1. Ganlu Xiaodu Dan (Euphorbiae Seed and Scutellaria Decoction, formula by Ye Tianshi): Also known as Puji Jiedu Dan. Wang Mengying considered this the primary formula for treating damp-warmth seasonal epidemics. It can treat fever with fatigue, chest tightness and abdominal distension, limb soreness and throat swelling, macules and rashes with jaundice, parotid swelling with thirst, dark urine with constipation, vomiting, diarrhea, malaria, dysentery, stranguria, turbid urine, sores, and ulcers. For pathogenic factors of summer-dampness and seasonal epidemics located in the Qi level, presenting with a pale white, thick greasy, or dry yellow tongue coating, it is generally effective.
Formula composition and usage: Fei Huashi (Prepared Talcum, 15 liang), Mian Yinchen (Artemisiae Scopariae Herba, 11 liang), Dan Huangqin (Scutellariae Radix, 10 liang), Shichangpu (Acori Tatarinowii Rhizoma, 6 liang), Chuan Beimu (Fritillariae Cirrhosae Bulbus) and Mutong (Akebiae Caulis, 5 liang each), Huoxiang (Pogostemonis Herba), Shegan (Belamcandae Rhizoma), Lianqiao (Forsythiae Fructus), Bohe (Menthae Haplocalycis Herba), and Baidoukou (Amomi Fructus Rotundus, 4 liang each). Dry the herbs in the sun, grind them raw into powder. Take 3 qian per dose, mixed with boiling water, twice daily. Alternatively, make pills using Shenqu (Massa Medicata Fermentata) paste, size of a bullet, and dissolve in boiling water before taking.
Wang Mengying stated: After the Spring Equinox, the weather gradually warms; after the Grain in Ear solar term, the earth gradually becomes humid. Warmth and humidity steam upwards, compounded by the scorching heat of the sun, making stones melt and metal flow. Humans exist within the intersection of heaven and earth qi, inhaling these pathogens through the mouth and nose. If they remain and do not dissipate, they form diseases of damp-warmth and epidemic pestilence. In the initial stage when the pathogen is still at the Qi level, all can be treated with this pill, yielding immediate results.
2. Sanren Tang (Three Kernels Decoction, from *Wenbing Tiaobian*): Treats headache and aversion to cold, white tongue coating without thirst, wiry, fine, and soggy pulse, pale yellow complexion, chest tightness with lack of appetite, high fever in the afternoon, symptoms resembling Yin deficiency.
Formula composition and usage: Xingren (Prunus Armeniacae Semen, 3 qian), Fei Huashi (Prepared Talcum, 6 qian), Baotongcao (Caulis Tetrapanacis, 2 qian), Baokourén (Amomi Fructus Rotundus, 2 qian), Houpo (Magnoliae Officinalis Cortex, 2 qian), Sheng Yiyiren (Coicis Semen, 6 qian), Banxia (Pinelliae Rhizoma Praeparatum, 5 qian), Zhuye (Bambusae Folium, 2 qian), Ganlan Water (8 bowls). Decoct the above herbs in 8 bowls of Ganlan Water, reduce to 3 bowls. Take 1 bowl per dose, three times daily.
This formula is Wu Jutong’s first choice for treating damp-warmth. Since damp-warmth cannot be treated with excessive sweating, it employs light and clear methods. For mild cases, cure is often achieved; for severe cases, its strength may be insufficient. Wu Xihuang in *Zhongxi Wenre Chuanjie* stated that this formula does not fully match the initial stage of damp-warmth, fearing it might cause dryness and thirst. This view is not entirely accurate. If dryness and thirst truly occur, it indicates that internal dampness has been cleared, leaving only excess heat, which should then be treated solely by clearing heat. Although named "Three Kernels," the main herb is actually Huashi (Talcum), which directs dampness out via urination and induces slight sweating, hence its widespread use. However, Houpo (Magnoliae Officinalis Cortex) is somewhat questionable; Wu’s concern about causing dryness likely stems from this. My opinion is that if the goal is to treat fullness, regulate Qi, broaden the middle, and dispel dampness, removing Houpo and replacing it with Gualoupi (Benincasae Exocarpium) would be better.
3. Initial Stage Damp-Warmth Formula (Guhuan Shi formula from Wu Xihuang's *Zhongxi Wenre Chuanjie*): Treats the same syndromes as mentioned above.
Formula composition and usage: Danshidouchi (Sojae Semen Praeparatum, 3 qian), Peilan (Eupatorii Herba, 2 qian), Fei Huashi (Prepared Talcum, 4 qian), Cangzhu Pi (Atractylodis Rhizoma Cortex, 1 qian), Fuling Pi (Poria Cum Pinio Cortex, 3 qian), Chenpi (Citri Reticulatae Pericarpium, 2 qian), Huoxiang Ye (Pogostemonis Folium, 2 qian), Lianqiao (Forsythiae Fructus, 3 qian), Yinhua (Lonicerae Japonicae Flos, 3 qian), Tongcao (Tetrapanacis Medulla, 1 qian), Gancao (Glycyrrhizae Radix et Rhizoma, 8 fen), Zhuye (Bambusae Folium, 2 qian). (If there is aversion to cold and no sweat, add Xingren/Prunus Armeniacae Semen). These three formulas are all light and clear agents. However, in terms of formulation hierarchy, Ganlu Xiaodu Dan is first, as the powder form stays in the intestines longer. Sanren Tang is second, and this formula is third. It is preserved for reference only.
4. Jiwen Zushi Tang (Clear Warmth and Expel Dampness Decoction, Yu Wuyan’s empirical formula): Treats severe cases of initial damp-warmth. Sheng Mahuang (Ephedrae Herba, raw, decocted first) 2-3 qian, Sheng Shigao (Gypsum Fibrosum, raw) 2-3 liang, Fen Gegen (Puerariae Lobatae Radix, processed) 3-4 qian, Jing Lianqiao (Forsythiae Fructus, cleaned) 3-4 qian, Zhi Banxia (Pinelliae Rhizoma Praeparatum) 3-4 qian, Sheng Shanzhi (Gardeniae Fructus, raw) and Liuyisan (Six-to-One Powder) 2-4 qian each, Yiyiren (Coicis Semen) and Fuling Pi (Poria Cum Pinio Cortex) 4-5 qian each. After taking this formula, "slight continuous sweating" must be achieved. About two to three hours after slight sweating begins, take the second decoction continuously, ensuring slight sweating lasts over four to five hours. Then, exterior heat will resolve with the sweat. If severe aversion to cold occurs in the first one or two days, add Guizhi (Cinnamomi Ramulus) 1-2 qian. Do not add if there is no aversion to cold.
5. Qingwen Huashi Tang (Clear Warmth and Transform Dampness Decoction, Yu Wuyan’s empirical formula): Take this formula after the previous one, once exterior heat has subsided slightly.
Formula composition and usage: Sheng Shigao (Gypsum Fibrosum, raw, decocted first) 1-2 liang, Fen Gegen (Puerariae Lobatae Radix, processed) 3 qian, Jing Lianqiao (Forsythiae Fructus, cleaned) 3 qian, Sheng Huangqin (Scutellariae Radix, raw) 3 qian, Shang Chuanlian (Coptidis Rhizoma, upper grade) 1 qian, Jinwen Jun (Rhei Radix et Rhizoma, brocade pattern) 1-2 qian, Liuyisan (Six-to-One Powder) 4 qian, Sheng Shanzhi (Gardeniae Fructus, raw) 3 qian, Dongguapi (Benincasae Exocarpium) and Dongguazi (Benincasae Semen) 3 qian each, Chao Jiangmi (Zizaniopsis Aquatica Semen, stir-fried) one wine cup. The purpose of this formula is to achieve mild bowel movement and profuse urination. When Dahuang (Rhei Radix et Rhizoma) and Huashi (Talcum) are used together, their heat-clearing and detoxifying power acts partly through urination.
The aforementioned formulas show that various practitioners, unable to quickly resolve damp-warmth through sweating, strive for light and clear, clever approaches. This works well for mild cases at the initial stage. However, for severe cases, especially those with extreme chest tightness, previous formulas are less effective. One must base treatment on classical formulas (*Jingfang*) and modify them with contemporary formulas (*Shifang*). Following Zhang Zhongjing’s approach to wind-dampness, aiming for slight sweating and gradual downward drainage resolves both sweat and dampness, allowing damp-warmth pathogens to exit via two paths.
My first formula uses Mahuang (Ephedrae Herba) first. Some argue Mahuang should not be used in summer due to fear of excessive sweating harming the patient, and since it is acrid-warm, using it for damp-warmth seems like adding fuel to the fire. However, although Mahuang is acrid-warm, it does not cause excessive sweating unless combined with Guizhi (Cinnamomi Ramulus). Thus, MaXingShiGan Tang treats pneumonia, Mahuang Lianqiao Chixiaodou Tang treats jaundice, and Yebi Tang treats wind-water edema. Regarding Yebi Tang, the combination of Mahuang and Shigao (Gypsum Fibrosum) effectively treats edema. Japanese scholars note that Mahuang causes sweating when taken hot and diuresis when taken cold, illustrating the brilliance of Zhang Zhongjing’s combinations. Combining acrid-warm Mahuang with acrid-cool Shigao reduces its ability to open the skin but enhances its ability to unblock the Triple Burner. Thus, 30-40% of wind-water pathogens resolve slowly through the skin, while 50-60% drain downwards through the Kidney and Bladder. Even individuals with dense pores who catch wind-cold and take Mahuang may have no sweat but profuse urination, leading to recovery. Mahuang promotes excretion through both skin and bladder. Therefore, Mahuang must be used raw; water-processed Mahuang is ineffective. My prescription uses Yebi Tang as the backbone, assisted by Gegen (Puerariae Lobatae Radix), because Gegen clears heat in the stomach/intestines and cerebrospinal system, preventing meningitis. Zhang Zhongjing used Gegen for wind-cold presenting with stiff neck and back. With sweat, combine with Guizhi; without sweat, combine with Mahuang. Traditional theory says it clears heat from the Du Meridian, which corresponds to the cerebrospinal system. Clearing Du Meridian heat prevents cerebrospinal disease. Since damp-warmth high fever easily attacks the brain causing inflammation, Gegen is added for prevention. Lianqiao (Forsythiae Fructus) clears the heart/chest and cools the diaphragm. Banxia (Pinelliae Rhizoma) drains the heart/chest and promotes water-dampness metabolism, suitable for vomiting due to internal damp-heat. Liuyisan (Six-to-One Powder) and Shanzhi (Gardeniae Fructus) cool the diaphragm, clear heat, and drain via Kidney/Bladder. Huashi (Talcum) is widely used; Liuyisan contains Huashi as the main ingredient. Yiyiren (Coicis Semen) and Fuling Pi (Poria Cum Pinio Cortex) promote dampness drainage downwards and remove skin dampness. Fuling also tonifies Qi. This combination integrates etiological and symptomatic treatments. My second formula focuses entirely on clearing interior heat. Detoxifying and heat-clearing herbs act via mild bowel movement with Dahuang (Rhei Radix et Rhizoma) and direct urination with Shanzhi (Gardeniae Fructus) and Huashi (Talcum)—this is the method of separating and draining. I have cured many patients this way. Achieving slow slight sweating, mild bowel movement, and smooth urination removes dampness and heat, curing the disease. This applies only to the initial stage. If delayed and mistreated, complications arise, requiring different treatments. Dosages vary based on constitution and severity; rigid formulas lead to errors.
Discussion on "Yongding Bai Xiao Wan" (Carbuncle and Carbuncle Hundred-Efficacy Pill): Originally named Dingchuang Wan or Badou Erhuang Wan. I use it frequently with 100% success. Extending its use to all carbuncles, boils, and swellings yields miraculous effects. Given its efficacy and my experience, I renamed it Yongding Bai Xiao Wan.
Dingchuang Wan was originally a formula by Qing dynasty physician Lu Chengyan, also recorded in Chen Xiuyuan’s medical books. It uses Badou (Crotonis Fructus, peeled and membrane removed) 3 qian, Ming Xionghuang (Realgar, bright) 3 qian, Sheng Dahuang (Rhei Radix et Rhizoma, raw) 3 qian.
Grind each into fine powder, then mix thoroughly. Add refined flour and vinegar paste to form pills the size of Sophora seeds. Mild cases: 4-5 pills per dose. Severe cases: 7-8 pills per dose. Extremely severe cases or spreading carbuncles: 10-12 pills, taken with plain water. Ensure the patient experiences 3-5 large bowel movements for cure. For weak constitutions, after 2-3 bowel movements, give cold boiled water or thin porridge to stop diarrhea immediately. Each bowel movement reduces pain and swelling. Even spreading carbuncles can be saved. This is a specific effective formula for carbuncles. Four cases follow as evidence.
1. Teacher Pei Ziliang suffered from "Hand Back Carbuncle" (colloquially "Hand Da"), with red swelling, burning pain, aversion to cold, and fever. External plasters failed. On the second day, I gave him 6 pills of Yongding Bai Xiao Wan. He had 4-5 large bowel movements; the last resembled phlegm. Swelling and pain vanished, and he recovered.
2. Mr. Liu, age 36, developed a Back Carbuncle for 4-5 days. Red halo like a plate, burning pain like fire. I urgently administered Yongding Bai Xiao Wan. He had 7-8 large bowel movements. Swelling disappeared instantly, pain reduced. By the next day, it almost completely resolved. I then gave heat-clearing and detoxifying herbs. He recovered after several doses.
3. Mr. Wang, early 20s, had a wet sore on the buttock, finger-sized, red, swollen, hot, painful, with much pus. Treatment by other doctors for one month failed. I predicted a one-week course. First, he took Yongding Bai Xiao Wan, having multiple large bowel movements. Pain and swelling decreased. Then, blood-cooling, detoxifying, and dampness-draining herbs were given. He recovered in seven days.
4. My third sister frequently suffered from Throat Carbuncles since her teens. The last cure prevented recurrence for over ten years. Last autumn, it recurred. Throat swelling blocked, unbearable pain, inability to eat for six days, only sipping tea. I diagnosed Heat-Toxin attacking upwards and urgently gave 8 pills of Yongding Bai Xiao Wan, crushed and slowly poured down with boiling water.
She felt heart-burning and anxiety. I told her she would feel lighter after bowel movement. Soon, she passed much dry stool. After 6-7 bowel movements, pain ceased, throat swelling subsided. Next morning, she ate a large bowl of porridge. Subsequent decoctions cleared residual toxins. She recovered in a few days.
Origin of Chan Yi Jiu (Cicada Slough Wine) for Treating Tetanus: I enjoy collecting diverse medical texts. Seven or eight years ago, I saw a "Broken Injury" formula in *Fu Qingzhu Nüke*. It was unremarkable. The text read: "Remove feet/tips of Cicada Slough, grind to 5 qian powder. Mix with one bowl of good wine, boil, add powder, stir, and drink for immediate life." I scrutinized it. Cicada Slough disperses wind and clears heat; using it for "broken injury" seemed inappropriate. The claim "drink for immediate life" for fatal bleeding injuries contradicts medical logic. Reflecting deeply, I realized: This must be Tetanus. The printer omitted the character "Wind". I noted it for verification.
Case 1: Liu Shaochu, a rickshaw puller, was hit by a truck on Hatong Road in November 1931 (Republic Year 20), injuring his skull and nose bridge, with significant bleeding and bone damage. Taken to a hospital on Baik Road. The next day, his father brought him home due to the hospital’s warning of critical condition. Police ordered transfer to a hospital on Haige Road. After three weeks, wounds healed, and he was discharged. Before discharge, he felt mentally unwell. At home, he developed fever and restlessness. Symptoms worsened the next day: spasms, rigidity, lockjaw, opisthotonos,舟-shaped hard abdomen, shortness of breath. Tetanus symptoms were complete. The wound reopened. I was consulted. I prescribed 5 qian of Cicada Slough powder, taken with yellow wine to induce sweating. He sweated profusely with a foul odor for nearly two hours.
He felt comfortable, spasms stopped. The next day, I gave Badou Erhuang Wan (12 pills) to purge. He passed foul, black, glue-like stool, feeling refreshed. Unexpectedly, his father stopped consulting me. Ten days later, news reported Liu’s death and a lawsuit between his father and the truck driver. Another ten days later, Liu’s relative told me: Liu had a mistress long associated with his father. His father, having another son in his hometown, hoped for Liu’s death to inherit the widow and receive compensation money. Thus, he was dissatisfied with my effective treatment, using it merely as a cover. Delay and relapse led to death. The lawsuit resulted in the judge accusing fraud; the father claimed the body and lost everything. I was angry that my therapy was not completed and vowed to try again later.
Case 2: Mrs. Zhang, over 60, living on Ximen Road. In April 1932 (Republic Year 21), she fell down stairs, injuring her head, with deep cuts and bleeding. Doctors treated her, and she was healing. Suddenly, tetanus symptoms appeared, fluctuating in severity. Sent to a major hospital, ineffective, symptoms worsened. Hospital declared incurable. Brought home, consulted many Western and Chinese doctors, all deemed incurable. Coffin prepared. My relative Han, friends with Zhang’s son, introduced me. I examined pus in the wound, flushed/swollen face resembling erysipelas, locked jaw, stiff neck/back, spasms, opisthotonos, drum-like abdomen, dark blue-black abdominal skin, hard to touch, panting. I said: "Critical condition, let me try." Prescribed 5 qian Cicada Slough powder mixed with wine. Force-fed due to locked jaw. Sweated profusely, foul-smelling, sticky like glue for nearly two hours. Face swelling subsided, except chin/mouth. Abdomen still drum-like. Gave 12 Badou Erhuang Wan pills. Passed much sticky black stool, ball-like, glue-like, jelly-like. After 5-6 times, stopped with cold thin porridge. Abdominal hardness reduced significantly, breathing eased. Next day, 3 qian Cicada Slough powder induced similar sweating. Facial redness/swelling cleared. Third day, 9 Badou Erhuang Wan pills passed more sticky black stool. Abdominal distension gone, non-tender. Sat/stood well. Took conditioning herbs and fully recovered.
Subsequent patients treated also showed miraculous effects.
Note: Badou Erhuang Wan is the same as Yongding Bai Xiao Wan.
Treatment of Baihui Ju (Abscess at Baihui Point): *Yizong Jinjian · Waikexinfa* records: "Baihui Ju develops at the top center of the head, opposite the ear tip when viewed from the side. Initially millet-sized; gradually swells, root coin-sized, grape-shaped, iron-hard, high-pointed red/swollen, hot/painful, root condensed, aversion to cold/high fever, extreme thirst, constipation, irritability. Surging-rapid pulse indicates Excess; flat/dark/hard root dispersed, aversion to cold/diarrhea, fine-rapid pulse indicates Deficiency. If swelling connects to ears/neck, rattling phlegm sound, no pus after seven days leads to death." Hu Gongbi said: "Toxins around Baihui point easily invade the brain marrow. Once brain marrow collapses, it is hardest to treat." As a severe surgical case, treatment in the first 2-3 days before suppuration involves shaving hair, disinfecting, applying Yiyang San externally, and taking Xingxiao Wan internally with hot aged wine (3 qian). Non-drinkers use half water/half wine. Pain stops, sleep follows. Next day, skin wrinkles, one more dose resolves it. If suppuration occurs after 4-5 days, still use Xingxiao Wan to reduce surrounding hardness, stopping toxin/pain. Even with pus, output is minimal. This is "large turning to small" method. For robust patients, Badou Erhuang Wan can also resolve it. Formulas and usage below.
1. Yiyang San (Formula from *Waik Zhengzhi Quan Shu*): Treats carbuncle/toxin red swelling, burning heat and pain. Tianhuafen (Trichosanthis Radix) 3 liang, Jianghuang (Curcumae Longae Rhizoma), Xiang Baizhi (Angelicae Dahuricae Radix), Chishao (Paeoniae Radix Rubra) 1 liang each. Grind to fine powder. Apply with egg white or vinegar.
2. Xingxiao Wan (Formula from *Waik Zhengzhi Quansheng Ji*): Treats all carbuncles/sores/carbuncles/boils, instantly reducing swelling and pain. Rushi (Boswelliae Resina) and Moyao (Myrrhae Resina, oil removed) 1 liang each, Shexiang (Moschus) 1.5 qian, Xiongjing (Realgar, purified) 5 qian. Grind each to fine powder. Huangmi (Oryza Sativa Var. Glutinosa) 1 liang. Weigh Rushi, Moyao, Xiongjing accurately, mix with Shexiang powder. Mash Huangmi, mix with powder, knead into pills size of radish seeds. Sun-dry. Take 3 qian per dose with hot aged wine. Cover to sweat. When sober, swelling/pain cease.
3. Yongding Bai Xiao Wan (Lu Chengyan’s Formula): Treats all carbuncles/carbuncles, for robust constitutions. Badou (Crotonis Fructus, peeled) 3 qian, Ming Xionghuang (Realgar, bright) 3 qian, Sheng Dahuang (Rhei Radix et Rhizoma, raw) 3 qian. Grind each to fine powder, mix thoroughly. Add refined flour and vinegar paste to form pills size of Sophora seeds. Mild cases: 5-6 pills. Severe cases: 7-8 pills. Take with plain water. After 3-4 bowel movements, stop with a small bowl of cold porridge soup.
