Bi (Obstruction) refers to pain. The *Neijing* states: "When one Yin and one Yang are knotted, it is called Hou Bi (Throat Obstruction)." By "one Yin," it refers to the Hand-Shaoyin Heart Meridian; by "one Yang," it refers to the Hand-Shaoyang Sanjiao (Triple Burner) Meridian. The Heart governs Jun Huo (Monarch Fire), and the Sanjiao governs Xiang Huo (Ministerial Fire). When these two fires clash and impact each other, they cause obstruction and pain in the throat. The treatment principle should be to disperse and clear the heat, primarily using Jiawei Ganjie Tang (Gan Jie Decoction with added herbs).
There is also a condition caused by sudden cold pathogens occurring out of season, which lie dormant in the Shaoyin Meridian and manifest after more than ten days. This is named Fuqi Yantong (Pharyngeal Pain due to Latent Qi), colloquially known as Shenshanghan (Cold Injury to the Kidney). The treatment principle should employ pungent-warm methods to disperse the pathogen, primarily using Banxia Guigan Tang (Banxia, Cinnamon Twig, and Licorice Decoction). Furthermore, there is a severe syndrome of Cold invading the Shaoyin Meridian, where Cold pathogens reside in the Lower Jiao, forcing the rootless and unanchored Fire to flare up towards the upper body, resulting in throat pain. Symptoms include cold extremities, a fine and deep pulse, and diarrhea with undigested food. In this case, simply using Lizhong Tang (Regulate the Middle Decoction) and Sini Tang (Reverse the Extremities Decoction) to treat the cold will naturally resolve the throat pain. These two scenarios belong to Cold syndromes, while others mostly belong to Heat syndromes; this distinction must not be overlooked.
Swelling, pain, and blockage in the throat and tonsils, accompanied by red filaments winding around, is thus named Chanhou Feng (Winding Throat Wind). Symptoms include spitting out saliva and froth, difficulty in swallowing food, and in severe cases, swelling extending externally so that the head feels as if wrapped by a snake. Initially, mix a small amount of Yuanming Fen (Refined Mirabilite) with Huangji Zhi (Juice of Yellow Pickled Vegetables) and instill it into the throat to stir up and remove phlegm, then moisten the throat with honey water. If the Huangji Zhi cannot extract the phlegm, use Tu Niuxi (Radix Achyranthis Bidentatae), crushed whole including the roots, mixed with sour vinegar for administration. If stubborn phlegm is firmly consolidated and cannot be expectorated, causing complete obstruction of the throat such that even a drop of water cannot pass, use Jiedu Xionghuang Wan (Detoxifying Realgar Pill). Grind seven pills with highly acidic vinegar and administer orally; vomiting will naturally occur, clearing the obstruction. Once cleared, Niuhuang Qingxin Wan (Calculus Bovis Heart-Clearing Pill) combined with Jiawei Ganjie Tang can be used. If the swelling extends externally to involve the neck, head, and face, appearing red like firelight, and medicinal power is insufficient to counteract it, urgently perform Bianci (Stone Needle Therapy) with a porcelain edge to release evil blood, then apply Ruxiang Mo (Frankincense Powder) mixed with Jizi Qing (Egg White) to moisturize; recovery will be immediate. Alternatively, use Bajiao Gen Zhi (Juice of Musa Basjoo Root) to moisten the throat and resolve toxicity. If the mouth swells with a purple-black color, urgently prick with silver needles to release blood, or puncture with a small knife, then wash with light salt water and insufflate Bingpian San (Borneol Powder). In cases where swelling is inside the throat where needle application is difficult, urgently bleed at Shaoshang (LU-11) Acupoint on the hand; the throat swelling will naturally open. Follow this by administering Jiedu Xionghuang Wan to ensure patency. Such diseases present critical and dangerous conditions. Without simultaneous administration of phlegm-expelling, detoxifying, decoctions, and pills, along with proper application of knives, needles, and stone needles, how can there be efficacy? Physicians treating these conditions must practice and master these techniques thoroughly in daily study; only then can they handle clinical cases effectively. Do not be negligent or slow, lest you harm people. Shaoshang (LU-11) Acupoint is located on the medial side of the thumb, approximately the width of a leek leaf from the nail bed. When needling, use...
