Among all living things, humans are the most spiritual; as the four seasons change, various diseases arise. To distinguish yin and yang syndromes, one must understand the principles of heaven and earth. Investigating the doubts of cold and heat, exterior and interior, clarifies the root and branch (Ben-Biao); administering methods of sweating (Han), purging (Xia), tonifying (Bu), and vomiting (Tu) broadly benefits all living beings.
The pathogen initially affects the hair follicles, then transmits to the skin and interstices. Aversion to wind and spontaneous sweating indicate a Guizhi Tang (Cinnamon Twig Decoction) syndrome; wheezing, fever, and aversion to cold indicate a Mahuang Tang (Ephedra Decoction) pattern. When both yin and yang are deficient and the body itches, Guizhi Mahuang Gehan Tang (Guizhi and Mahuang Half-Combined Decoction) acts synergistically; when chills and fever alternate and the pulse is still deficient, Guizhi Er Yuebi Yi Tang (Guizhi Twice-Yuebi Once Decoction) should be combined.
Thereupon, ginger and jujube are assisted, mixed with fermented soybean and scallion. For free urination and defecation but aversion to wind, Ganjiang (Dried Ginger) is used; for palpitations and abdominal pain, Jianzhong Tang (Middle-Joint Building Decoction) is indicated. For an extremely hot pulse and vexation, Renshen (Ginseng) is added to Baihu Tang (White Tiger Decoction); for a vexatious and surging pulse with wheezing, Xingren (Apricot Kernel) is added to Qinglong Tang (Azure Dragon Decoction). For chest binding (Jie Xiong), diarrhea, and resolved exterior syndrome, Zhuque Tang (Vermilion Bird Decoction) yields results; for dizziness, muscle twitching, and watery diarrhea, Xuanwu Tang (Black Tortoise Decoction) achieves efficacy.
Thus, Jiegeng (Platycodon Root) widens the throat, and Gancao (Licorice Root) calms palpitations. For abdominal fullness, Houpo (Magnolia Bark) and Shengjiang (Fresh Ginger) are used; for mental restlessness and oppression in the heart, Zhizi (Gardenia Fruit) and Chishi (Fermented Soybean) are employed. For difficult urination, Zhuling Tang (Polyporus Decoction) is administered; for phlegm-rheum in the diaphragm, Gouti San (Melon Seed Powder) induces vomiting. Or, when heat arises with abdominal and hypochondriac fullness; or again, when madness occurs with tidal fever and palpitations. When the interior is affected but the exterior remains unresolved, or when urination is scanty and stool is constipated, one must clearly distinguish between Da Chaihu Tang (Major Bupleurum Decoction) and Xiao Chaihu Tang (Minor Bupleurum Decoction), and directly narrate the applications of Da Chengqi Tang (Major Order the Qi Decoction) and Xiao Chengqi Tang (Minor Order the Qi Decoction). Chest binding often involves thirst, so the practitioner should rely on Xianxiong Tang (Depress the Chest Decoction); delirium and madness require Dahuang (Rhubarb) to first regulate the stomach.
One should know that Qinglong Tang (Azure Dragon Decoction) and Shizao Tang (Ten Jujube Decoction) address water retention in the exterior or interior; Sanhuang Tang (Three Yellows Decoction) and Sini Tang (Reversed Extremities Decoction) address dampness with cold or warmth. For diarrhea, Tongmai Sini Tang (Pulse-Reviving Reversed Extremities Decoction) is used; for irritability, Fuling Sini Tang (Poria Reversed Extremities Decoction) is indicated. For absent pulse, Danggui Sini Tang (Angelica Reversed Extremities Decoction) is used; for blood loss, Renshen (Ginseng) is required (Sini Jia Renshen Tang). For sweating, Fuzi (Aconite Root) is added (Fuzi Xiexin Tang); for aversion to wind, Huangqin (Scutellaria Root) is included (Huangqin Xiexin Tang); for water under the hypochondrium, Shengjiang (Fresh Ginger) is added (Shengjiang Xiexin Tang); for nausea and vomiting in the heart, Renshen (Ginseng) is added (Renshen Xiexin Tang). The former four cases generally belong to Sini Tang categories; the latter four cases are similar to Xiexin Tang categories. Warming cold excess and resolving chest binding praises the subtle merit of "Sanbai" (Three Whites) decoctions; generating fluids and unblocking water pathways appreciates the profound use of Wuling San (Five-Ingredient Powder with Poria).
For hemoptysis, Huangqin (Scutellaria Root) is suitable; for pus in stool, Taoren (Peach Kernel) can be added. For jaundice, Yinchen Hao Tang (Artemisia Capillaris Decoction) is sought first, but Gouti (Melon Seed) should be used prior (first emetic, then clearing); for macular toxins, Zixue Dan (Purple Snow Pill) should be used after Shengma Gegen Tang (Cimicifuga and Kudzu Decoction) (first releasing the exterior, then clearing heat). For warming and drying dampness, Guizhi (Cinnamon Twig), Fuzi (Aconite Root), and Cangzhu (Atractylodes Rhizome) can be entered; within Lizhong Tang (Regulate the Middle Decoction), adding Jiang (Ginger) and Fuzi (Aconite) is appropriate.
Furthermore, regarding additional discussions on modifications of Qinglong Tang and Chaihu Tang: for residual heat in the body, Guizhi Tang requires additions; for hardness and fullness under the hypochondrium, Dazao (Jujube) can be removed; for excessive sweating, Fuzi (Aconite Root) is added to Xiao Chaihu Tang (Minor Bupleurum Decoction); for Ben Tun (Running Piglet Qi), Guizhi Jia Gui Tang (Guizhi Plus Cinnamon Twig Decoction) is used; for wheezing, Xingren (Apricot Kernel) is added to Xiao Qinglong Tang (Minor Azure Dragon Decoction); for coughing, Wuweizi (Schisandra Berry) is added to Xiao Chaihu Tang (Minor Bupleurum Decoction). Gouti (Melon Seed) treats persistent vexation and thirst; Fuling (Poria) treats difficult urination. For internal cold, Zhuyu (Cornus Fruit) is used; for external swelling, Fangji (Stephania Root) is employed. Shengjiang (Fresh Ginger) stops vomiting, Banxia (Pinellia Tuber) treats hiccups. Using Zhike (Bitter Orange Peel) is due to alcohol toxin lingering in the chest; using Huangbo (Phellodendron Bark) is due to epistaxis. One may obtain Renshen (Ginseng) to remove vexation, or take Fuling (Poria) to cure palpitations. For severe headache like breaking, Congbai Lianxu (Scallion White with Roots) is used; for coughing and rebellious qi nearing exhaustion, Ganshi Bingdi (Dried Persimmon with Peduncle) is employed. For diarrhea, Ganjiang (Dried Ginger) is used; for body pain, Zhichi Tang (Gardenia and Fermented Soybean Decoction) is applied. Is it not just the exclusive use of these two formulas, but also the common recommendation of many herbs? Wonderful! I have refined this knowledge for over forty years, and my merit in saving lives has been witnessed.
