After returning home, the fever remained largely unchanged, and the symptoms of mental restlessness and thirst persisted as before. At 7:00 a.m. the next morning, I was invited to examine the patient. Observing his symptoms and inquiring further, I learned that he had eaten a ball of sticky rice cake the previous morning, which likely caused the rise in temperature, leading to shock in the evening. I informed them, "This is a warm-heat disease (Wenbing). Counting from the onset, the fever has lasted for three days. In two more days, there will surely be faint red rashes on the chest." Upon unbuttoning the clothes to inspect, indeed, a few scattered granules were found on the chest, hidden beneath the skin and not obvious. I told them, "This condition is quite severe. I am prescribing a formula for you to take as directed. If bowel movements occur, sweating ensues, and the fever subsides, the rashes will also recede. If the fever persists, the rashes will continue to emerge and multiply; this corresponds to what Western medicine calls typhus with maculopapular rash." Therefore, I prescribed Yi Jie Si Qing Tang (One-Solution Four-Clearing Decoction) for him. I instructed that the herbs must be decocted at home to be effective; otherwise, if entrusted to a pharmacy for pre-decoction, it would certainly be ineffective, and thus I explained the decoction method.
After I left, the family followed the instructions. One hour after ingestion, the patient gradually became calm, with slight perspiration on the body. Approximately two hours later, he had another bowel movement, passing stool twice consecutively, with profuse sweating on the skin. By noon, after taking the second decoction, he had two more bowel movements, and the fever subsided by about seventy to eighty percent. The next morning, the family invited me again. The father led me upstairs and saw that Geng Di was sitting up organizing his schoolbag. His father exclaimed in surprise, "You just had your fever break, why are you exerting yourself mentally like this?" Geng Di replied, "There is a quiz in two days, so I am preparing." Hearing this, I smiled and said, "The quiz can be made up later; physical health is paramount. Do you not remember the shock you experienced the night before? Please do not overexert yourself." After examination, I slightly modified the previous prescription, and after one more dose, the patient stabilized.

Yangming Heat Disease Transmitting to Shaoyang Syndrome: A woman suffered from a heat illness at the beginning of late summer. Initially treated with Yin Qiao San (Honeysuckle and Forsythia Powder) by physicians, it was ineffective. Seeing her irritability, she was treated with Zhi Zi Chi Tang (Gardenia and Fermented Soybean Decoction), which also failed. I examined her high fever with spontaneous sweating, dry mouth, crimson tongue, intense thirst, reddish hue to the skin resembling intoxication, hot to touch, restlessness, yellow-red urine, and a flooding, rapid pulse. I cleared the heat using Bai Hu Ren Shen Tang (White Tiger with Ginseng Decoction) plus Tian Hua Fen (Trichosanthis Radix), after which the fever subsided and spirit calmed. Three days later, residual pathogenic factors transmitted to Shaoyang, presenting with alternating chills and fever, bitter taste in the mouth, dry throat, pain in the hypochondrium, and irritability with desire to vomit. I administered Xiao Chai Hu Tang (Minor Bupleurum Decoction) plus Bing Lang (Arecae Semen) and Huo Xiang (Agastaches Herba); after two doses, she recovered.
Zhu Meizhen, aged over 30, residing at Baxian Bridge, Baxian Fang, contracted a heat illness in mid-June. Initially, no physician was consulted. After two days of persistent fever and worsening condition, a doctor was summoned. He was given Yin Qiao San; one dose was ineffective, and a second dose also failed. Another doctor was consulted who, noting her irritability, prescribed Zhi Zi Chi Tang, which was also ineffective. Her husband's friend, Gu Yuzhi, suggested inviting me. As I ascended the stairs, before entering the room, I heard the patient loudly say, "If only I could drink ice water, I would thank heaven and earth!" Hearing this, I immediately recognized it as a Bai Hu Tang syndrome. Upon entering, four or five people stood around comforting her, saying, "The doctor is here; please let him examine you. Do not drink ice water." After sitting down, I observed her face and skin were red, resembling intoxication, with sclera showing watery redness. Her skin was hot to the touch, covered in sweat that wetted my hand, and she was restless. During pulse diagnosis, due to restlessness, she pulled her hand away three times; all six pulses were flooding and rapid. I asked, "How many days have you desired cold drinks?" She replied, "I wanted to drink more since the day before yesterday, especially these past two days, and now I crave ice water." Her husband said, "Other doctors said cold drinks are forbidden, as they might trap the pathogen inside. So I dared not give it, but my wife insists. What should I do?" I said, "If you had snow stored from winter, I could prescribe it, but it is hard to obtain now. It is early June; fresh watermelons are available. Buy some and feed them to her." I instructed her to buy a watermelon first, then prescribed Bai Hu Ren Shen Tang plus Tian Hua Fen, advising to decoct Shi Gao first, then add other herbs, before departing. After eating the watermelon, her irritability lessened. Following the decoction method, she sweated profusely at first, then less; fever rose initially then fell. Within three hours, her temperature normalized, skin redness faded, and irritability ceased completely. Taking the second decoction that evening, she slept soundly, and by morning, the illness had vanished.
I did not follow up the next day. Two days later, the family invited me again, stating, "She recovered after the previous medication, so we didn't trouble you. But since last night, she suddenly developed alternating chills and fever. When feeling cold, she wants blankets; when hot, she discards them. This has repeated three or four times during the night and morning. Could it be malaria?" I explained, "It is nothing else. After the previous dose broke the fever, residual Yangming heat was not fully cleared, transmitting to Shaoyang. It resembles malaria but is not true malaria. If I had continued treatment then, clearing residual pathogens with two doses, this complication would have been avoided." Her pulse was wiry and rapid; other symptoms matched the description above. I prescribed Xiao Chai Hu Tang plus Bing Lang and Huo Xiang; after two doses, she recovered. Subsequently, I prescribed regulating formulas to support healthy qi.
[Bai Hu Ren Shen Tang Plus Tian Hua Fen Formula]: Sheng Shi Gao 3 liang, Fei Zhi Mu 6 qian, Zhi Gan Cao 3 qian, Xi Dang Shen 5 qian, Tian Hua Fen 4 qian.
[Xiao Chai Hu Tang Plus Bing Lang and Huo Xiang Formula]: Chun Chai Hu 3 qian, Sheng Huang Qin 3 qian, Xi Dang Shen 4 qian, Jiang Zhi Ban Xia 3 qian, Zhi Gan Cao 3 qian, Hua Bing Lang 3 qian, Guang Huo Xiang 3 qian, Sheng Jiang 1 qian, Hong Zao 10 pieces.
Heat Illness Yangming Meridian Syndrome: A woman contracted a heat illness in mid-summer. Initially experiencing chills, she quickly stopped feeling cold. By the fourth day, the condition worsened. She lay naked on the floor, restless, tossing and turning, with red eyes and skin resembling intoxication, profuse pearl-like sweating, intense thirst, flooding and rapid pulse, and a crimson, dry tongue. She was first given watermelon juice, followed by Bai Hu Ren Shen Tang plus Tian Hua Fen.
In Shanghai, a Mrs. Chen, aged 42, contracted a heat illness three days before the Dragon Boat Festival in May. This aligns with the *Neijing* statement: "Illnesses before Summer Solstice are Warm diseases; illnesses after are Heat diseases." Initially chills and fever, she soon experienced only fever without chills, with continuous sweating. By the third day, she sweated profusely as if washed; other treatments failed. On the fourth day, I was called. She was lying supine on a cement floor, naked except for shorts, skin red like intoxication, eyes congested, restless, tossing and turning without peace, sweating profusely, limbs slightly cold, but chest burning hot, intense thirst craving ice water. Pulse was flooding and rapid, faint and hollow upon heavy pressure. Tongue was crimson and dry, lacking moisture.
I identified it as a Bai Hu Tang syndrome. However, preparing and decocting the medicine takes time, so I ordered her to buy an old-season watermelon, extract the juice, and drink it freely. I prescribed Bai Hu Ren Shen Tang plus Tian Hua Fen, instructing to decoct Shi Gao first, then add other herbs and Jing Mi (Oryza Sativa). Once the rice was cooked and the soup ready, filter and administer. After drinking the watermelon juice, she expressed great relief, and sweating decreased. After taking the herbal decoction, within two hours, sweating ceased, fever broke, and irritability stopped. After the second decoction, she slept through the night. By morning, the illness had disappeared.
[Bai Hu Ren Shen Tang Plus Tian Hua Fen Formula]: Sheng Shi Gao 6 liang, Fei Zhi Mu 8 qian, Zhi Gan Cao 3 qian, Xi Dang Shen 4 qian, Tian Hua Fen 4 qian, Jing Mi 1 liang.
Heat Illness with Maculopapular Rash Syndrome: Five days into a summer heat illness, maculopapular rashes appeared, mixed in size, red with purple hues, accompanied by persistent high fever, spontaneous sweating, mental restlessness, dry mouth and thirst for cold drinks, crimson dry tongue, red sclera, and flooding, rapid pulses. Factory doctors were ineffective. I instructed giving watermelon juice first, then administering Bai Hu Ren Shen Tang plus Ge Gen, Di Huang, Shi Hu, and Shan Zhi. After one dose, fever broke, irritability ceased, and rash color lightened. Learning that bowels were unopened and epigastric fullness with mild irritability existed, I used Zhu Ye Shi Gao Tang combined with Tiao Wei Cheng Qi Tang. Bowels opened, heat cleared, and she recovered.
Yu, a worker managing the kitchen at Zhonghua Weaving Factory near Sanguan Tang Bridge, Caojiadu, contracted a heat illness in late summer 1952. Traditional Chinese Medicine classifies heat illnesses occurring between Summer Solstice and Start of Autumn based on seasonal timing. Initially slight chills, then temperature rose above 40°C without chills, accompanied by headache and limb soreness. Factory doctors were ineffective. Next day: dry mouth and thirst, internal heat like fire. Third day: increased irritability and restlessness. Fourth day: appearance of maculopapular rashes, mixed sizes, severe distress. Despite penicillin and chloramphenicol treatment, the condition worsened daily.
On the fifth day, a coworker shopping at Yuchang Kiln Goods Store mentioned the case, suggesting hospital transfer. The owner advised, "This is likely Typhoid/Shaoyin disease; only Dr. Yu can treat it effectively and with certainty." With permission, the coworker invited me. Entering the factory, medical staff watched me enter. Upstairs, the patient lay on bed, naked waist and shorts, restless, forcing calm upon my entry. For left pulse, he held chest with right hand; for right pulse, left hand on chest, indicating heart irritability. Pulse was flooding and rapid; tongue dry and crimson; sclera red with blood vessels; skin showed mixed red-purple rashes. Thirst for cold drinks confirmed interior-exterior heat. I ordered a large watermelon for juice. Prescribed Bai Hu Ren Shen Tang as main formula, adding Ge Gen, Sheng Di, Shi Hu, and Shan Zhi. One dose taken as directed.
I reassured him it was harmless and to take medicine calmly. After one dose, 60% of the illness resolved.
Next day's follow-up: High fever reduced, irritability calmed, rash color faded from purple-red. Pulses smoothed. Patient reported lingering epigastric fullness and mild irritability. Bowels unopened for four days. Tongue crimson edges faded, root thin greasy with dry yellow coating. Changed to Zhu Ye Shi Gao Tang combined with Tiao Wei Cheng Qi Tang. After heavy use of Bai Hu, switching to Zhu Ye Shi Gao allows for mild purgation amidst cooling. After medication, bowels opened, symptoms resolved, yellow coating cleared, appetite returned. Third visit: Instructed to brew Zhu Ye, Shi Gao, and Yi Yi Ren as tea for three days. Additionally, Xiang Sha Liu Jun Zi Tang plus Shen Qu and Mai Ya to benefit qi and harmonize stomach.
[Bai Hu Ren Shen Tang Plus Ge Di Hu Zhi Formula]: Xi Dang Shen 4 qian, Sheng Shi Gao 4 liang, Fei Zhi Mu 6 qian, Fen Ge Gen 5 qian, Xian Sheng Di 6 qian, Xian Shi Hu 3 qian, Sheng Shan Zhi 3 qian, Zhi Gan Cao 2.5 qian, Chao Jing Mi 1 liang.
[Zhu Ye Shi Gao Tang Combined with Tiao Wei Cheng Qi Tang Formula]: Xian Zhu Ye 1 liang, Sheng Shi Gao 2 liang, Mai Men Dong (decored) 4 qian, Lu Dang Shen 3 qian, Jin Wen Jun 2 qian, Yuan Ming Fen 2 qian (taken separately), Zhi Gan Cao 2 qian, Chao Jing Mi 5 qian.
Severe Vomiting in Pediatric Heat Illness: A 4-year-old boy suffered from heat illness. Restless, frequent vomiting preventing medication intake, high fever, no sweating, hot skin, red eyes, dry mouth, scorched lips, dental plaque, offensive breath, epigastric tenderness, constipation, slightly cold extremities. Six or seven doctors failed. I instructed sleeping on a damp surface overnight; vomiting stopped, slight sweat appeared. Then treated with Cheng Qi Zeng Ye Fa plus Ge Gen, Huang Qin, and Shi Gao.
Young son of Mr. Cai Shengnan on Shunchang Road, Shanghai, aged 4, born of a concubine, dearly loved. Accustomed to miscellaneous foods, parents did not restrict him. Late April, post-Summer Solstice, he contracted a heat illness (called Heat Disease, not Warm Disease, per TCM theory). Friend Ma Shunkang introduced me. Upon ascending stairs, family was weeping.
The child on bed was uneasy, restless, vomiting frequently, even rejecting small sips of boiled water. Sweat on forehead only, neck and body dry and hot. Red eyes, dry mouth, scorched lips, offensive breath, epigastric pain, cold extremities. Illness duration: 4 days. Bowels unopened for 4 days. Previous medications failed as nothing could stay down. Seven Western and Chinese doctors visited, all ineffective due to inability to administer drugs.
After contemplation, I asked Mr. Cai, "Do you still want this child?" Mr. Cai wept, "How could I not want him! What do you mean?" I said, "If you don't want him, I have a method to try; if you do want him, I have no method." Mr. Cai asked for meaning. I said, "Your son's severe vomiting prevents drug intake. If vomiting stops, there is hope. I have a method, but you may not agree. Without my advice, danger is certain." Mr. Cai asked what method. I said, "If you truly don't want him, place him on the ground, preferably mud. Since Shanghai has no mud, clean a cement floor with water. Wet a large towel, place it on the cement. Lay your son on it, allow him to toss and turn overnight. Check tomorrow. But I fear you cannot bear it." Mr. Cai asked meaning. I said, "Condition is critical; now is not time for theory. Believe as you wish." Soon, Mr. Cai called servant, "Come, do as Mr. Yu says, quickly prepare." Servant cleaned cement floor, placed wet towel. Mr. Cai laid son supine. Initially restless, after 15 minutes, irritability decreased. I left, unaware of subsequent events. Next day 9 a.m., invited back. Mr. Cai joyfully reported, "After sleeping until 10 p.m., vomiting stopped, slight sweat appeared, fever subsided, slept peacefully, mind clear." Crisis passed, pulse smoothed. Prescribed Zeng Ye Cheng Qi Tang plus Ge Gen, Huang Qin, and Sheng Shi Gao. After bowel movement, foul odor, slight sweat. Reduced dose for second administration, multiple bowel movements ensued. Interior-exterior heat cleared, illness resolved miraculously.
Later, Mr. Cai asked, "You saved my son's life. Can you explain the medical principle?" I said, "Now I can tell you. Charcoal in furnace burns briefly to ash. If lit charcoal is placed on damp land, it extinguishes quickly, yet remains charcoal. Why? Dampness absorbs fire energy; fire absorbs dampness. Water-fire exchange extinguishes fire. Your son's condition is similar. Warm-heat transmitted internally, combining with Stomach excess. Middle jiao blocked, Stomach qi rebellious, causing vomiting. Unless heat or excess is removed, vomiting continues. Since drugs cannot be taken, shall we let him die? Thus, I used this method. Diagnosed as Heat-Excess syndrome, daring to place him on cold damp ground. Indeed, lying down broke fever; breaking fever stopped vomiting. Then Cheng Qi Tang seized established excess, Zeng Ye method rescued depleted yin. Hence efficacy. If you refused to place him on damp ground, he would surely have died." Mr. Cai was shocked and convinced. This became known as a miraculous cure.
[Zeng Ye Cheng Qi Tang Plus Ge Gen, Huang Qin, Shi Gao Formula]: Run Yuan Shen 2 qian, Da Mai Dong (decored) 2 qian, Xi Sheng Di 3 qian, Jin Wen Jun 1.5 qian, Yuan Ming Fen 2 qian, Fen Ge Gen 2 qian, Sheng Huang Qin 1.5 qian, Sheng Shi Gao 6 qian.
Postpartum Heat Illness (1): Postpartum heat illness rapidly transforms into dryness. Reddish face, red eyes, cracked lips, scaly tongue coating with purple-crimson edges, desire for cold food, restlessness, occasional delirium, epigastric tenderness, hidden chest rashes, dry skin, constipation, scanty dark urine, hiccups. Treated with modified Bai Hu Cheng Qi Zeng Ye Fa.
Mrs. Wang, 26, living in Sanpai Lou, Shanghai City, experienced persistent postpartum fever in June, unresponsive to various doctors. Invited me after 20+ days. Patient had reddish face, red eyes, peeling cracked lips, scaly tongue with purple-crimson edges, dry fluids, dry mouth craving cold water, occasional delirium, restlessness, epigastric pain, soft abdomen, hidden chest rashes, dry dull skin, forehead sweat, deafness from high fever, multi-day constipation, scanty dark urine, burning chest, cold extremities, hiccups. Seeing this, I felt uncertain. Postpartum state, deficiency with excess pathogens, making medication very difficult.
