Regarding the swelling, pain, and distension of hemorrhoids and fistulas (zhilou), the *Neijing* states: "Due to excessive fullness, the sinews and vessels become lax and relaxed, leading to hemorrhoids in the intestines." If hemorrhoids do not heal, they transform into fistulas. They should be treated using methods for resolving dampness. One may first use Daoshui Wan (Water-Guiding Pill) and Yugong San; after purgation, subsequently administer Zhike Wan (Citrus Aurantium Pill) and Muxiang Binglang Wan (Aucklandia and Betel Nut Pill). Additionally, consume okra soup, spinach, pig or sheep blood, etc., to promote bowel movement and intestinal lubrication. Strictly abstain from sexual intercourse, and avoid consuming chicken, fish, alcohol, vinegar, and other such substances.
For Huoluan (cholera-like syndrome with sudden vomiting and diarrhea) where vomiting and diarrhea persist without cessation, one may use Wuling San (Five-Ingredient Poria Powder) and Yiyuan San (Primordial Powder) in equal proportions, mixed with ice water and taken orally, five to seven qian. If ice water is unavailable, fresh drawn cold water may be used to mix Guiling Ganlu San (Cassia Twig, Poria, and Sweet Dew Powder), Yulu San (Jade Dew Powder), or Qingliang Yinzi (Cooling Drink), taken orally, five to seven qian. Alternatively, mixing with Xiangru Tang (Mosla Decoction) and taking five to seven qian is also acceptable. If none of the above medicines are available, drinking three to five cups of Dijiang (Earth Slurry) is also effective. To prepare Dijiang, dig a small pit in clean ground, pour in one bucket of fresh drawn water, stir it until muddy, wait for it to clarify, and then drink. Drinking three to five cups will lead to immediate recovery. Strictly prohibit the use of Baizhu Tang (Atractylodes Macrocephala Decoction), Jianggui Wu Fu (Ginger, Cinnamon, Aconitum carmichaelii, and Prepared Aconite Root), and various other drying and heating medications. Ingestion of these will certainly result in death. Chao Shi stated: "Huo" refers to illness arising suddenly and swiftly; "Luan" refers to the disorder of yin and yang. This is all caused by the mutual interference of clear and turbid qi of yin and yang.
For elderly patients suffering from prolonged illness with constipation and difficult, sluggish defecation, administering Shengong Wan (Divine Merit Pill), Mahuang Wan (Ephedra Pill - *Note: Context implies Hemp Seed/Pu Tao Ren usually, but text says Mahuang Wan, likely a typo for Mahuang/Maren or specific formula name in source, keeping literal*), or Sisheng Wan (Four Generations Pill) will lead to recovery. Frequently consuming okra, spinach, and pig or sheep blood will naturally promote smooth bowel movements. The *Neijing* states: "Nourishing the orifices with slipperiness," which is precisely this principle. If this condition does not heal, it can cause blindness.
For adults and children suffering from Shashi Lin (gravel strangury) and the five types of Lin syndrome characterized by dribbling and urinary retention (benglóng), accompanied by abdominal pain around the umbilicus, Yiyuan San (Primordial Powder) is the primary treatment, administered with flowing water. Bazheng San (Eight-Normality Powder) and Shiwei San (Pyrosia Leaf Powder) should be taken according to their respective prescriptions. All three of these formulas can be modified and administered.
For any condition caused by indigestion of cold food or un-dissipated residual alcohol, presenting similarly to Shanghan (Cold Damage) with symptoms such as fever, aversion to cold, shivering, headache and neck stiffness, back and spine rigidity, and deep pulses in both hands, the Biejie method (simultaneous release of exterior and interior) should not be used. Instead, only Daoyin Wan (Drink-Guiding Pill) should be used, fifty to sixty pills, adjusted according to deficiency or excess, to induce five to seven bowel movements. Once the damaged cold food and residual alcohol are thoroughly expelled, the headache and other ailments will resolve spontaneously. Subsequently, administer Wuling San (Five-Ingredient Poria Powder) with ginger and jujube, decocted in flowing water, taken five to six doses. Do not consume Jiuzheng Jinshi Wan (Alcohol Syndrome Appetite-Increasing Pill), as these medications contain Ban Dou (Croton Seed) and possess hot toxicity.
