If Spring-Warmth (Chunwen) persists for over ten days without resolution, manifesting as coma, delirious speech, a scorched black tongue coating, purple or black macules, and unbearable restlessness, this indicates Heat sinking into the Blood level causing Blood stasis. It is appropriate to use Xijiao Dihuang Tang (Rhino Horn and Rehmannia Decoction) added with Zicao (Arnebiae Radix seu Lithospermi Radix), Yuan Shen (Scrophulariae Radix), Lian Qiao (Forsythiae Fructus), Guang Yujin (Curcumae Radix), Xian Changpu (Acori Tatarinowii Rhizoma), and Zixue Dan (Purple Snow Pill), to cool the Blood and resolve macules.
When Heat transmits to the Yangming level and fails to resolve, it inevitably enters the Blood level. When Heat and Blood stasis intertwine, they cannot be cleared by simple cooling methods alone. One must rely on the blood-cooling properties of Xijiao (Rhinocerotis Cornu), Sheng Di Huang (Rehmanniae Radix), Yuan Shen (Scrophulariae Radix), and Lian Qiao (Forsythiae Fructus); assist with the blood-breaking actions of Chi Shao (Paeoniae Radix Rubra), Mu Dan Pi (Moutan Cortex), Tao Ren (Persicae Semen), and Zicao (Arnebiae Radix seu Lithospermi Radix); employ Yu Jin (Curcumae Radix) and Changpu (Acori Tatarinowii Rhizoma) to open the orifices; and use Zixue Dan (Purple Snow Pill) and Zhu Ye (Bambusae Folium) to clear the Heart. Only when the spirit becomes clear, the tongue moist, and the macules turn red and vibrant, is there hope for survival. However, if the macules are purple-black, Stomach Heat has reached its extreme. If restlessness appears, the pattern of internal closure and external collapse has already formed, and danger awaits immediately. Although one may attempt to reverse this heavenly mechanism, it is likely merely a futile effort of human intervention.
If Spring-Warmth Heat fails to resolve, accompanied by hardness and pain in the lower abdomen, free urination, black stools, coma, delirious speech, and狂妄 (wild behavior), this is Blood Accumulation (Xuexue). It is appropriate to use Xijiao (Rhinocerotis Cornu), Sheng Di Huang (Rehmanniae Radix), Tao Ren (Persicae Semen), Mu Dan Pi (Moutan Cortex), Chi Shao (Paeoniae Radix Rubra), Gui Wei (Angelicae Sinensis Radix Caulis), Ling Zhi (Trogopterori Faeces), Chai Hu (Bupleuri Radix), and Huang Qin (Scutellariae Radix). In severe cases, add Da Huang (Rhei Radix et Rhizoma) and Die Chong (Eupolyphaga Seu Steleophaga), to break stasis and expel pathogens.
The previous paragraph discussed Heat sinking into Blood causing stasis; this paragraph discusses Heat binding with Blood. Zhang Zhongjing stated: "Free urination, black stools, clarity during the day, and delirium at night" constitute the Blood Accumulation syndrome. Once Heat enters the Blood level, it cannot be resolved without breaking the Blood. Therefore, imitating Xijiao Dihuang Tang (Rhino Horn and Rehmannia Decoction), we add Gui Wei (Angelicae Sinensis Radix Caulis), Tao Ren (Persicae Semen), Ling Zhi (Trogopterori Faeces), Die Chong (Eupolyphaga Seu Steleophaga), and Da Huang (Rhei Radix et Rhizoma) to break Blood and expel pathogens. This allows the broken stasis to dissipate and the pathogenic Heat to emerge, thereby stopping the manifestations of delirium and wild behavior. Regarding Zhang Zhongjing's discussion on Blood Accumulation: there is the case where Taiyang disease fails to resolve, transmitting from the Fu organ to the Blood level, treated with Tao Ren Cheng Qi Tang (Peach Kernel and Magnolia Bark Decoction); there is the case where Yangming disease fails to resolve, transmitting from the Qi level to the Blood level, treated with Xijiao Dihuang Tang (Rhino Horn and Rehmannia Decoction); and severe cases are all treated with Didang Tang (Definite Attack Decoction) to purge. Currently, Warm Pathogen-induced Blood Accumulation mostly enters the Blood level via Yangming; thus, Xijiao Dihuang Tang (Rhino Horn and Rehmannia Decoction) is used to clear it. Tao Hua (Tao Jie'an) used Xiaochaihu Tang (Minor Bupleurum Decoction) added with Gui Wei (Angelicae Sinensis Radix Caulis), Chu Zha (Crataegi Fructus), Tao Ren (Persicae Semen), and Mu Dan Pi (Moutan Cortex), applicable when there is alternating chills and fever involving the Shaoyang level, and because once Heat enters the Blood, it requires ascending and dispersing methods to resolve.
