ephedra, Aconite, and Asarum Decoction] is indicated.
