Mild cases manifest as eruptions occurring in two or three batches, varying in size, sparse on the head and face, absent from the chest and eyes, with roots that are red, vibrant, plump, and glossy, resembling water droplets; accompanied by no thirst, no diarrhea, and no restlessness.
Severe cases manifest as simultaneous eruption, dense as silkworm eggs, with depressed apices resembling cornels (Cornus officinalis), or flat-topped and grayish-white; accompanied by thirst, diarrhea, restlessness, warmth of the head but coldness of the feet, and persistent body heat. If treated according to proper methods, eight or nine out of ten cases will be cured.
The seven errors causing a transition from mild to severe are: failing to observe dietary restrictions and being careless with food intake, thereby injuring the Spleen and Stomach; previous diarrhea leading to interior deficiency, preventing the pathogenic toxins from being expelled; exposure to wind and cold—whereas the Qi of spring and summer is favorable and that of autumn and winter is unfavorable, it is strictly taboo to catch a cold and damage the exterior; engaging in sexual intercourse; administering cooling medicines instead of those that tonify Blood and Qi and strengthen the Spleen; using cooling and purgative agents leads to interior cold, preventing toxic Qi from being attacked and expelled; contact with foul odors affects Blood and Qi—pleasant scents promote smooth progression, while foul odors hinder it; smooth progression facilitates eruption and crusting, while hindrance makes recovery difficult; allowing strangers to enter, fearing that outsiders coming from childbirth chambers or funeral sites, or those who are drunk or have consumed fishy/foul foods, bring harmful foul influences through contact.
The five precautions for transitioning from severe to mild are: carefully avoiding wind and cold; even drafts within the room should be avoided, except in summer, but if strong winds cause chilliness, avoidance is necessary; maintaining consistent warmth—adding clothes when cold, removing them when hot, striving for balance without excess or deficiency; regulating diet—avoiding cold items such as persimmons, oranges, watermelons, water caltrops, and honeyed sweets to prevent internal injury to Stomach Qi; especially avoiding fatty meats and greasy foods to prevent diarrhea; avoiding salty foods to prevent thirst; avoiding fishy substances such as alcohol, green onions, garlic, fish, and mutton to prevent pruritus of sores. It is essential to ensure the sufficiency of the Spleen and Stomach, so that sores erupt and crust easily. Thick stools indicate harmonious diet and no diarrhea. Once daily is considered regulated; two to three times daily is considered unobstructed; once every three to four days is considered constipation. By following the prescription, avoiding wind and cold, regulating diet, and prescribing medication based on detailed syndrome differentiation, one can hopefully prevent mild cases from becoming severe.
