Gardeniae Fructus (Zhizi) and Phellodendri Cortex (Baihuangbo), both should be stir-fried with salt and wine, and a small amount of approximately one fen of Cinnamomi Ramulus (Rougui) should be added to clear Kidney Fire. If blood in urine exits from the urethral orifice, accompanied by dysuria, frequency, forming strangury (Lin syndrome) with pain, or if blood is mixed with urine, this originates from the Bladder; add Gardeniae Semen (Zhiziren), Polygoni Citrini Herba (Qumai), Achyranthis Bidentatae Radix (Niuxi), and Talcum (Huashi) to the base formula to clear Bladder Fire. If blood in urine is present without pain, this indicates Heart heat transferring to the Small Intestine, so blood exits from the essence orifice (Jingqiao); add Coptidis Rhizoma (Huanglian), Gardeniae Fructus (Zhizi), and Scutellariae Radix (Tiaohuangqin) to the base formula to clear the fire of this meridian (Small Intestine Meridian). If blood appears before stool during defecation, it is called "near blood" (Jinxue), indicating the source is the Large Intestine; add Arecae Semen (Binglang), Aurantii Fructus Immaturus (Zhishi), Sophorae Flavescentis Flos (Huaihua), and Scutellariae Radix (Tiaohuangqin) to the base formula to clear Large Intestine Fire. If blood appears after stool during defecation, it is called "far blood" (Yuanxue), indicating the source is the Small Intestine; add Akebiae Caulis (Mutong) and Coptidis Rhizoma (Huanglian) stir-fried with Cornus Officinalis Fructus (Yu) to the base formula to clear Small Intestine Fire. For bleeding from the mouth and nose, add Rhinocerotis Cornu (Xijiao), Scutellariae Radix (Qin), and Coptidis Rhizoma (Lian) to clear heat; or add Imperatae Rhizoma Spica (Maohua), Nymphaeae Nelumbinis Stamen (Oujie), Palmi Folium Cinis (Zonglühui), and Taraxaci Herba (Chaopuhuang) to stop bleeding; or add Allii Tuberosi Juice (Jiuji), Pueri Urina (Tongbian), Camelliae Japonicae Flos (Shanchahua), and Moutan Cortex (Mudanpi) to dissipate stasis. For bleeding from the intestines (implied context of rectal bleeding based on previous sentence structure, though text says "blood coming out of large intestine"), add Sophorae Flavescentis Flos (Huaihua), Platycladi Cacumen (Cebaiye), and Scutellariae Radix (Tiaohuangqin) to clear heat; or add Sanguisorbae Radix (Diyu), Schizonepetae Spica (Jingjie), Angelicae Dahuricae Radix (Baizhi), and Imperatae Rhizoma (Maogen) to stop bleeding. For bleeding from the urinary tract, add Polygoni Citrini Herba (Qumai), Ophiopogonis Radix (Maidong), and Gardeniae Semen (Zhiziren) to clear heat; or add Talcum (Huashi), Akebiae Caulis (Mutong), and Cirsii Japonici Herba (Da Ji / Xiao Ji) to promote blood circulation. Whether promoting circulation, clearing heat, or stopping bleeding, treatment must be administered according to the novelty, chronicity, and urgency of the condition, all using Siwu Decoction as the sovereign drug (Jun Yao).
(Ancestral Formula)
Treating hematuria.
Use Plantaginis Herba (Cheqiancao) leaves and Smilacis Glabrae Rhizoma (Jinlingcao) leaves (commonly known as Modoucao).
Crush these two ingredients to extract natural juice, one cup, and drink on an empty stomach for immediate cessation of bleeding.
Another: Hair Ash Pill (Fahui Wan)
Use hair from infants. If unavailable, use hair from healthy young adults; cut hair is superior, naturally fallen hair is secondary. Burn the hair into ash and grind finely. Separately, crush fresh Platycladi Cacumen (Cebaiye) to extract juice, mix with glutinous rice flour paste to form pills the size of Sophora seeds. Take 50 pills per dose, washed down with warm water, or preferably with Siwu Decoction decoction. Take on an empty stomach.
Another Formula: Treating rectal bleeding.
Burn bamboo leaves to ash while preserving their nature (Cunxing), bind with rice paste into pills the size of Sophora seeds. Take 70-80 pills per dose, washed down with rice water on an empty stomach.
(Medical Case)
A man in his forties, who had a history of excessive alcohol consumption, suffered from rectal bleeding. He visited the toilet two to three times a day, passing approximately one sheng of blood each time. I prescribed Siwu Decoction supplemented with Scutellariae Radix (Tiaohuangqin), Saposhnikoviae Radix (Fangfeng), Schizonepetae Spica (Jingjie), Angelicae Dahuricae Radix (Baizhi), and Sophorae Flavescentis Flos (Huaihua). After several days of administration, there was no effect. Subsequently, I used two qian and seven fen of ash from oak galls (Roudou), mixed into the previous herbal decoction for him to take. Additionally, moxibustion was applied to Mingmen (GV4), located on the midline of the spine opposite the umbilicus. The bleeding ceased, and he recovered safely; the disease did not recur thereafter.
