[Shi] (Wang Yinjun) The patient experiences aversion to cold, fatigue and lassitude, or shivering chills. This condition may cause the smallpox lesions to retract and subside internally. It is appropriate to use Chen Zhongwen's Yigong San (Isolated Efficacy Powder). (Formula see "Smallpox eruption slow" section.)
[Qian] (Qian Yi) If shivering chills are severe accompanied by teeth grinding and black depression of the lesions, Baixiang Wan (Hundred Miracles Pill) should be used to purge. (Formula see "Black crusts" section.) I once treated smallpox with shivering chills using Baizhu (Atractylodis Rhizoma), Danggui (Angelicae Sinensis Radix), Huangqin (Scutellariae Radix), etc., and achieved a cure.
[Dan] (Zhu Danxi) Hu family's smallpox fever is due to blood deficiency with residual pathogenic heat/toxin.
Chenpi (Citri Reticulatae Pericarpium), Baizhu (Atractylodis Rhizoma), Dangguishen (Angelicae Sinensis Radix), Baishaoyao (Paeoniae Radix Alba) (each 3 qian), Niubangzi (Arctii Fructus, crushed, stir-fried, 2 qian), Mutong (Akebiae Caulis), Xijiao (Rhinocerotis Cornu), Sheng Ganjiejie (Glycyrrhizae Radix et Rhizoma raw with nodes), Chuanxiong (Ligustici Chuanxiong Rhizoma) (each 1.5 qian).
Divide into six doses. Decoct with one and a half bowls of water. Take slightly warm before meals.
[Huo] (from *Huoyou Xinshu*) Lianqiao San (Forsythia Powder) treats all types of heat; it also treats sores and rashes with miraculous effect.
Lianqiao (Forsythiae Fructus), Fangfeng (Saposhnikoviae Radix), Zhizi (Gardeniae Fructus), Gancao (Glycyrrhizae Radix et Rhizoma) (equal parts).
Grind the above into fine powder, decoct with water and take orally. (Haizang said: Treats heat located in the exterior without jue-ni [cold extremities]; this is a Shaoyang meridian formula.)
Yilao said: Liangge San (Cool the Diaphragm Powder) without Dahuang (Rhei Radix et Rhizoma) and Mangxiao (Natrii Sulfas) can resolve heat in the six meridians. This not only clears heat but also treats pediatric exanthematous heat conditions, promoting eruption. Use half the original formula mixed with half Fangfeng (Saposhnikoviae Radix) and Jingjie (Schizonepetae Spica), decocted in plain water.
[He] (Zhang Yuansu) Zhizi Jinhua Wan (Gardenia Golden Flower Pill)
Huangqin (Scutellariae Radix), Huanglian (Coptidis Rhizoma), Huangbai (Phellodendri Cortex), Shan Zhi (Gardeniae Fructus, stir-fried) (equal parts).
Grind into fine powder, make pills with water drops, size of beans. Take five pills per dose, washed down with warm water.
[Qian] (Qian Yi) Sanhuang Wan (Three Yellows Pill) treats various heat syndromes.
Huangqin (Scutellariae Radix, 0.5 liang), Dahuang (Rhei Radix et Rhizoma, baked), Huanglian (Coptidis Rhizoma) (each 2.5 qian).
Grind into fine powder, make pills with paste, size of mung beans. Take five to seven pills, or fifteen to twenty pills per dose, after meals with rice drink.
Regarding Qian's statements above: For mild heat, use detoxification. Haizang considers that detoxifying agents include Sanhuang Wan (Three Yellows Pill) and Jinhua Wan (Golden Flower Pill).
Sisheng San (Four Saints Powder) (Formula see "Eruption slow"). Haizang Zicao Mutong Tang (Purple Coral and Akebia Decoction) (Formula see same). Daochi San (Directing Red Powder) (Formula see "Treating deficiency/excess"). Bazheng San (Eight Upright Powder) (Formula see "Strangury section").
[Dan] (Zhu Danxi) At the initial stage, if there is irritability, delirious speech, mania, extreme thirst and desire to drink, drinking water will later result in uneven crusting of the lesions. Urgently use cooling herbs to resolve the branch symptoms, such as Yiyuan San (Benefit Origin Powder).
The above five formulas are diuretic agents to reduce fever. Qian said: For severe heat, promote urination. He also said: Body heat, irritability and thirst, abdominal fullness with panting, difficult urination and defecation, red face, chest oppression and confusion, severe vomiting—these indicate the need to promote urination. Using Daochi San (Directing Red Powder) here exemplifies this principle.
[Qian] (Qian Yi) Xuanfeng San (Dispersing Wind Powder)
Binglang (Arecae Semen, two pieces), Chenpi (Citri Reticulatae Pericarpium), Gancao (Glycyrrhizae Radix et Rhizoma) (each 0.5 liang), Qianniu (Pharbitidis Semen, four liang, half raw half cooked).
Grind into fine powder. For children aged two or three, mix with honey water and take 0.5 qian; for older children, take 1 qian. Take before meals. Yilao adds Fangfeng (Saposhnikoviae Radix).
[Yun] (Yun Qizi) Tongge Wan (Unblocking Diaphragm Pill) is an herb to unblock qi and blood in the upper and lower jiao.
Dahuang (Rhei Radix et Rhizoma), Qianniu (Pharbitidis Semen), Mutong (Akebiae Caulis) (equal parts).
Grind into fine powder, make pills with water drops, size of millet grains. Take thirty to fifty pills per dose, adjusting according to the child's age and deficiency/excess status.
Regarding Qian's statement above: For severe heat, if promoting urination does not improve the condition, it is suitable to use Xuanfeng San (Dispersing Wind Powder) to purge.
[Zihe] (Zhang Congzheng) Cai Heliu had a boatman who accidentally saw a bundle of decaying cattail reeds floating downstream. As he moored the boat near the bank, he seemed to hear faint crying. Upon opening and inspecting, he was surprised to find a child, about four or five years old, covered entirely in dense sores/rash with no space between them, yet his eyes were bright and clear. The child was hungry and asking for food, so the boatman fed him porridge. The child's wife was angry, saying, "We have many children of our own; how can we risk catching this sore disease?" She privately reasoned that since the Cai River current was slow, the child must not be far. Holding one of the child's shoes, she went upstream against the current, asking everywhere, but no one had seen such a child. After walking twenty li, she reached another village. The boatman shouted in the market, "A child of this age, covered in sores, was thrown into the river and has now revived." Suddenly, someone came out from a tavern and said, "I am so-and-so from such-and-such village. My child, aged four or five, died of sores." The boatman showed the shoe, and the man cried, "This is my son!" He rushed to check, was shocked to see the child alive, shed tears, bowed in gratitude to the boatman, and took the child home. The child is now over twenty years old. This child was originally dead but lived because of water. However, it is unknown what specific sore/rash disease he suffered from, whether it was a cold syndrome or a heat syndrome.
