1. The manifestation of Taiyang Disease (disease caused by pathogen invading the Taiyang meridian) is characterized by a floating pulse, headache and stiffness/pain in the head and nape, and aversion to cold.
2. In Taiyang Disease, if there is fever, sweating, aversion to wind, and a slow/moderate pulse, it is named Zhong Feng (Wind Strike).
3. In Taiyang Disease, whether fever has developed or not, there must be aversion to cold, body pain, vomiting/counterflow qi, and a tight pulse in both Yin and Yang positions; this is named Shang Han (Cold Damage).
4. On the first day of Shang Han, the Taiyang meridian receives the pathogen. If the pulse is calm, the disease does not transmit; if there is a strong desire to vomit, or irritability/restlessness, and the pulse is rapid and urgent, the disease is transmitting.
5. If after two or three days of Shang Han, symptoms of Yangming Disease and Shaoyang Disease do not appear, the disease has not transmitted.
6. In Taiyang Disease, if there is fever and thirst, but no aversion to cold, it is Wen Bing (Warm Disease). If after inducing sweating, the body becomes scorching hot, it is named Feng Wen (Wind-Warmth). Feng Wen is characterized by a floating pulse in both Yin and Yang positions, spontaneous sweating, heavy body, drowsiness, snoring during breathing, and difficulty speaking. If purgation is mistakenly applied, it leads to difficult urination, fixed gaze, and incontinence. If fire therapy is mistakenly applied, mild cases develop slight yellowing of the skin, while severe cases resemble convulsive epilepsy with spasms. If smoked with fire, one mistake may prolong life slightly, but a second mistake will hasten death.
7. Diseases with fever and aversion to cold originate in the Yang meridians; diseases without fever but with aversion to cold originate in the Yin meridians. Those originating in Yang heal in seven days; those originating in Yin heal in six days. This is because the Yang number is seven and the Yin number is six.
8. In Taiyang Disease, if headache persists for more than seven days and then heals spontaneously, it is because the pathogen has completed its course through the meridian. If the pathogen intends to transmit to the next meridian, acupuncture at the Foot Yangming meridian prevents transmission and leads to recovery.
9. The time when Taiyang Disease tends to resolve is from Si hour (09:00-11:00) to Wei hour (13:00-15:00).
10. For patients with Wind disorders, if the exterior syndrome resolves but they still feel unwell, they will recover in twelve days.
11. If a patient has high body heat but desires clothing, the heat is in the skin while cold is in the bones/marrow. If the patient feels very cold but rejects clothing, the cold is in the skin while heat is in the bones/marrow.
12. In Taiyang Zhong Feng, the Yang pulse is floating and the Yin pulse is weak. Floating Yang indicates spontaneous heat generation; weak Yin indicates spontaneous sweating. Symptoms include shivering aversion to cold, rustling aversion to wind, mild fever, nasal congestion with sound, and dry retching. Guizhi Tang (Cinnamon Twig Decoction) is the main treatment.
Guizhi (Ramulus Cinnamomi, peeled, 3 liang), Shaoyao (Radix Paeoniae Alba, 3 liang), Gancao (Radix Glycyrrhizae Preparata, 2 liang), Shengjiang (Rhizoma Zingiberis Recens, sliced, 3 liang), Dazao (Fructus Jujubae, broken, 12 pieces).
Decoct the above five ingredients. Crush three ingredients. Use 7 sheng of water, simmer over low heat to obtain 3 sheng, remove dregs. Take 1 sheng at a suitable temperature. Shortly after taking, drink over 1 sheng of hot thin porridge to assist medicinal power. Cover with bedding for about one shichen (2 hours) to induce mild sweating all over the body; profuse sweating like flowing water must be avoided, otherwise the disease will not be eliminated. If sweating occurs and the disease resolves after one dose, stop subsequent doses; the full dose need not be consumed. If no sweating occurs, take another dose according to the previous method. If still no sweating, shorten the interval between subsequent doses, completing three doses within half a day. For severe cases, administer day and night, observing continuously. If one dose is finished but symptoms persist, prepare another dose. If sweating does not occur, up to two or three doses may be administered. Avoid raw/cold, sticky/slipy, meat/noodle, five pungent spices, alcohol/dairy, and foul-smelling foods.
13. In Taiyang Disease, with headache, fever, sweating, and aversion to wind, Guizhi Tang (Cinnamon Twig Decoction) is the main treatment. (Use the first formula above.)
14. In Taiyang Disease, with stiffness and tension in the nape and back, and conversely with sweating and aversion to wind, Guizhi Jia Gegen Tang (Cinnamon Twig Decoction plus Kudzu Root) is the main treatment.
Gegen (Radix Puerariae Lobatae, 4 liang), Mahuang (Herba Ephedrae, nodes removed, 3 liang), Shaoyao (Radix Paeoniae Alba, 2 liang), Shengjiang (Rhizoma Zingiberis Recens, sliced, 3 liang), Gancao (Radix Glycyrrhizae Preparata, 2 liang), Dazao (Fructus Jujubae, broken, 12 pieces), Guizhi (Ramulus Cinnamomi, peeled, 2 liang).
Decoct the above seven ingredients. Use 1 dou of water. First decoct Mahuang and Gegen, reducing by 2 sheng, skim off the foam. Add other ingredients, decoct to obtain 3 sheng, remove dregs. Take 1 sheng warm. Cover to induce mild sweating. Porridge is not required. Other care and contraindications follow the Guizhi Tang method. (Note by Chen Yi et al.: In Zhang Zhongjing's original text, Guizhi is used for Taiyang Zhong Feng with spontaneous sweating, and Mahuang for Shang Han without sweating. Here, the symptom is sweating with aversion to wind, yet Mahuang is in the formula, which may not be the original intent. Volume 3 mentions Ge Gen Tang for no sweating with aversion to wind, which matches this formula's use of Mahuang. The name "Guizhi Jia Gegen Tang" here likely implies adding only Gegen to Guizhi Tang.)
15. In Taiyang Disease, after purgation, if qi rushes upward, Guizhi Tang may be given using the previous method. If qi does not rush upward, it must not be given.
16. In Taiyang Disease, after three days, if sweating, vomiting, purgation, or warm needle therapy has been applied but the disease remains unresolved, this is a Huai Bing (Mishandled Disease). Guizhi Tang is not suitable. Observe the pulse and symptoms to identify the error and treat accordingly. Guizhi Tang is originally for releasing the muscle layer. If the patient has a floating and tight pulse, fever, and no sweating, it must not be given. Always remember this to avoid errors.
17. For patients who are habitual drinkers, Guizhi Tang must not be given, as it will cause vomiting; drinkers dislike sweetness.
18. For patients with chronic asthma, when preparing Guizhi Tang, adding Houpo (Cortex Magnoliae Officinalis) and Xingzi (Semen Armeniacae Amarum) is beneficial.
19. Anyone who vomits after taking Guizhi Tang will subsequently vomit pus and blood.
20. In Taiyang Disease, after sweating, if sweating leaks incessantly, the patient has aversion to wind, difficult urination, slight spasms in the four limbs, and difficulty flexing/extending, Guizhi Jia Fuzi Tang (Cinnamon Twig Decoction plus Aconite) is the main treatment.
Guizhi (Ramulus Cinnamomi, peeled, 3 liang), Shaoyao (Radix Paeoniae Alba, 3 liang), Gancao (Radix Glycyrrhizae Preparata, 3 liang), Shengjiang (Rhizoma Zingiberis Recens, sliced, 3 liang), Dazao (Fructus Jujubae, broken, 12 pieces), Fuzi (Radix Aconiti Lateralis Preparata, 1 piece, processed, peeled, broken into 8 slices).
Decoct the above six ingredients. Use 7 sheng of water, decoct to obtain 3 sheng, remove dregs. Take 1 sheng warm. Originally Guizhi Tang, now with added Fuzi; care follows the previous method.
21. In Taiyang Disease, after purgation, if the pulse is hurried (Cu) and there is chest fullness, Guizhi Qu Shaoyao Tang (Cinnamon Twig Decoction minus Peony) is the main treatment. (Cu also interpreted as Zong.)
Guizhi (Ramulus Cinnamomi, peeled, 3 liang), Gancao (Radix Glycyrrhizae Preparata, 2 liang), Shengjiang (Rhizoma Zingiberis Recens, sliced, 3 liang), Dazao (Fructus Jujubae, broken, 12 pieces).
Decoct the above four ingredients. Use 7 sheng of water, decoct to obtain 3 sheng, remove dregs. Take 1 sheng warm. Originally Guizhi Tang, now with Shaoyao removed; care follows the previous method.
22. If there is slight aversion to cold, Guizhi Qu Shaoyao Jia Fuzi Tang (Cinnamon Twig Decoction minus Peony plus Aconite) is the main treatment.
Guizhi (Ramulus Cinnamomi, peeled, 3 liang), Gancao (Radix Glycyrrhizae Preparata, 2 liang), Shengjiang (Rhizoma Zingiberis Recens, sliced, 3 liang), Dazao (Fructus Jujubae, broken, 12 pieces), Fuzi (Radix Aconiti Lateralis Preparata, processed, peeled, broken into 8 slices, 1 piece).
Decoct the above five ingredients. Use 7 sheng of water, decoct to obtain 3 sheng, remove dregs. Take 1 sheng warm. Originally Guizhi Tang, now with Shaoyao removed and Fuzi added; care follows the previous method.
23. In Taiyang Disease, after eight or nine days, if symptoms resemble malaria with fever and aversion to cold, more heat than cold, no vomiting, normal bowel/bladder function, and episodes occurring 2-3 times a day: if the pulse is slightly slow/moderate, recovery is imminent. If the pulse is faint and there is aversion to cold, this indicates deficiency of both Yin and Yang; further sweating, purgation, or vomiting must not be induced. If the face shows a flushed/hot color, the disease is not resolving due to inability to sweat slightly, causing body itching. Guizhi Mahuang Ge Ban Tang (Half-and-Half Decoction of Cinnamon Twig and Ephedra) is suitable.
Guizhi (Ramulus Cinnamomi, peeled, 1 liang 16 zhu), Shaoyao (Radix Paeoniae Alba), Shengjiang (Rhizoma Zingiberis Recens, sliced), Gancao (Radix Glycyrrhizae Preparata), Mahuang (Herba Ephedrae, nodes removed, each 1 liang), Dazao (Fructus Jujubae, broken, 4 pieces), Xingren (Semen Armeniacae Amarum, soaked in hot water, skin/tips removed, 24 pieces).
Decoct the above seven ingredients. Use 5 sheng of water. First boil Mahuang 1-2 times, skim foam. Add other ingredients, decoct to obtain 1 sheng 8 he, remove dregs. Take 6 he warm. Originally stated as mixing 3 he of Guizhi Tang and 3 he of Mahuang Tang for a single dose; care follows the previous method. (Note by Chen Yi et al.: Based on calculations, this formula represents one-third of each original decoction, not half-and-half; it should be called He Ban Tang.)
24. In Taiyang Disease, if initial administration of Guizhi Tang causes vexation and no resolution, first acupuncture Fengchi (GB20) and Fengfu (GV16) points, then administer Guizhi Tang for recovery. (Use the first formula above.)
25. After taking Guizhi Tang, if profuse sweating occurs and the pulse becomes surging/large, administer Guizhi Tang as before. If symptoms resemble malaria with two episodes per day, sweating will resolve it; Guizhi Er Mahuang Yi Tang (Two-parts Cinnamon Twig One-part Ephedra Decoction) is suitable.
Guizhi (Ramulus Cinnamomi, peeled, 1 liang 17 zhu), Shaoyao (Radix Paeoniae Alba, 1 liang 6 zhu), Mahuang (Herba Ephedrae, nodes removed, 16 zhu), Shengjiang (Rhizoma Zingiberis Recens, sliced, 1 liang 6 zhu), Xingren (Semen Armeniacae Amarum, skin/tips removed, 16 pieces), Gancao (Radix Glycyrrhizae Preparata, 1 liang 2 zhu), Dazao (Fructus Jujubae, broken, 5 pieces).
Decoct the above seven ingredients. Use 5 sheng of water. First boil Mahuang 1-2 times, skim foam. Add other ingredients, decoct to obtain 2 sheng, remove dregs. Take 1 sheng warm, twice daily. Originally stated as mixing 2 parts Guizhi Tang and 1 part Mahuang Tang for 2 sheng, divided into two doses. Now combined into one formula; care follows the previous method.
26. After taking Guizhi Tang, if profuse sweating is followed by severe vexation, thirst, unresolved symptoms, and a surging/large pulse, Baihu Jia Renshen Tang (White Tiger Decoction plus Ginseng) is the main treatment.
Zhimu (Rhizoma Anemarrhenae, 6 liang), Shigao (Gypsum Fibrosum, crushed, wrapped in cotton, 1 jin), Gancao (Radix Glycyrrhizae Preparata, 2 liang), Jingmi (Semen Oryzae, 6 he), Renshen (Radix Ginseng, 3 liang).
Decoct the above five ingredients. Use 1 dou of water. Cook until rice is done. Remove dregs. Take 1 sheng warm, three times daily.
27. In Taiyang Disease, with fever and aversion to cold, more heat than cold, and a faint/weak pulse, this indicates Yang deficiency. Sweating must not be induced. Guizhi Er Yuebi Yi Tang (Two-parts Cinnamon Twig One-part Yuebi Decoction) is suitable.
Guizhi (Ramulus Cinnamomi, peeled), Shaoyao (Radix Paeoniae Alba), Mahuang (Herba Ephedrae), Gancao (Radix Glycyrrhizae Preparata, each 18 zhu), Dazao (Fructus Jujubae, broken, 4 pieces), Shengjiang (Rhizoma Zingiberis Recens, sliced, 1 liang 2 zhu), Shigao (Gypsum Fibrosum, crushed, wrapped in cotton, 24 zhu).
Decoct the above seven ingredients. Use 5 sheng of water. First boil Mahuang 1-2 times, skim foam. Add other ingredients, decoct to obtain 2 sheng, remove dregs. Take 1 sheng warm. Originally stated as combining Yuebi Tang and Guizhi Tang; now combined into one formula with 2 parts Guizhi Tang and 1 part Yuebi Tang.
28. After taking Guizhi Tang or purgation, if headache/stiffness in head/nape, mild fever, no sweating, fullness/slight pain below the heart, and difficult urination persist, Guizhi Qu Gui Jia Fuling Baizhu Tang (Cinnamon Twig Decoction minus Cinnamon Twig plus Poria and Atractylodes Macrocephala) is the main treatment.
Shaoyao (Radix Paeoniae Alba, 3 liang), Gancao (Radix Glycyrrhizae Preparata, 2 liang), Shengjiang (Rhizoma Zingiberis Recens, sliced), Baizhu (Rhizoma Atractylodis Macrocephalae), Fuling (Poria, each 3 liang), Dazao (Fructus Jujubae, broken, 12 pieces).
Decoct the above six ingredients. Use 8 sheng of water, decoct to obtain 3 sheng, remove dregs. Take 1 sheng warm. Recovery occurs when urination becomes free. Originally Guizhi Tang, now with Guizhi removed and Fuling/Baizhu added.
29. In Shang Han, with floating pulse, spontaneous sweating, frequent urination, vexation, slight aversion to cold, and calf spasms, mistakenly administering Guizhi Tang to attack the exterior is an error. This leads to cold reversal of limbs, dry throat, irritability, and vomiting/counterflow qi. Prepare Gancao Ganjiang Tang (Licorice and Dried Ginger Decoction) to restore Yang. If cold reversal resolves and feet become warm, prepare Shaoyao Gancao Tang (Peony and Licorice Decoction) to extend the legs. If stomach disharmony causes delirium, give a small amount of Tiaowei Chengqi Tang (Stomach-Regulating Order-the-Qi Decoction). If heavy sweating and burning needles were used, Sini Tang (Frigid Extremities Decoction) is the main treatment.
Gancao Ganjiang Tang: Gancao (Radix Glycyrrhizae Preparata, 4 liang), Ganjiang (Rhizoma Zingiberis, 2 liang). Decoct in 3 sheng water to 1.5 sheng, remove dregs, divide into two warm doses.
Shaoyao Gancao Tang: Baishaoyao (Radix Paeoniae Alba), Gancao (Radix Glycyrrhizae Preparata, each 4 liang). Decoct in 3 sheng water to 1.5 sheng, remove dregs, divide into two warm doses.
Tiaowei Chengqi Tang: Dahuang (Radix et Rhizoma Rhei, peeled, washed with clear wine, 4 liang), Gancao (Radix Glycyrrhizae Preparata, 2 liang), Mangxiao (Natrii Sulfas, 0.5 sheng). Decoct first three in 3 sheng water to 1 sheng, remove dregs, add Mangxiao, boil briefly. Take small amounts warm.
Sini Tang: Gancao (Radix Glycyrrhizae Preparata, 2 liang), Ganjiang (Rhizoma Zingiberis, 1.5 liang), Fuzi (Radix Aconiti Lateralis, raw, peeled, broken into 8 slices, 1 piece). Decoct in 3 sheng water to 1.2 sheng, remove dregs, divide into two warm doses. Strong individuals may use 1 large Fuzi and 3 liang Ganjiang.
30. Question: The syndrome resembled Yangdan, but treatment worsened it, causing cold reversal, dry throat, calf spasms, and delirium. The Master said: "Hands and feet will warm at midnight, and legs will extend." It happened as predicted. How was this known? Answer: The Cun Kou pulse was floating and large. Floating indicates Wind; large indicates Deficiency. Wind generates slight heat; deficiency causes calf spasms. The disease resembled Guizhi pattern, so Fuzi was added to warm the meridians due to Yang collapse. Cold reversal, dry throat, irritability, Yangming internal binding, and delirium were treated with Gancao Ganjiang Tang. At midnight, Yang returned, feet warmed. Calves remained slightly spasmodic, so Shaoyao Gancao Tang was given again to extend them. Chengqi Tang was used to loosen stools slightly, stopping delirium. Thus, recovery was predicted.
