Ryōya Sokan from Naniwa was over sixty years old, emaciated and unable to eat. Initially, his condition resulted from food stagnation (Shangshi). Various physicians treated him with Xiangsha Liujunzi Tang (Aromatic Sandalwood Six Gentlemen Decoction) and Qiwei Baizhu San (Seven-Ingredient Atractylodes Macrocephala Powder), but to no avail. Yumatsu administered Yixiao San (Heterogeneous Efficacy Powder) combined with Danggui (Angelica sinensis), and he recovered after two or three doses.
Another case involved a woman from the Kimuta shop who refused grain food, consuming only other substances. All treatments failed. Yumatsu prescribed Siwu Tang (Four Substances Decoction) supplemented with Renshen (Ginseng), Baizhu (Atractylodes macrocephala), and Jupi (Tangerine Peel), resulting in her recovery. When disciples asked for the rationale, he explained: "When the blood of the Spleen and Stomach is deficient, dryness and lack of moisture prevent eating." Danggui has a sweet flavor and can tonify the blood within the Spleen; thus, it serves as an agent to promote appetite. The Classic states: "The hands grasp when they receive blood; the feet walk when they receive blood; the eyes see when the Liver receives blood." Based on this, one may infer that the text should originally have included the phrase "the Stomach eats when it receives blood" following "the eyes see when the Liver receives blood."
Daqi Qi Tang (Great Seven-Qi Decoction) is used to treat persistent coughing in women; its underlying principle is worth pondering.
In cases of paralysis lasting for years, if the patient suddenly exhibits movement of the limbs and bright, clear eyes, it indicates an acute change in the condition (a critical sign).
For patients with chronic illness or extreme deficiency, if the flesh at the Chi pulse position (wrist area corresponding to Kidney/Yin) is sunken, and the fingertips cannot flex rapidly, the prognosis is generally poor. (Kuranaka Kaku-ryō stated: "Whenever a patient presents with emaciated muscles, sunken flesh behind the wrist with a flat appearance, and inability to approximate the middle and ring fingers when grasped by the physician, regardless of the disease, it is a death omen. Even if dietary intake remains normal, this is merely the breath of a wandering soul. In cases of Laozai (consumption/tuberculosis), this sign inevitably appears after several months. However, in cases following Shanghan (Cold Damage), Liji (Dysentery), or Hongxiangjiao (Beriberi/Edema), recovery is often possible, likely because the root vitality remains.")
The *Mai Yao* (Essentials of Pulse Diagnosis) aims to elucidate the principles of preserving life found in *Yisheng Weilun*; no other approach is necessary. One may also read sections on pulse diagnosis within Materia Medica texts.
For dysentery, vomiting, or hiccups where medicines cannot be retained, administering a small dose of single-ingredient Huanglian (Coptis chinensis) allows both medicine and food to be taken. If signs of roundworms (Hui Zheng) appear thereafter, combining the previous formula with Daqi Qi Tang is effective. This method originates from Drue's treatment of Lady Taiyi.
Yumatsu’s *Yifang Koujue Ji* (Collection of Mnemonics for Medical Formulas) is a pioneering work compiling theories and formula analyses. Reading it reveals profound academic wealth and effortless mastery. However, regarding his *Beishan Yi An* (Medical Cases of Beishan), it merely imitates ancients and rigidly adheres to rules, ultimately lacking elevated and unrestrained spirit. It is acceptable for learners to read it, but omission is also permissible.
