Whenever there is a paroxysmal pain in the ears, it indicates that wind pathogen has invaded the Kidney Meridian (Kidney Channel), which is difficult to treat. It will subsequently transform into aversion to cold and fever, stiffness of the spine and straightness of the back, presenting symptoms similar to muscle relaxation and ligamentous laxity, known as Huang Er Shang Han (Yellow Ear Cold Damage). This condition should not be treated as ordinary Shang Han (Cold Damage); it is appropriate to use Xiao Xu Ming Tang (Minor Continuation of Life Decoction) with Fuzi (Aconite Lateral Root) removed, adding Can Sha (Bombyx Batryticatus), Tian Ma (Gastrodia Rhizome), Qiang Huo (Notopterygii Rhizoma et Radix), and Du Huo (Angelicae Pubescentis Radix). Subsequently, Fang Ji Jing Jie Bai Du San (Poria and Schizonepeta Decoction to Overcome Toxins) can be used, adding Xi Xin (Asari Radix et Rhizoma), Bai Zhi (Angelicae Dahuricae Radix), Chan Tui (Cicadae Periostracum), Huang Qin (Scutellariae Radix), Chi Shao (Paeoniae Radix Rubra), and Zi Jin Pi (Phellodendri Cortex).
"Jie" (Relaxation/Separation) refers to the dissipation of muscles. "Bi Zu" (Thighs and Feet/Limbs) refers to the inability of tendons to bind the bones, manifesting as a syndrome of muscle relaxation and skeletal instability. The symptoms resemble cold but are not truly cold, resemble heat but are not truly heat; the four limbs and bone joints feel loose, weak, and lethargic, accompanied by fatigue, irritability, and pain. Appetite is poor, and food lacks flavor. This is colloquially called Sha Bing (Sha Disease). The *Neijing* (Classic of Internal Canon) refers to this as Jie Bi Zu. Investigating its causes, it may result from excessive alcohol consumption, damp-evil invasion, wind-cold exposure, excessive sexual activity, or irregular menstruation and disharmony of Qi and Blood in women. All these can cause this syndrome, which resembles Sha Bing but is actually distinct. Treatment should first involve applying hot water-soaked cloths to the arms and scraping them with Ramie fiber. In severe cases, puncturing the Shi Xuan (Ten Extremities) acupoints and Wei Zhong (Bladder 39) for bloodletting can open the interstices (Cou Li) and promote smooth flow of Qi and Blood, leading to recovery. Su He Xiang Wan (Styrax Pill) is appropriate to take.
Sha Bing occurs primarily in regions like Lingnan, Min, and Guang. There, stream toxins, Sha wind, Shui Nu (Water Archer), She Gong (Arrow Shooter), Guo, Duan Hu (Short Fox), and Xia Xu (Shrimp Whisker) insects can contain sand and shoot at humans. Those poisoned by them experience aversion to cold and high fever, a sensation of their entire body decomposing, resembling the initial stage of Shang Han. Local treatment involves palpating the painful areas, inserting horn tubes into the skin, and sucking out the Sha toxin orally. Externally, garlic is roasted and mashed into a paste to seal the wound, resulting in cure. Among these insects, Xia Xu is the most toxic. If not treated early, eight or nine out of ten patients die. The toxin penetrates deep into the bones, appearing like shrimp whiskers, and the sores resemble Ding Zhong (carbuncles). There are birds such as HT in these regions that specifically eat these insects. Therefore, burning the feathers and feces of these birds to ash and ingesting them, or placing cages containing these birds near the patient, allows the Sha toxin to exit naturally upon smelling the odor, leading to recovery.
Shang Han Bai He Bing (Cold Damage Lily Disease) is characterized by restlessness in walking, standing, sitting, and lying down, as if possessed by spirits. Su Song stated: Named after the lily, yet treated with lily bulb, I do not understand the rationale. Shi Cai stated: This also achieves the effect of clearing the Heart and calming the Spirit.
