Pregnant women who suffer miscarriage and uncontrollable metrorrhagia due to sexual intercourse are often thought by people to be caused by extreme fire agitation. Who would know that this is actually due to Qi collapse! Metrorrhagia originates from Qi deficiency, while excessive fire stems from Kidney Water depletion. Once Kidney Water is depleted, the source of Qi generation becomes dry; once the Qi source is dry, how can Qi fail to collapse! This fire agitation is the branch (manifestation), while Qi collapse is the root (essence). The Classics state: "In treating disease, one must seek the root." When the root is solidified, the branch will naturally stabilize. If one only focuses on stopping bleeding without urgently consolidating Qi, the Qi will scatter and cannot return quickly, so how can the blood stop! If Jing (Essence) is not greatly supplemented, the water source will not grow rapidly, and the fire will become even more blazing. Not examining the root but merely addressing the branches, I have never seen such cases successfully treated.
The formula used is Guqi Tianjing Tang (Solidify Qi and Fill Essence Decoction).
Ren Shen (Ginseng Radix et Rhizoma, 1 liang), Huang Qi (Astragali Radix, 1 liang, raw), Bai Zhu (Atractylodis Macrocephalae Rhizoma, 5 qian, stir-fried with soil), Shu Di Huang (Rehmanniae Radix Praeparata, 1 liang, steamed nine times), Dang Gui (Angelicae Sinensis Radix, 5 qian, washed with wine), San Qi (Notoginseng Radix et Rhizoma, 3 qian, ground into powder and taken with decoction), Jing Jie Sui (Schizonepetae Spica, 2 qian, stir-fried until black).
Decoct in water and take orally. After one dose, bleeding stops; after two doses, the body feels at ease; after four doses, full recovery is achieved. The brilliance of this formula lies not in clearing fire, but solely in tonifying Qi and Jing. Its unique efficacy arises because these herbs are warm and moistening, capable of eliminating great heat. Since this heat is deficient heat, tonifying Qi naturally controls bleeding, and tonifying Jing naturally stops bleeding; the intent is to treat the root.
Commentary: Miscarriage with metrorrhagia is mostly caused by sexual intercourse. For those over forty years old, the dosage of Ren Shen should be doubled, and Shu Di Huang should be halved, because Qi is deficient and Fire is weak; otherwise, it may lead to unrecoverable Qi collapse. All pregnant women must restrain desires and carefully avoid sexual activity; do not voluntarily enter dangerous situations. Be cautious!
Verse: Pregnant women engaging in sex act recklessly, leading to miscarriage and flooding blood flow.
Fire blazes, Water dries up, accompanied by Qi collapse; Solidifying Qi and filling Essence is the superior method.
For Solidifying Qi and filling Essence, use Shu Di Huang; Ginseng, Astragalus, Schizonepeta, and Notoginseng are excellent.
Stir-fry Atractylodes and Angelica, decoct for four doses; Tonify Qi and Essence to apply the miraculous prescription.
Pregnant women who suffer miscarriage due to falls or sprains, expelling purple clots and fainting nearly to death, are generally said to be afflicted by Blood stasis. Who would know that this is actually an injury to the Blood Chamber (Xueshi)! The Blood Chamber is connected to the Fetus Sac, relying on each other like lips and teeth. If the Fetus Sac is injured, the Blood Chamber is also damaged; if the lips perish, the teeth feel cold, which is logically inevitable. However, when the Fetus Sac is injured and bleeds, the injury is shallow; when the Blood Chamber is injured and bleeds, the injury is deep. Shallow injuries cause abdominal pain; deep injuries cause cardiac fainting. Although both result from trauma, the treatment differs between pre-miscarriage and post-miscarriage states. If the fetus is unstable but not yet miscarried, one should protect the fetus and not lightly remove blood stasis; if miscarriage has occurred with severe hemorrhage, one should dissipate stasis without further injuring Qi. Since the fetus has fallen and blood has been lost, the Blood Chamber is empty, leaving only Qi. If Qi is injured again, how can we ensure there is no risk of Qi collapse? The Classics say: "Blood is Ying (Nutritive), Qi is Wei (Defensive)." If Wei is not secure, Ying has no foundation to remain stable. Therefore, one must tonify Qi to generate Blood; as new Blood is generated, Blood stasis will naturally dissipate.
The formula used is Liqi Sanyu Tang (Regulate Qi and Dissipate Stasis Decoction).
Ren Shen (Ginseng Radix et Rhizoma, 1 liang), Huang Qi (Astragali Radix, 1 liang, raw), Dang Gui (Angelicae Sinensis Radix, 5 qian, washed with wine), Fu Ling (Poria, 3 qian), Hong Hua (Carthami Flos, 1 qian), Dan Pi (Moutan Cortex, 3 qian), Jiang Tan (Zingiberis Rhizoma Carbonisatum, 5 qian).
Decoct in water and take orally. After one dose, bleeding stops; after two doses, fainting disappears; after three doses, complete safety is restored. This formula uses Ren Shen and Huang Qi to tonify Qi; when Qi is vigorous, it can control Blood. It uses Dang Gui and Dan Pi to generate Blood; when Blood is generated, stasis is hard to retain. It uses Hong Hua and Black Ginger to invigorate Blood circulation; when Blood circulates freely, fainting can be eliminated. It uses Fu Ling to promote diuresis; when water passages are clear, Blood easily returns to its vessels.
Commentary: If the fetus has not yet fallen, add Du Zhong (Eucommiae Cortex, stir-fried until charred) 1 qian and Xu Duan (Dipsaci Radix, stir-fried until black) 1 qian; if the fetus has fallen, use the original formula. For unceasing metrorrhagia, add Guan Zhong Tan (Osmundae Rhizoma Carbonisatum) 3 qian; for blood obstruction causing cardiac fainting, add Yuan Hu Tan (Corydalis Rhizoma Carbonisatum) 1 qian.
Verse: Sprains and falls cause miscarriage with flowing blood clots; fainting nearly to death means life is about to end.
The injury to the Blood Chamber is severe; Regulating Qi and dissipating stasis can cure the disease.
Ginseng and Astragalus tonify Qi, providing sufficient strength; Angelica and Moutan generate Blood, making stasis hard to retain.
Carthamus and Ginger invigorate Blood to remove fainting; Poria promotes diuresis so Blood naturally gathers.
