The hip bone, also known as the coxal bone or condylar bone, features an outward-facing concavity shaped like a mortar (acetabulum), which accommodates the upper end of the femur (thigh bone), shaped like a pestle. This joint is named "Ji" or "Bi Shu," corresponding to the location of the Huantiao acupoint (GB30), commonly referred to in folk terms as the buttock joint (hip joint). If dislocation occurs, reduction is difficult because the gluteal muscles are large and thick, making it hard to grasp manually. It requires three or four strong assistants: the patient lies on their side; one person holds the body, another pulls the knee downward while extending, one hand presses the head of the femur to advance it step by step, and another hand rotates the thigh flexing it so the knee approaches the abdomen, then extends it again. If a popping sound is heard from the joint, reduction is successful. The displaced tendons should be pushed forward into place. Administer Sheng Xue Bu Sui Tang (Blood-Generating and Marrow-Nourishing Decoction), followed by Jia Wei Jian Bu Hu Qian Wan (Modified Tiger-Hiding Pill for Strengthening Steps). If the joint cannot be reduced, the buttock will protrude and gait will be slanted, resulting in lifelong disability. Exercise utmost caution!
The Da Jian bone, namely the femur, has its upper end shaped like a pestle inserting into the acetabulum of the Bi Shu, and its lower end shaped like a hammer connecting to the tibia/fibula (text implies lower leg bones). Collectively called "Gu," it is the general term for the two main branches of the lower limbs, commonly known as the thigh bone. Below it lies the patella (kneecap), also called Lian Hai or Bin Gu, which is round and flat, covering the ends of the femur and lower leg bones; it is a mobile bone. The tendons on the medial aspect pass upward from the thigh to the flanks and downward from the lower leg to the dorsum of the foot. The *Neijing* states: "The knee is the residence of the tendons." Injury may cause pain radiating upward to the waist with limited flexion, swelling moving downward to the lower leg, coldness and hardness of the foot and calf, slanted gait, or severe injury to the hip leading to later development of jaundice. Administer Zi Jin Dan (Purple Gold Pill), followed by herbs such as Yin Chen (Artemisiae Scopariae Herba) to treat jaundice and conclude therapy. If the patella is dislocated laterally, the medial tendons swell; if medially, the tendons become straight and swollen. Carefully examine how the bone is tilted and misaligned, apply manual manipulation techniques to restore position, and administer Bu Jin Tang (Tendon-Strengthening Decoction). For hip dislocation, one hand presses the knee while the other grasps the thigh, pulling vertically up and down, rotating the knee flexed until it contacts the patella area; if a joint sound is heard, reduction is achieved; administer Sheng Xue Bu Sui Tang. If the patella is fractured into two pieces, use silk vine fibers to make a hoop, bind with cloth strips, take four strands of raw thread, follow the method illustrated in the "Embracing Knee Diagram," mold with hands into a circle, secure the bone with the hoop, bind below the thigh, and lie flexed for over a month; administer Jie Gu Zi Jin Dan (Bone-Knitting Purple Gold Pill). If the fracture persists after binding, lifelong claudication will result, and it is incurable.
