Pediatric cold lin (cold strangury) is caused by wind-cold pathogens invading the Bladder, leading to cold in the Lower Jiao, thereby forming cold lin. The manifestations include urinary obstruction, unbearable distending pain, frequent dribbling, and dull pain in the lower abdomen.
Ancient methods primarily treated this with Wu Ling San (Five-Ingredient Powder with Poria) with increased doses of Rou Gui (Cinnamon Twig) and Xiao Hui Xiang (Fennel). The ingredients included Tu Chao Bai Zhu (Earth-fried Atractylodis Rhizoma), Ze Xie (Alismatis Rhizoma), Zhu Ling (Polyporus), Rou Gui (Cinnamon Twig), Xiao Hui Xiang (Fennel), Chi Fu Ling (Red Poria), etc.
[Chen Shouzhen's Note] When pediatric cold pathogens invade the Bladder, to treat lin, one must first dispel the cold. Neither Ma Huang (Ephedrae Herba) can be used to dispel cold from the Bladder, nor Fu Ling (Poria) and Ze Xie (Alismatis Rhizoma) to clear heat from the Bladder, because the Bladder inherently lacks heat and possesses only cold. The prescription uses Shang Rou Gui (Cortex Cinnamomi) 3 fen, Xiao Hui Xiang (Foeniculi Fructus) 1 qian, Xi Mu Tong (Akebiae Caulis) 8 fen, Li Zhi He (Litchi Seed) 1.5 qian, Chi Xiao Dou (Adzuki Bean) 1.5 qian, Chi Fu Ling (Poria) 1 qian, with Sheng Jiang Pi (Zingiberis Pericarpium) 3 fen as a guiding drug.
Pediatric hot lin (hot strangury) is formed due to heat accumulation in the Bladder. Its manifestations include urinary retention, dribbling, and painful dysuria. Ancient methods primarily treated this with Shi Wei Dao Chi Tang (Ten-Ingredient Decoction to Guide Out Heat from the Heart). The ingredients included Sheng Di Huang (Rehmanniae Radix), Shan Zhi Zi (Gardeniae Fructus), Mu Tong (Akebiae Caulis), Qu Mai (Dianthi Herba), Hua Shi (Talcum), Dan Zhu Ye (Lophatheri Herba), Yin Chen Hao (Artemisiae Scopariae Herba), Huang Qin (Scutellariae Radix), Sheng Gan Cao (Glycyrrhizae Radix et Rhizoma), and Zhu Ling (Polyporus), totaling ten ingredients.
[Chen Shouzhen's Note] For pediatric hot lin, using the full ten-ingredient formula immediately may result in an excessive dose. Instead, use Xi Sheng Di Huang (Rehmanniae Radix Preparata) 2 qian, Xiao Mu Tong (Akebiae Caulis) 1 qian, Cun Mai Dong (Ophiopogonis Tuber) 1 qian, Che Qian Zi (Plantaginis Semen) 1 qian, Dan Zhu Ye (Lophatheri Herba) 1 qian, Sheng Gan Cao Shao (Tip of Glycyrrhizae Radix et Rhizoma Raw) 3 fen, with Deng Xin (Typhae Stamen) 30 cun as a guiding drug. For deficiency cases, add Xi Yang Shen (American Ginseng Radix et Rhizoma) 5 fen.
Pediatric stone lin (stone strangury) presents with pain in the urethra during urination and the passage of sand-like stones. This is caused by long-term heat accumulation in the Bladder, similar to how a kettle develops white scale at the bottom after prolonged heating over fire. For mild cases, ancient methods primarily used Kui Zi San (Abutiloni Seed Powder). The ingredients included Chao Sang Bai Pi (Mori Cortex), Qu Mai (Dianthi Herba), Zhi Zi (Gardeniae Fructus), Chi Fu Ling (Poria), Mu Tong (Akebiae Caulis), Che Qian Zi (Plantaginis Semen), Zhi Gan Cao (Honey-fried Glycyrrhizae Radix et Rhizoma), and Kui Zi (Abutiloni Semen). For severe cases, ancient methods primarily used Ba Zheng San (Eight-Ingredient Powder for Rectification). The ingredients included Bian Xu (Polygoni Avicularis Herba), Qu Mai (Dianthi Herba), Hua Shi (Talcum), Mu Tong (Akebiae Caulis), Chi Fu Ling (Poria), Che Qian Zi (Plantaginis Semen), Sheng Da Huang (Rhei Radix et Rhizoma Raw), Sheng Zhi Zi (Gardeniae Fructus Raw), with Deng Xin (Typhae Stamen) as a guiding drug.
[Chen Shouzhen's Note] Pediatric heat binding in the Bladder leads to lin only after a long period, with discharge resembling sand and stones. This results from previous lack of proper treatment; treating it now is not too late. The prescription uses Sheng Di Huang (Rehmanniae Radix) 2 qian, Da Mu Tong (Akebiae Caulis) 1 qian, Chuan Huang Lian (Coptidis Rhizoma) 5 fen, Fei Hua Shi (Prepared Talcum) 3 qian, Chi Fu Ling (Poria) 1.5 qian, Sheng Gan Cao Shao (Tip of Glycyrrhizae Radix et Rhizoma Raw) 5 fen, with Xian Zhu Ye (Fresh Lophatheri Herba) 15 leaves and Deng Xin (Typhae Stamen) 10 stems as guiding drugs. For severe cases, add Sheng Jin Wen (Raw Rhei Radix et Rhizoma) 6 fen.
