Postpartum generalized body pain is mostly caused by the opening of all joints during childbirth, leading to empty blood vessels that cannot nourish the body, or by retained blood (stasis) taking advantage of the deficiency to invade the meridians, both resulting in pain. The general principle of treatment: If there is generalized body pain that worsens upon pressure, it indicates stasis of blood; use Siwu Tang (Four Substances Decoction) with added Heijiang (Charred Ginger), Taoren (Peach Kernel), Honghua (Safflower), and Zelan (Eupatorium), to tonify while resolving stasis. If the pain alleviates slightly upon pressure, it indicates blood deficiency; use Siwu Tang (Four Substances Decoction) with added Heijiang (Charred Ginger), Renshen (Ginseng), and Baizhu (Atractylodes Macrocephala Rhizome), to tonify and nourish. If accompanied by wind-cold pathogens, symptoms such as fever and aversion to cold, headache, nasal congestion, and hot breath will appear, indicating an external contraction; use Guyi San (Ancient Powder) with added Danggui (Angelica Sinensis), Chuanxiong (Ligusticum Chuanxiong Rhizome), Qinjiao (Gentiana Macrophylla Root), and Heijiang (Charred Ginger) to disperse the pathogen. If the pain persists after dispersion, it may be due to blood deficiency, and Bazhen Tang (Eight Treasures Decoction) should be used for tonification. This outlines the differentiation for treating body pain.
For postpartum wind invasion causing tendon and vessel traction and stiffness, convulsions, unconsciousness, or fever with aversion to cold, headache, and body pain, grind the aforementioned herbs into powder. Take three qian per dose, mixed with ginger decoction.
Another prescription: Mix the aforementioned herbs with Danggui (Angelica Sinensis) in equal parts and grind into powder. Indications are as above, named Qinghun San (Clear Soul Powder).
Bazhen Tang (Eight Treasures Decoction) (see Deficiency Syndrome section).
Medical texts state: The lower part of the waist is governed by the Kidney. Due to labor injuring Kidney Qi during childbirth, wind-cold pathogens may lodge there, causing lower back pain. Pain extending from the lower back up to the spine and down to the legs and knees indicates Wind; isolated lower back pain without radiation indicates Deficiency. For Wind, use Duhuo Jisheng Tang (Notopterygium and Sangjisheng Decoction); for Deficiency, use Bazhen Tang (Eight Treasures Decoction) supplemented with Duzhong (Eucommia Bark), Xuduan (Dipsacus Root), Rougui (Cinnamon Bark), etc. If lochia fails to clear and flows into the thighs, causing stabbing pain like needle pricks that cannot be touched, immediately use Taoren Tang (Peach Kernel Decoction) to resolve stasis, preventing the formation of abscesses. Generally, diseases are treated by tonifying deficiency and purging excess; when deficiency coexists with excess or vice versa, careful consideration must be given to the balance between tonification and purgation.
Duhuo Jisheng Tang (Notopterygium and Sangjisheng Decoction) (see Lower Back Pain section).
Bazhen Tang (Eight Treasures Decoction) (see Deficiency Syndrome section).
Taoren (Peach Kernel, fried and ground, 10 kernels) Danggui (Angelica Sinensis, 3 qian) Niuxi (Achyranthes Bidentata Root, 2 qian) Zelan (Eupatorium Fortunei Herb, 3 qian) Sumu (Calamus Wood, 1 qian). Decoct in water, take with hot wine on an empty stomach.
