Shaoyang fever (Fever of the Shaoyang stage) is accompanied by aversion to cold and shivering. Yangming fever (Fever of the Yangming stage) presents with body heat and spontaneous sweating. Shaoyang fever (Fever of the Shaoyang stage) often involves dry retching. The manifestations of fever in the Three Yang stages are not uniform. (Zhang Zhongjing stated: Fever accompanied by aversion to cold indicates onset on the Yang aspect. Generally, the Three Yang stages mostly present with fever. In the Taiyang syndrome (Syndrome of the Taiyang stage), it is described as "shivering with aversion to cold" and "mild fever." Therefore, when Taiyang fever occurs, there is aversion to cold and shivering. In the Yangming syndrome (Syndrome of the Yangming stage), it is described as "body heat with sweating," without aversion to cold but rather aversion to heat. Therefore, when Yangming fever occurs, there is spontaneous sweating. In the Shaoyang syndrome (Syndrome of the Shaoyang stage), it is described as "headache, fever, hardness and fullness below the costal region, and dry retching." Therefore, when Shaoyang fever occurs, vomiting ensues.) Generally speaking, cases with predominant cold are easier to treat, whereas those with predominant heat and less cold indicate extreme cold. (Extreme cold generates heat; thus, predominant heat signifies the extremity of cold. Predominant cold indicates a superficial disease, hence it is easy to treat.) To resolve fever, Xiaochaihu Tang (Minor Bupleurum Decoction) or Dachaihu Tang (Major Bupleurum Decoction) may be used, with skill lying in assessing the depth of the condition. (If fever occurs without exterior signs, Xiaochaihu Tang (Minor Bupleurum Decoction) or Dachaihu Tang (Major Bupleurum Decoction) should be employed. For shallow heat, Xiaochaihu Tang (Minor Bupleurum Decoction) is appropriate; for deep heat, Dachaihu Tang (Major Bupleurum Decoction) is appropriate. Xiaochaihu Tang (Minor Bupleurum Decoction) releases the muscle layer, while Dachaihu Tang (Major Bupleurum Decoction) purges interior excess. External symptoms and internal pulse should serve as the standard.) Initially, the Three Yin stages do not present with fever, except for two specific confirmations in Shaoyin syndrome (Syndrome of the Shaoyin stage). Pulselessness with fever belongs to the Ma Huang Fu Zi Xi Xin Tang (Ephedra, Aconite, and Asarum Decoction) pattern; interior cold with exterior false heat is suitable for Sini Tang (Recovery from Collapse Decoction). (Zhang Zhongjing stated: When Shao Yin disease (Disease of the Shao Yin stage) first manifests, fever appears contrary to expectation, and the pulse is sunken; this is treated with Ma Huang Fu Zi Xi Xin Tang (Ephedra, Aconite, and Asarum Decoction). He also stated: In Shao Yin disease (Disease of the Shao Yin stage), with diarrhea of undigested food, interior cold with exterior false heat, and cold limbs, Tongmai Sini Tang (Pulse-Recovering Sini Decoction) is indicated.)
Tidal fever (Tidal fever) indicates excess and warrants purgation; the words of Zhang Zhongjing can be relied upon. (Zhang Zhongjing stated: Tidal fever (Tidal fever) indicates excess. The general principle is that purgation is appropriate.) One must further examine whether the pulse is floating or sunken. If it is merely wiry and floating, the exterior pathogen remains, and purgation is contraindicated. (A floating pulse indicates the pathogen is at the exterior; a sunken pulse indicates the interior. If the pulse is merely wiry and floating, indicating an existing exterior syndrome, purgation should not be used.) Aversion to cold with a floating pulse indicates an exterior syndrome; administer Xiaochaihu Tang (Minor Bupleurum Decoction) and do not purge. (Zhang Zhongjing stated: In Yangming disease (Disease of the Yangming stage), if there is tidal fever (Tidal fever), profuse sweating, and slight aversion to cold, the fever does not follow a tidal pattern, and Chengqi Tang (Cathartic Decoction) should not be given.) Abdominal fullness with obstruction calls for Xiao Chengqi Tang (Minor Order the Qi Decoction); harmonize the Stomach Qi (Stomach's physiological function) without excessive purging. (Zhang Zhongjing stated: If there is abdominal fullness and constipation, Xiao Chengqi Tang (Minor Order the Qi Decoction) may be given to mildly harmonize the Stomach Qi (Stomach's physiological function), avoiding severe purgation.) There are three interpretations of tidal fever (Tidal fever), all pertaining to Yangming disease (Disease of the Yangming stage) and the decision regarding Xiaochaihu Tang (Minor Bupleurum Decoction). The first involves tidal fever (Tidal fever) accompanied by belching. The second involves mild fever and loose stools. (Zhang Zhongjing stated: In Yangming Wind-Strike (Wind-strike affecting the Yangming stage), with a wiry, floating, large pulse, shortness of breath, generalized abdominal fullness, pain in the hypochondrium and heart region, obstruction of Qi upon prolonged pressure, dry nose, inability to sweat, lethargy, jaundice of the whole body and eyes, difficulty in urination, tidal fever (Tidal fever), and frequent hiccups, Xiaochaihu Tang (Minor Bupleurum Decoction) is indicated. He also stated: In Yangming disease (Disease of the Yangming stage), with tidal fever (Tidal fever), loose stools, normal urination, and persistent fullness in the chest and hypochondrium, Xiaochaihu Tang (Minor Bupleurum Decoction) is indicated.) The third involves onset at dusk, followed by mild diarrhea and increased hiccups. (Hiccup, pronounced 'yi lie qie', refers to rebellious Qi. Zhang Zhongjing stated: When a person develops tidal fever (Tidal fever) at dusk, if severe, they may lose consciousness during the episode.) There is also one syndrome in Taiyang disease (Disease of the Taiyang stage) involving tidal fever (Tidal fever), which is Jie Xiong (Chest Binding) presenting with tidal fever (Tidal fever). (Zhang Zhongjing stated: After excessive sweating and subsequent purgation in Taiyang disease (Disease of the Taiyang stage), if there is no bowel movement for five to six days, dry tongue with thirst, slight tidal fever (Tidal fever) at dusk, hardness, fullness, and pain from the epigastrium to the lower abdomen, Da Xianxiong Tang (Major Chest-Binding Decoction) is indicated.)
