For treating difficult labor and malpresentation, its efficacy is significant.
Bai Cao Shuang (Soot from hundred-year-old stove) and Bai Zhi (Angelicae Dahuricae Radix) (neither should be processed by fire; each ground into fine powder, in equal parts).
Take 3 qian per dose. Mix with child's urine (Tong Bian) and rice vinegar to form a paste. Add boiling water and take orally; or decoct with wine, add a small amount of child's urine, and take hot. The book states: "Blood stops when it sees black." This medicine not only facilitates normal delivery but also strongly consolidates blood, preventing blood depletion, which is wonderful.
Yang Zijian said: In all deliveries, one must first know these ten conditions, so that both mother and child may be saved. Midwives in the world are rarely skilled, often causing errors. I therefore discuss them in detail out of deep concern.
The first is called Zheng Chan (Normal Delivery). After ten months of pregnancy, when yin and yang qi are sufficient, sudden pain in the waist and abdomen occurs. The fetus turns itself, facing the birth canal. Amniotic fluid breaks, blood flows, and the baby is born naturally.
The second is called Cui Chan (Induced Labor). When the fetal head has reached the birth canal and there is simultaneous waist and abdominal pain, but delivery does not occur, then administer medicine to induce labor. Or if the duration is too long and the mother is exhausted and unable to deliver, administer medicine to support qi and blood, enabling the fetus to be born quickly.
The third is called Shang Chan (Traumatic Labor). Pregnancy is not yet full-term. Due to injury or disturbance, umbilical and abdominal pain occurs, leading to sudden desire to deliver. Or if催产 medicines are taken recklessly, forcing the fetus to be born quickly. Such births may cause injury to the infant; caution is required.
The fourth is called Dong Chan (Cold Labor). In cold weather, the mother's qi and blood stagnate, making rapid delivery difficult. Therefore, clothing should be thick, the delivery room warm, and lower garments even warmer, so the fetus can be born easily. One must avoid excessive heat from fire, lest it cause syncope due to blood loss (Xue Yun).
The fifth is called Re Chan (Hot Labor). In midsummer, the parturient should maintain a moderate temperature. Overheating causes dizziness and vertigo, leading to syncope due to blood loss. One must carefully avoid cool pavilions, water towers, strong winds, and rainy days.
The sixth is called Heng Chan (Transverse Presentation). This refers to the situation where the fetus is turning, but the mother exerts force too urgently, forcing the fetal body into an abnormal position. The mother should lie comfortably on her back. An experienced midwife should first push the fetal body to align with gravity, head towards the birth canal. Using the middle finger to probe the fetal shoulder ensures the umbilical cord is not tangled. Then, administer medicine to induce labor, and instruct the mother to push.
The seventh is called Dao Chan (Breech Presentation). This refers to the situation where the fetus has not turned, but the mother exerts force recklessly, causing hands or feet to emerge first. The mother should lie comfortably on her back. The midwife should gently push the limbs back inside and wait for the fetus to align naturally. If delivery does not occur after a long time, the midwife should insert a hand into the vagina, manipulate the fetus to turn towards the birth canal from one side, and administer induction medicine.
The eighth is called Pian Chan (Asynclitic Presentation). This refers to the situation where the fetus has turned but is not aligned with the birth path. The mother, anxious to push, forces the fetal head to one side. Although the vertex appears, it is actually the frontal angle or ear. The mother should lie on her back. The midwife should gently correct the head's position. Administer induction medicine and instruct the mother to push; the fetus will then be born. If the occiput deviates towards the anus and the forehead faces up, the midwife should warm cotton clothes, wrap their hand, and gently support from outside the anus. Instruct the mother to push, and the fetus will be born.
The ninth is called Zuo Chan (Sitting Presentation). The mother lies on her back. The midwife gently pushes the fetus upward, presses the fetal shoulder with the middle finger to free the umbilical cord, realigns the fetal body, and instructs the mother to push; the fetus will be born.
The tenth is called Pan Chang Chan (Prolapsed Uterus Delivery). During labor, the uterus prolapses before the fetus is born. When the uterus protrudes, place it in an extremely clean, unbroken lacquerware vessel. Ancient prescriptions use 49 seeds of Bi Ma Zi (Ricini Semen), ground into a paste, applied to the mother's crown. Once the uterus retracts, wash it off immediately. For the degree of prolapse, use a small amount of water used for sharpening knives to moisten it. Then, decoct Ci Shi (Magnetitum) and drink it to retract the uterus. The Magnetitum must be one verified by Feng Shui practitioners. Ancient methods included wiping the mother's face and back with vinegar water, but this may cause fright and dispersion of qi, which is highly inadvisable. Another method involves rubbing sesame oil on the thumb, lighting a lamp, blowing it out, and using the smoke to熏 (fumigate) the mother's nose, causing the uterus to retract.
The above discussion on the Ten Types of Labor is indeed precise and comprehensive. However, it does not address the condition of non-dilation of the pubic symphysis (Jiao Gu Bu Kai). This suffices to show that medical arts are complex and difficult, inaccessible to those who are superficial or impatient.
