Patient Jin (Occupation: vegetable garden keeper, age 56, onset in August)
The treatment of Summerheat-Turbidity Internal Closure (Fushu Neibi) is not easy. (First, rub the teeth with a sour plum [Wumei], and once the jaw opens, immediately administer one pill of Zhibao Dan [Precious Treasure Pill], taken down with Shichangpu Decoction.)
Lingjiao Pian (Antelope Horn Slice), Chuan Yujin (Curcuma Aromatica Rhizome), Zhuli (Bamboo Sap), Xian Shichanggen Hechong (Fresh Acorus Tatarinowii Root Juice, mixed and taken orally), Yi Yuan San (Benefit the Origin Powder), Lianqiao (Forsythia Fruit), Danpi (Moutan Cortex), Shijueming (Haliotidis Shell), Cheqiancao (Plantago Herb), Niubangzi (Arctii Fructus), Chun Nen Gouteng (Uncaria Rhynchophylla Tender Branch), Chen Danning (Arisaema Cum Bile Old Preparation), Xiaozhibao Dan One Pill (Small Precious Treasure Pill).
On the following day, after administering the medicine, the internal closure was immediately resolved, but the mental state was occasionally unclear. The previous method was still followed; the original prescription was used minus Zhibao Dan and Zhuli, plus Niuhuang Qingxin Wan (Calculus Bovis Heart-Clearing Pill) and Zhuru (Bamboo Sap).
Patient Qian (Age 11)
Vexation originates from the Heart, restlessness originates from the Kidney. The pathogenic heat has deeply entered the Ying (Nutrient) Level, and body fluids have been depleted. Symptoms include high fever, thirst, and restlessness. There is fear that the pathogenic heat may reverse from the Ying Level to the Heart Master, agitating the Palace City and stirring up Liver Wind, leading to convulsions and syncope. The pulse is wiry, slippery, and rapid. The treatment should employ clearing and resolving methods, hoping for a turning point in the condition. Attached is the formula for correction.
Xuanshen (Scrophulariae Radix), Bohe (Menthae Haplocalycis Herba), Chun Gouteng (Uncaria Rhynchophylla Tender Branch), Xian Zhuli (Fresh Bamboo Sap), Shichangzhi Tongchong (Acori Tatarinowii Rhizoma Juice, mixed and taken orally), Niuhuang Qingxin Wan (Calculus Bovis Heart-Clearing Pill), Lianqiao (Forsythiae Fructum), Chuan Yujin (Curcumae Rhizoma), Shijueming (Haliotidis Shell), Cheqiancao (Plantaginis Herba), Lingjiao Pian (Saigae Tataricae Cornu), Danpi (Moutan Cortex), Jing Danning (Arisaema Cum Bile), Zixue Dan (Purple Snow Pill).
Patient Yu (Age 15, onset on July 10th)
The disease has lasted for more than ten days. Heat has injured the Ying Yin. Summerheat-Damp-Heat pathogens have deeply entered the Jueyin Channel. Symptoms include internal heat, vexation, thirst, physical fatigue, dizziness, darkness of vision, coldness of the four limbs, occasional tidal fever, white papules (Baipi) on the skin that were not successfully vented, wind-stirring convulsions, and caution is needed against collapse. The pulse is wiry, slippery, and rapid, lacking spirit upon palpation. The treatment should clear the Heart, wash away phlegm, calm the Liver, and open orifices. Attached is the formula for correction.
Taishenxu with three Rose Flowers stewed together and taken orally (Tai Shen Xu Meihua Tongdun Chong), Chun Nen Gouteng (Uncaria Rhynchophylla Tender Branch), Qinghaozi (Artemisiae Annuae Herba), Zhuli (Bamboo Sap), Niuhuang Qingxin Wan (Calculus Bovis Heart-Clearing Pill), Zhen Chuju (Chrysanthemi Flos), Shijueming (Haliotidis Shell), Zhen Chuanlian Banban (Coptidis Rhizoma, mixed with 3 fen), Chuan Yujin (Curcumae Rhizoma), Danning (Arisaema Cum Bile), Danpi (Moutan Cortex), Zhu Fushen (Poria Cocos with Sclerotium dyed red), Bohe (Menthae Haplocalycis Herba).
Yi Yuan San (Note: Since the formula already contains Danning, Niuhuang, and Chuanlian, this powder is not needed).
On the following day, coldness of the limbs had subsided, and wheezing and sweating had stopped. However, Lung fluids and Stomach yin had been consumed by heat. Tidal fever persisted, stool flowed freely with heat, left hypochondriac fullness and pain with tenderness, mental fatigue, limb weakness, lack of appetite and intake. The pulse was deficient, rapid, approaching wiry, and the coating was yellow and rough. The treatment should nourish and clear to eliminate residual pathogens. Dietary control and avoidance of wind are necessary to prevent recurrence. Another formula is recorded separately for correction.
Tai Shen Xu with three Rose Flowers stewed together and taken orally (Tai Shen Xu Meihua Tongdun Chong), Dong Baishao (Paeoniae Radix Alba), Qinghaozi (Artemisiae Annuae Herba), Cheqiancao (Plantaginis Herba), Xiao Qingpi (Citri Reticulatae Pericarpium Viride), Lianxin Maidong (Ophiopogonis Radix with heart), Zuo Muli (Ostreae Concha), Danpi (Moutan Cortex), Sheng Guya (Hordei Fructus Germinatus), Jin Bian Shihu (Dendrobii Nobile Caulis), Dan Biejia (Trionycis Carapax), Chun Nen Gouteng (Uncaria Rhynchophylla Tender Branch), Zhu Fushen (Poria Cocos with Sclerotium dyed red). If the tongue coating is yellow and rough, indicating residual pathogens are not yet cleared, the use of cloying and greasy tonics like Renshen and Maidong should be weighed carefully. Regarding symptoms of lack of appetite and intake, extra care must be taken to discern whether greasy tonification is appropriate.
