厥阴肝气郁结，胸膈胀满、目昏，君火内迫，阴精滑泄，脉象不柔软，应当使用交通心肾并苦寒泄热的方法（兼有心悸过度，视为肝火所致，因服用过多补药无效）。

坎离（心肾）不交，惊恐自汗，近期兼有精液滑脱，精气神俱亏，脉象空虚无里。需要重剂频繁进补（足部痿弱行走困难）。
炙黄芪（三钱）、五味子（四分，炙）、茯神（二钱）、牡蛎（五钱）、黄柏（一钱）、贡干参（一两）、熟地（八钱）、麦冬（三钱）、枣仁（三钱）、淮山药（三钱）、湘莲（七粒）。
失血兼有精液滑脱，肝肾虚损，筋脉拘急而坐卧不宁，六脉细软无力，属于下虚而血不养肝。伴有腹泻和自汗，这是大虚的证候，必须用重剂培补。
炙黄芪（二钱）、北五味子（四分）、茯神（二钱）、金狗脊（一钱五分）、甘杞子（枸杞子，二钱）、熟地（五钱）、麦冬肉（二钱）、枣仁（三钱）、龙齿（二钱）、川郁金（一钱）。

咳血复发，近期兼有遗精滑泄，幸好脉不数、无气喘，推测阴分不至于大亏，乃是阴络受损。先理气止血，后行补益。

阴分不足，又兼湿火上冲，脑顶感觉下陷，遗精滑泄，督脉痿痛，采用清上焦、纳下元的治疗方法。
熟地、龟板、莲须、石决明、首乌（何首乌）、黄柏、茯苓、冬桑叶。
When Minister Fire (Xiang Huo) is hyperactive and accompanied by Damp-Heat pouring downward, it leads to erection with seminal emission. The six pulses are wiry and slippery. Treatment employs the method of nourishing Yin and clearing heat, assisted by bitter purgation.
Fresh Rehmanniae Radix (Sheng Di Huang), Lotus Stamen (Lian Xu), Red Poria (Chi Ling), Moutan Cortex (Dan Pi), Coix Seed (Yi Yi Ren), Phellodendri Cortex (Chuan Bai), Tortoise Plastron (Gui Ban), Dioscoreae Hypoglaucae Rhizoma (Bi Xie), Alisma Rhizome (Ze Xie), Licorice Root (Gan Cao).
The essence gate is not secure, causing flaccidity and pain in the waist and spine. The six pulses are fine, soft, and powerless. Astringing essence and warming tonification should be applied.
Fried Rehmanniae Radix Preparata (Chao Shu Di), Chinese Yam (Huai Shan Yao), Dipsacus Root (Chuan Duan), Schisandra Berry (Bei Wu Wei), Lotus Seed Meat (Xiang Lian Rou), Processed Atractylodes Macrocephala Rhizome (Zhi Yu Zhu), Rosa Laevigata Fruit (Jin Ying Zi), Eucommia Bark (Du Zhong), White Poria (Bai Fu Ling).
For the pill formula, remove Rosa Laevigata Fruit, add Codonopsis Root (Dang Shen), Dogwood Spine (Gou Ji), Fish Maw (Xian Yu Jiao), and Amomum Fruit (Sha Ren).
Jueyin Liver Qi stagnation causes chest and diaphragm distension and blurred vision. Minister Fire presses inward, causing Yin essence to leak. The pulse is not soft. The method of communicating Heart and Kidney with bitter purgation should be used (accompanied by excessive palpitations, viewed as Liver Fire, caused by excessive intake of tonifying medicines which proved ineffective).
Coptis Rhizome (Chuan Huang Lian), Gardenia Fruit (Hei Shan Zhi), Alisma Rhizome (Ze Xie), Lotus Stamen (Lian Xu), Biota Seed (Bai Zi Shuang), Processed Pinellia Tuber (Fa Ban Xia), Red Poria (Chi Ling), Curcuma Root (Yu Jin), Tangerine Leaf (Ju Ye).
Kan and Li (Heart and Kidney) do not communicate. There is fright and spontaneous sweating. Recently accompanied by seminal slipperiness. Essence, Qi, and Spirit are all deficient. The pulse is empty and powerless. Heavy doses administered frequently are required for tonification (flaccidity of feet making walking difficult).
Prepared Astragali Radix (Zhi Huang Qi, 3 qian), Schisandra Berry (Wu Wei Zi, 4 fen, prepared), Fuxen Sclerotium (Fu Shen, 2 qian), Oyster Shell (Mu Li, 5 qian), Phellodendri Cortex (Chuan Bai, 1 qian), Tribute Ginseng (Gong Gan, 1 liang), Prepared Rehmanniae Radix (Shu Di, 8 qian), Ophiopogon Tuber (Mai Dong, 3 qian), Ziziphus Seed (Zao Ren, 3 qian), Chinese Yam (Huai Shan Yao, 3 qian), Lotus Seeds (Xiang Lian, 7 grains).
Hemorrhage accompanied by seminal slipperiness indicates deficiency and damage to Liver and Kidney. Tendons are cramped, causing restlessness in sitting and lying down. The six pulses are fine, soft, and powerless. This belongs to lower deficiency where blood fails to nourish the Liver. Accompanied by diarrhea and spontaneous sweating, this is a severe deficiency syndrome requiring heavy-dose tonification.
Prepared Astragali Radix (Zhi Huang Qi, 2 qian), Schisandra Berry (Bei Wu Wei Zi, 4 fen), Fuxen Sclerotium (Fu Shen, 2 qian), Golden Dogwood Spine (Jin Gou Ji, 1.5 qian), Goji Berry (Gan Qi Zi, 2 qian), Prepared Rehmanniae Radix (Shu Di, 5 qian), Ophiopogon Tuber Flesh (Mai Dong Rou, 2 qian), Ziziphus Seed (Zao Ren, 3 qian), Dragon Teeth (Long Chi, 2 qian), Curcuma Root (Chuan Yu Jin, 1 qian).
Heart and Kidney do not communicate, leading to frequent dreams and seminal emission. There is a history of hematochezia. Black Guipi Decoction (Hei Guipi Tang) should be used for regulation.

Hemoptysis recurs, recently accompanied by seminal emission. Fortunately, the pulse is not rapid and there is no panting, suggesting that the Yin aspect is not severely depleted; rather, the Yin collaterals are damaged. First regulate and stop bleeding, then tonify.

Yin aspect is insufficient, accompanied by upward surging of Damp-Heat. There is a sensation of sinking at the vertex of the brain, seminal emission, and flaccidity pain in the Du Meridian. Treatment involves clearing the Upper Jiao and grasping the Lower Yuan.
