(Wet-Warmth, Middle-Dampness, Wind-Dampness)
Wet-warmness (Shiwen), middle-dampness (Zhongshi), and wind-dampness (Fengshi) share the same name but differ in substance. When summerheat (Shu) and dampness (Shi) struggle together, wet-warmness is formed. It presents with excessive sweating in the chest region, severe headache as if split open, cold reversal in both lower legs, delirious speech due to suffering, and a pulse that is soggy and weak at the cun position (Yang) and small and rapid at the chi position (Yin). (The above describes the syndrome of Wet-Warmness.) The second type is middle-dampness disease, characterized by a deep and slow pulse. It manifests as generalized body pain accompanied by jaundice, loose stools, and scanty or difficult urination. (Zhang Zhongjing stated: In Taiyang disease, joint pain with irritability and a deep, slow pulse is named middle-dampness. Its symptoms include difficulty in urination and loose stools; one should promote diuresis. He also stated: For patients with dampness diseases, there is generalized body pain, fever, and a yellowish skin color resembling smoked tobacco. Such patients may have only head sweating, stiffness in the back, and a desire to cover themselves and seek warmth. If purgatives are used prematurely, it leads to chest fullness, urinary difficulty, and a tongue coating resembling moss, caused by heat in the lower abdomen and cold in the chest, resulting in thirst for water but inability to drink, and dry mouth with irritability. He further stated: In dampness diseases, body pain, fever, yellow face, wheezing, headache, nasal congestion, and irritability occur with a large pulse. The patient can eat normally, and there is no abdominal pathology. If the head suffers from cold-dampness, causing nasal congestion, inserting medicine into the nose will cure it.) These originate from the qi of wind, rain, mountains, and marshes; contracting them causes this disease. The third type is wind-dampness, presenting with a floating pulse alone, painful and heavy limbs difficult to turn or side, slight sweating on the forehead, mild bodily edema, aversion to removing blankets, and aversion to cold with shivering. (This discusses wind-dampness. Regarding wind-dampness syndrome, Zhang Zhongjing stated: Generalized body pain and fever that worsens in the afternoon (Shi hour) is named wind-dampness. This disease results from sweating while exposed to wind or prolonged exposure to cold. Thus, the pulse is floating, there is slight sweating on the forehead, and an aversion to removing blankets.) To treat wind-dampness, one aims to moisten the body; only when both wind and dampness are expelled is the treatment correct. Zhang Zhongjing stated: When wind and dampness struggle, causing generalized body pain, the method requires sweating to resolve it. If it rains continuously and the physician suggests sweating, but the illness does not resolve after sweating, why? The answer is: If sweating is profuse, only the wind is expelled while dampness remains, hence it does not resolve. For treating wind-dampness, slight sweating indicates that both wind and dampness are expelled. Fangji Huangqi Shuzhu Fuzi Decoction (Aristolochia Root and Astragalus Root with Atractylodes Macrocephala and Prepared Aconite Root Decoction), when used correctly, ensures efficacy. (Fangji Huangqi Decoction and Shuzhu Fuzi Decoction both treat wind-dampness with spontaneous sweating.)
