The Shaoyang Gallbladder Meridian (Shaoyang Dan Jing) courses through the hypochondriac region. When pathogenic factors invade this syndrome, pain in the hypochondrium becomes unbearable. (Zhongjing stated: The Shaoyall meridians course through the hypochondrium and pass through the ears. Because pathogens reside here, hypochondriac pain and deafness occur.) If there is hardness and fullness below the heart (Xin Xia), radiating to the hypochondrium, Shizao Tang (Ten Date Decoction) is definitely indicated. (Zhongjing stated: In Taiyang Wind Strike, following emesis or purgation, vomiting, headache, epigastric stuffiness, hardness, fullness, and pain radiating from the lower hypochondrium occur. If the exterior is resolved but the interior is not harmonized, Shizao Tang governs it.) Yangming presents with hardness and fullness, constipation, neck stiffness, anorexia, and tidal fever. If the disease transitions into the Shaoyang meridian, Xiaochaihu Tang (Minor Bupleurum Decoction) is strictly applicable. (Zhongjing stated: In Yangming disease, with constipation, hardness and fullness in the hypochondrium, and coating on the tongue, Xiaochaihu Tang may be given. Also stated: With neck stiffness and hypochondriac fullness, Xiaochaihu Tang may be given. Also stated: After five or six days of Shanghan, with Fengzhong, alternating chills and fever, bitter distension in the chest and hypochondrium, silent desire not to eat, Xiaochaihu Tang governs it. Also stated: When Yangming disease does not resolve and transitions to Shaoyang, with hardness and fullness in the hypochondrium and dry retching, Xiaochaihu Tang governs it.) If a patient has accumulations (Pi Ji) penetrating beside the umbilicus, with pain radiating to the genital tendons, this condition is named Zang Jie (Visceral Knotting). (Zhongjing stated: When the patient has hypochondriac pain, pre-existing accumulations, pain beside the umbilicus, and pain radiating from the lower abdomen into the genital tendons, it is named Zang Jie.)
Abdominal pain can be excess or deficiency; one must observe the symptoms and pulse. If the Chi pulse is wiry accompanied by diarrhea or dysentery, this is Yangming deficiency pain, requiring Jianzhong Tang (Establish the Middle Decoction). (Zhongjing stated: In Shanghan, if the Yang pulse is choppy and the Yin pulse is wiry, according to the method, urgent abdominal pain should occur; first give Jianzhong Tang.) If the Guan pulse is excess and large, with constipation, plus abdominal fullness, it resides within the Interior Excess category. (Zhongjing stated: When the patient has no bowel movement, periumbilical abdominal pain, irritability, and periodic exacerbations, this indicates dry feces.) For Yin syndrome abdominal pain, Sini San (Frigid Extremities Powder) is used. (Zhongjing stated: In Shaoyin disease, with cold extremities, or cough, or palpitations, or difficult urination, or abdominal pain, or heavy sensation during diarrhea/dysentery, Sini San governs it. Also stated: In Shaoyin disease, with clear undigested stool in diarrhea, interior cold and exterior heat, faint pulse nearly absent, pulse unobtainable, or abdominal pain, Tongmai Sini Tang (Pulse-Reviving Frigid Extremities Decoction) governs it. Also stated: In Shaoyin disease, if two or three days pass without improvement, reaching four or five days, with abdominal pain and difficult urination, Zhenwu Tang (True Warrior Decoction) governs it.) Abdominal pain resulting from erroneous purgation is treated with Guizhi Tang (Cinnamon Twig Decoction) variants. (In Taiyang disease, if the physician erroneously applies purgation, leading to abdominal fullness and intermittent pain, it belongs to Taiyin; Guizhi Jia Shaoyao Tang (Cinnamon Twig Decoction Augmented with Paeonia) governs it. For severe excess pain, Guizhi Jia Dahuang Tang (Cinnamon Twig Decoction Augmented with Rhubarb) is used.) If pathogens in the stomach cause heat in the chest and vomiting, Huanglian Tang (Coptis Decoction) can eliminate it. (Zhongjing stated: In Shanghan, with heat in the chest and pathogens in the stomach, abdominal pain, and desire to vomit, Huanglian Tang governs it.)
