[Chen] When a patient with smallpox (Dou Chuang) presents with fever, dry mouth, and incessant irritability and thirst, water must absolutely not be given, nor should cold substances such as honey, persimmons, watermelon, or oranges be consumed.
(If the pulse is excess and there is internal excess heat, cold foods need not be avoided.) During the period when smallpox lesions are about to scab or are in the process of scabbing, if they suddenly fail to scab, accompanied by abdominal distension, irritability, and thirst, cold substances like water and honey must not be administered. Similarly, if the head feels warm while the toes are cold, or if there is diarrhea, thirst, and rapid breathing, these substances must not be given. By the twelfth or thirteenth day, after the scabs have fallen off, if the scars remain dark, either depressed or raised, and the skin is still tender, bathing should be avoided, and roasted, fried, spicy, pungent (Wu Xin), or toxic foods should not be eaten, lest heat-toxin fumigate the chest and diaphragm, causing many opacities (Yi Zhang) to form in the eyes.
[Hai Zang] The common people only know how to pity and spoil, trusting in customs rather than administering medicine. They worry only after the disease has become severe, stroking and comforting the patient without restriction. How can they know that such affection is sufficient to harm them, and such cherishing is sufficient to abandon them? Failing to treat early and failing to complete the treatment properly leads ultimately to death, resulting in labor without merit. At this point, they blame the physician. Alas! Their reasoning is profoundly flawed.
[Dan Xi] If the eruption is sparse, there is no toxicity; if dense, there is toxicity. Using cooling herbs to resolve it, even if dozens of doses are taken, causes no harm and avoids the risk of damaging the eyes.
[Jie Gu] If the eruption appears densely packed like needle heads from the start, and the condition is severe, one should mildly release the exterior and cool the interior. The primary formula is Lianqiao Shengma Tang (Forsythia and Cimicifuga Decoction).
[Yuan Zhi] Niuzangzi Tang (Arctii Fructus Decoction): Treats rashes that have erupted densely with heat on the body surface. Administer this medicine urgently to prevent subsequent symptoms of cyanosis, drying, and black sinking.
Arctii Fructus (Niuzangzi, stir-fried, 2 qian)
Angelicae Sinensis Radix Corpus (Danggui Shen, soaked in wine)
Glycyrrhizae Radix et Rhizoma (Gancao, honey-fried)
Bupleuri Radix (Chaihu)
Forsythiae Fructus (Lianqiao)
Scutellariae Radix (Huangqin)
Astragali Radix (Huangqi) (each 1 qian)
Paeoliae Cortex (Digupi, 2 qian)
Decoct these ingredients. Take 2 qian per dose, decoct in water, remove the dregs, and take warm on an empty stomach. After taking the medicine, do not give milk or food immediately.
[Tian Shi] Tiao Gan San (Liver-Regulating Powder): Treats excessive rash eruption; suitable for administration to prevent the rash from affecting the eyes.
Rhino-Cornu (Sheng Xijiao, 2.5 qian)
Gentianae Radix (Caolongdan, 1 qian)
Astragali Radix (Huangqi, 0.5 liang)
Rhei Radix et Rhizoma (Dahuang, 2 qian)
Mori Cortex (Sangbaipi, stir-fried until yellow, 0.5 liang)
Ephedrae Herba (Mahuang, nodes removed)
Uncariae Ramulus cum Uncis (Goutengzi) (each 1 qian)
Gypsum Fibrosum (Shigao, 0.5 liang)
Trichosanthis Semen (Gualouren, peel removed)
Glycyrrhizae Radix et Rhizoma (Gancao, stir-fried, each 2 qian)
Grind into a coarse powder. Take 2 qian per dose, decoct in water, and take warm after meals. Slight purgation indicates efficacy.
[Hai Zang] For eruptions that are too profuse, use Xijiao Dihuang Tang (Rhino-Cornu and Rehmannia Decoction) or Digupi Niuzangzi Tang. (Xijiao Dihuang Tang formula is seen under Epistaxis. Digupi Niuzangzi Tang is Digupi San with added Arctii Fructus.)
[Dan Xi] Another prescription to resolve toxin from sores:
Luffae Fructus Retusus (Sigua)
Cimicifugae Rhizoma (Shengma)
Paeoniae Radix Alba (Shaoyao, wine-stir-fried)
Glycyrrhizae Radix et Rhizoma (Gancao, raw)
Crataegi Fructus (Shanzha)
Phaseoli Semen Mungo (Heidou)
Phaseoli Semen Adzukii (Chixiaodou)
Rhino-Cornu (Xijiao)
Decoct these ingredients in water and take orally.
[Jie Gu] If the eruption is already dense and severe, accompanied by slight wheezing and desire to drink, this indicates a heat syndrome. Use wind-dispelling herbs to mildly purge.
[Dan Xi] Treats men suffering from smallpox due to劳伤 (labor injury/overexertion), presenting with unconsciousness and lack of intact skin on the body. (See formula under "Smallpox Not Yet Healed".)
