Taiyang disease presenting with *ao nao* (restlessness and irritability). How is this explained? "Ao" refers to annoyance, and "nao" describes a state of depression. It manifests as a feeling of annoyance and irritability in the heart region, with a sense of oppression and discomfort, and a feeling of helplessness. This condition is more severe than ordinary vexation and is termed *ao nao*. It arises because, after inappropriate purgation, yang pathogenic factors from the exterior sink inward due to deficiency, becoming stagnant and unable to disperse, lodging between the chest and epigastrium. Thus, such symptoms occur. The Classics state: "When the exterior syndrome is unresolved, if the physician incorrectly uses purgation, the stomach becomes empty, and external pathogens disturb the diaphragm, causing *ao nao* in the heart." It also states: "After purgation, vexation increases, and there is *ao nao* in the heart resembling hunger." These are various forms of *ao nao* caused by the sinking of yang qi into the interior. The treatment methods for these conditions involve either emesis or purgation. If, after sweating, vomiting, or purging, one experiences deficiency vexation and insomnia, and in severe cases, restlessness turning over in bed with *ao nao* in the heart, Zhizi Chi Tang (Gardenia and Fermented Soybean Decoction) should be administered to induce emesis, thereby draining the constrained heat. If Yangming disease has been treated with purgation, and there is still heat on the exterior, warm hands and feet, no formation of Jie Xiong (chest binding), *ao nao* in the heart, a sensation of hunger but inability to eat, and only sweating on the head, this too belongs to pathogenic heat constrained in the chest; Zhizi Chi Tang should be used to induce emesis and expel the constrained heat. If, after purgation in Yangming disease, there is *ao nao* and vexation in the heart; or if a Yangming patient presents with no sweat, difficult urination, and *ao nao* in the heart, jaundice will inevitably develop. These two scenarios belong to pathogenic heat constrained in the stomach; Dachengqi Tang (Major Order the Qi Decoction) or Yinchen Tang (Artemisia Capillaris Decoction) should be used to purge and eliminate the internal heat. Those who understand these principles, applying emesis and purgation appropriately and using decoctions correctly, will find no illness that cannot be cured. However, if sweating is indicated but emesis is used instead, or if warm-natured treatments are applied to heat syndromes, hundreds of transformational syndromes may arise, with many cases developing macules and jaundice. Physicians, please investigate these matters thoroughly.
Regarding tongue coating in Taiyang disease, how is this explained? The tongue is the organ of the Heart, corresponding to the Fire element of the South; its normal color should be red and lustrous. After three or four days of Taiyang disease, a membrane appears on the tongue, white and slippery like moss. In severe cases, it may become dry or rough, yellow, or black. These variations indicate the depth of the heat pathogen. When the pathogen is at the exterior, there is no coating on the tongue. When the pathogen transmits to the interior and body fluids become congealed and obstructed, a coating forms on the tongue. When cold pathogens first transmit inward and have not fully transformed into heat, or when the pathogen is in the half-exterior/half-interior stage, or lodged in the chest, the tongue will present a white and slippery coating. The Classics state: "A tongue appearing to have a coating indicates heat in the Dantian and cold in the chest," which signifies the initial transmission of the pathogen into the interior. In Yangming disease, if there is hardness and fullness under the hypochondrium, constipation, vomiting, and a white coating on the tongue, Xiaochaihu Tang (Minor Bupleurum Decoction) may be given, indicating the pathogen is in the half-exterior/half-interior stage. If Yangming disease is incorrectly purged, leading to an empty stomach, disturbance of the diaphragm by external pathogens, *ao nao* in the heart, and a coating on the tongue, Zhizi Chi Tang (Gardenia and Fermented Soybean Decoction) is the primary treatment, indicating the pathogen is lodged in the chest. For Zang Jie (Organ Binding) syndromes, purgation might seem appropriate, but if the tongue coating is slippery, purgation is contraindicated because the pathogen has not fully transformed into heat and still retains exterior cold. When the pathogen transforms into heat, the tongue coating becomes rough rather than slippery. The Classics state: "In Taiyang disease lasting seven or eight days without resolution, with heat constrained in the interior, fever in both the interior and exterior, frequent aversion to wind, extreme thirst, a large dry tongue with vexation, and a desire to drink several liters of water, Baihu Jia Renshen Tang (White Tiger Decoction plus Ginseng) is the primary treatment." This indicates that heat has consumed body fluids, and the previously slippery coating has dried up. If heat gathers in the Stomach, the tongue turns yellow, indicating deep-seated heat. The *Jin Gui Yao Lue* (Essential Prescriptions of the Golden Cabinet) states: "If the tongue is yellow and purgation has not yet been used, administer purgation, and the yellow coating will disappear." If the tongue color turns black, it indicates extreme heat. The *Huangdi Zhenjing* (Yellow Emperor's Classic of Acupuncture) states: "Death occurs in heat diseases characterized by dry mouth and a black tongue." This is because the Heart is the sovereign organ, opening into the tongue; black is the color of the Kidney. When seen on the tongue (the Heart's manifestation), where Fire (Heart) meets Water (Kidney), extreme pathogenic heat causes mutual destruction (Water overcoming Fire), indicating inevitable death. Observing the mouth and tongue allows one to discern the favorable or unfavorable course of the disease.
