Madness of excessive laughter for over two years, yet still feeling shy when seeing people, refusing to disclose the condition. Previous physicians attributed the laughter to heat entering the Blood Chamber (Xue Shi) and intended to treat it as Mania (Dian). I inquired whether the laughter occurred during menstruation. The patient's father replied it was after menstruation. I reasoned that since the laughter did not occur post-menstruation, and the woman laughed alone without reaching a state of severe mania or incoherent speech, this was actually due to hidden emotional distress difficult to express. I then asked about her marital relationship. The father stated his son was a scholar studying away from home, returning only for one night before going back to school; upon return, he sometimes slept with the father-in-law and never entered the room to talk with his wife. I told the father that treatment must proceed from two aspects: first, allowing the woman to live with her husband to fulfill her desires, thereby resolving her hidden distress. Once Liver Qi Stagnation is resolved, Gallbladder will naturally regain its upright strength, and the laughter will subside. Medication serves only as an auxiliary measure. Prescribed Wendan Tang (Phlegm-Draining Decoction) modified:
Zhu Ru (Bambusa Textilis Caulis), Zhi Shi (Immature Bitter Orange) 1.5 qian, Yuan Zhi (Polygala Root), Fu Shen (Poria with Hostwood), Dang Shen (Codonopsis Root), Jie Shou Chang Pu (Acorus Tatarinowii Rhizome), Yu Jin (Curcuma Aromatica Rhizome), Chen Pi (Citrus Reticulata Pericarpium) each 2 qian, Ban Xia (Pinellia Tuber) 1.5 qian, Sheng Jiang (Fresh Ginger), Da Zao (Jujube Fruit) each 2 pieces.
The father followed my method and achieved remarkable results.
(17) Stroke-like Syndrome
Male, 60 years old
Suffered from severe Cold Damage (Shang Han), cured after taking dozens of doses of medicine himself. One day, while getting a haircut, the eyebrow corner bled slightly, and he suddenly collapsed. Pulse was extremely surging and large, tongue center red with slight yellow coating, lips purple, eyes closed, teeth clenched. Diagnosed as Stroke affecting the Blood aspect, treated with Hei Shen San (Black Divine Powder).
Hei Jing Jie (Schizonepetae Herba Praeparata cum Cinere) 5 qian, decocted in water, administered with Tong Bian (Child's Urine). After one hour, the patient opened his eyes, sighed, left hand trembled, seemed slightly conscious but silent, salivating. Instructed to take:
Hei Dou (Sojaes Semen Nigrum) 3 he (stir-fried), decocted together with Hei Jing Jie. At 9 PM that night, the patient cried "Ouch," turned to sleep facing inward, with no major changes.
Second-day examination: complexion faintly red, pulse no longer surging but fluctuating, lips unchanged. Asked what he wanted to eat? Merely shook head slightly, unable to speak, occasionally touching chest with hand. His son said: "Ate a bowl of glutinous rice and preserved meat the day before, felt chest oppression since." Treated as food stagnation post-illness, prescribed Yueju Wan (Yueju Pill) modified:
Shan Zha Tan (Crataegi Fructus Carbonized) 2 qian, Cang Zhu (Atractylodis Rhizoma) 2 qian, Shen Qu (Massa Medicata Fermentata) 2 qian, Ban Xia (Pinellia Tuber) 1.5 qian, Guang Pi (Tangerine Peel) 1.5 qian, Ji Nei Jin (Galli Gigerii Endothelium Corneum) 2 qian, Chuan Shi Hu (Dendrobii Officinalis Caulis) 2 qian, Chen Zong Tan (Charred Rice Dumpling Wrapper) one piece (decocted with pork bone ash). Two doses taken, illness significantly reduced, chest oppression resolved, able to speak. Cured by self-administering Baohe Wan (Harmonize the Stomach Pill) and Huang Jin Wan (Golden Pill).
Female, 55 years old
Obese constitution, accustomed to alcohol and meat. One day while eating, bowls and chopsticks fell suddenly, body threw under table, overturning dishes. Saliva foaming, sudden mutism, jaw clenching, lips cyanotic, eyes closed, mouth deviated, body immobile, face red with sweating, coma, pulse present but rate indistinct. Identified as Stroke-like syndrome affecting the Fu organs, likely Heat-Phlegm obstructing ascending and descending mechanisms. Wan Mou nearby suggested: "Elderly stroke is mostly deficiency, prescribe tonics." I believed although elderly stroke is often deficiency, this case was Wind generated by extreme Heat, requiring clearing Heat and resolving Phlegm. Large doses of tonics would be unsuitable. Must first open orifices and unblock passages; once consciousness returns, then prescribe medication. Administered one pill of Zhibao Dan (Precious Treasure Pill). Three hours later, patient exhaled deeply, wiped sweat, requested tea. Upon sipping, suddenly vomited a basin of phlegm-saliva, profuse sweating, tongue stiff, mouth deviated left, slight picking at clothes, complaining of internal burning heat. Another day, another Zhibao Dan pill given, plus prescription:
Zhu Ru (Bambusa Textilis Caulis) 3 qian, Jie Shou Chang Pu (Acorus Tatarinowii Rhizome) 1.5 qian, She Dan Jian Bei Mo (Snake Gallbladder and Fritillaria Thunbergii Bulb Powder) one unit, Jing Jie Tan (Schizonepetae Herba Carbonized) 2 qian, Gou Teng (Uncariae Ramulus cum Uncis) 2 qian, Gua Lou Ren (Trichosanthis Semen) 2 qian, Guan Ji Li (Tribuli Fructus) 2 qian, Yu Jin (Curcuma Aromatica Rhizome) 2 qian, Bai Fan (Alumen) 2 qian, Bo He (Menthae Haplocalycis Herba) 2 qian, Ju Luo (Citri Reticulatae Pericarpium Venae) 2 qian, Xian Sheng Di (Rehmanniae Radix Recens) 2 qian, Dang Gui (Angelicae Sinensis Radix) 2 qian, Gui Ban (Testudinis Carapax et Plastrum) 3 qian, Xian Sang Zhi (Mori Ramulus Recens) 3 chi (substitute Sang Ji Sheng if unavailable), Zhu Li (Bambusa Textilis Succus) one cup.
Four packets taken consecutively, mouth deviation improved, hands could touch clothes, speech clearer. Modified prescription:
Ling Yang Jiao (Capricornis Cornu) 1 qian, Guan Ji Li (Tribuli Fructus) 2 qian, Ci Ji Li (Tribuli Fructus Spinosus) 1.5 qian, Sang Ji Sheng (Taxilli Caulis) 2 qian, Dang Gui (Angelicae Sinensis Radix) 2 qian, Gui Ban (Testudinis Carapax et Plastrum) 3 qian, Gou Teng (Uncariae Ramulus cum Uncis) 2 qian, Chuan Jian Bei (Fritillariae Thunbergii Bulbus) 3 qian, Hai Fu Shi (Pumex) 2 qian, She Dan Chen Pi (Citri Reticulatae Pericarpium with Snake Gallbladder) one unit (mixed).
Two doses taken, speech articulate, tongue stiffness resolved, hands could hold bowl, but finger manipulation clumsy, loud snoring at night.
Patient anxious for recovery, fearing deficiency, took Western Ginseng (Yang Shen) 1 liang steamed in water daily. Three days later, relapse: enuresis, mutism, jaw clenching, inability to swallow fluids. Reconsulted me. I believed recovery would take months, strict avoidance of tonics needed, hospitalization recommended.
Hospitalized 40 days, no significant improvement, subsisting on lily powder or chicken soup, discharged. Another physician prescribed Xiao Xuming Tang (Minor Life-Sustaining Decoction), causing profuse sweating and limb shaking. Another physician prescribed Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) 5 qian charred and powdered, mixed with water. Four doses taken, slight consciousness returned. Two days later, reconsulted me. Noting prior syphilis and excessive use of Ginseng and Deer Antler, prognosis poor, disability inevitable. This woman bedridden for six years due to reckless tonic use should serve as a lesson.
(18) Stubborn Phlegm Obstructing Orifices
Male, 60 years old
Accustomed to alcohol, pork, and beef, frequent wheezing and cough. Returned from provincial capital, suddenly fell unconscious that evening. Pulses floating and large, tongue normal color, lips dark red, face cyanotic, dazed in bed. Every morning after breakfast, experienced vertigo, unable to open eyes until 4 PM. Judged vertigo not as Collapse syndrome, but stubborn Phlegm blocking Heart orifices, preventing Clear Yang from ascending, mimicking syncope. Prescription:
Shi Chang Pu (Acori Tatarinowii Rhizoma), Yu Jin (Curcumae Rhizoma), Hai Fu Shi (Pumex), Dai Zhe Shi (Haematitum), Ju Luo (Citri Reticulatae Pericarpium Venae), Xuan Fu Hua (Inulae Flos wrapped in cloth) each 2 qian, Jing Xing (Arisomatis Rhizoma) 1.5 qian, Xi Xiong (Ligustici Chuanxiong Rhizoma) 1.5 qian, Zhu Li (Bambusa Textilis Succus) one cup. Instructed to consume lean pork stewed clear broth (defatted), temporary porridge diet, no solid food. Condition stabilized. Clear stewed broth aids Stomach Yang ascent, benefits Stomach Qi, expels Phlegm. Patient has strong Liver due to alcohol, dry Stomach, stubborn Phlegm blocking Heart orifices; treatment focused on resolving Phlegm, soothing depression, relieving vertigo.
(19) Dian-Kuang (Manic-Depressive Disorder)
Female, 30 years old

Two doses brought peace.
Female, 33 years old
Running outside, speaking wildly, ignoring social boundaries, shameless. Arms tightly contracted, pulse undetectable. Based on physical strength and wild behavior, diagnosed as Excess syndrome. Laughing expression showed frowning and anger, suggesting hidden grievance unfulfilled, igniting Liver Fire upward, causing Hyperactive Heart Yang and mental confusion. Body hot to touch. Initially prescribed Xiaoyao San (Free and Easy Wanderer Powder) plus Tian Zhu Huang (Bambusae Concretio Silicea) 2 qian, Hu Po Xie (Electrum) 1.5 qian, Tie Luo (Ferrum) one pinch, decocted. Two doses taken, followed by:
Yu Jin (Curcumae Rhizoma) 2 liang, Bai Fan (Alumen) 1 liang, water-made pills, size of pepper, 20 pills swallowed daily with warm water until finished.
Illness gradually decreased, no longer cursing or hot, but self-talking, laughing, anorexia, craving cold food, demanding to return to Henan. Ten days later, still muttering, constipation, able to go out. Instructed to take Tianwang Buxin Dan (Emperor of Heaven's Heart-Nourishing Pill) 5 qian daily with warm water. Also instructed to stew pig stomach with one chi of red satin fabric twice, achieving stability.
Male, age unknown
Jumping and talking wildly, restless day and night, anorexic. Reported madness after fever, previous medicines ineffective. Pulse deep to bone, pale complexion, brown tongue center, solitary laughter, poking people, self-talking incessantly, startled by sounds. (20) Heart-Kidney Non-Interaction
Male, age unknown
Dizziness, sudden collapse, unconsciousness, recurring every few days. Pulse floating, rapid, soft; pale complexion, blood failing to nourish features, spiritual fatigue, emaciation, furrowed brows, low voice, dream-disturbed sleep, waist distension, occasional seminal emission. Both Chinese and Western medicine treated as Epilepsy (Xian Zheng), long-term medication yielded little effect. I diagnosed Heart-Kidney Non-Interaction: Clear Yang not ascending, Stomach Yin not distributing, Liver-Kidney Heat forcing separation, Water and Fire failing to interact, causing vertigo. Etiology: excessive masturbation depleting Kidney Water, failing to nourish Wood. Treatment: Nourish Kidney, regulate Liver.
Yuan Zhi (Polygalae Radix) 2 qian, Fu Shen (Poria with Hostwood) 3 qian, Gui Ban (Testudinis Carapax et Plastrum) 3 qian, Suan Zao Ren (Ziziphi Spinosae Semen) 2 qian, Shu Di (Rehmanniae Radix Preparata) 4 qian, Dang Gui (Angelicae Sinensis Radix) 3 qian, Nu Zhen Zi (Ligustri Lucidi Fructus) 2 qian, Li Shen Xu (Codonopsis Radix Tenuis) 2 qian, Fu Pen Zi (Rubi Fructus) 2 qian, Tu Si Zi (Cuscutae Semen) 2 qian, Hang Shao (Paeoniae Radix Alba) 2 qian, Rou Cong Rong (Cynomorii Herba) 2 qian, Gou Qi Zi (Lycii Fructus) 2 qian. Powdered, honey-pilled, size of paulownia seed, 40 pills swallowed daily with salted water.
Medicated for months, no recurrence for over a year. Reported one recent episode.
Female, 60 years old
Palpitations, irritability, restlessness day and night, startled by sounds, unable to go out, fear of strangers. Family sought single prescriptions due to fear of strangers. Instructed to buy raw and cooked Suan Zao Ren (Ziziphi Spinosae Semen) 1 liang each, decoct concentrated juice, taken at night. Next day consultation: patient emaciated, dilated pupils, fearful state, companion advising calmness. Pulse floating and wiry, Guan position not floating but forceful, rate indistinct. Diagnosed as Liver Heat, Gallbladder Cold, Phlegm obstructing ascent/descent. Prescription:
Zhu Ru (Bambusa Textilis Caulis) 3 qian, Chang Pu (Acori Tatarinowii Rhizoma), Yu Jin (Curcumae Rhizoma), Gua Lou Ren (Trichosanthis Semen), Jian Bei (Fritillariae Thunbergii Bulbus), Ju Luo (Citri Reticulatae Pericarpium Venae), Suan Zao Ren (Ziziphi Spinosae Semen), Yuan Zhi (Polygalae Radix) each 2 qian, Zhu Li (Bambusa Textilis Succus) one cup.
Two doses taken, pulse unchanged, indicating ineffectiveness. Switched to Heart-Nourishing and Spirit-Calming agent.
Fu Shen (Poria with Hostwood) 2 qian, Yuan Zhi (Polygalae Radix), Zhi Shi (Aurantii Immaturus Fructus), Dang Gui (Angelicae Sinensis Radix), Bai He (Lilii Bulbus), Ye Jiao Teng (Polygoni Multiflori Caulis), E Jiao Zhu (Asini Corii Colla prepared beads), Lian Zi Xin (Nelumbinis Plumula) each 2 qian, Suan Zao Ren (Ziziphi Spinosae Semen) 1.5 qian, Bai Zi Ren (Platycladi Semen) 1.5 qian.
Three doses taken, pulse no longer floating, Chi position rapid, less fearful. Changed prescription: Yuan Zhi (Polygalae Radix) 2 qian, Suan Zao Ren (Ziziphi Spinosae Semen) 3 qian, Ling Ci Shi (Magnetitum) 2 qian, Fu Shen (Poria with Hostwood) 2 qian, Xi Dang (Codonopsis Radix) 3 qian, Bai Zi Ren (Platycladi Semen) 2 qian, Lian Zi Xin (Nelumbinis Plumula) 2 qian, Nu Zhen Zi (Ligustri Lucidi Fructus) 2 qian, Shu Di (Rehmanniae Radix Preparata) 2 qian, E Jiao Zhu (Asini Corii Colla prepared beads) 2 qian, Tie Luo (Ferrum) one pinch, Hu Po Xie (Electrum) 1.5 qian.
Four doses taken, spirit normalized, all symptoms resolved. When asked for rationale, I explained: Heart is Fire organ, Kidney is Water organ. Kidney Qi carrying Phlegm attacks Heart; Water controls Fire, causing Heart oscillation and loss of autonomy. Must restore positions so they do not control but support each other. Thus, first resolve Phlegm and warm Gallbladder, then nourish Heart and calm Spirit, finally anchor and nourish Heart and Kidney.
(21) Yin Pathogen Attacking Liver
Male, 33 years old
Mouth and eyes pulled to left, eyes swollen and stuffy, walking forward fall, normal diet, pulses floating, red tongue. Diagnosed as Yin Pathogen attacking Liver. First prescribed:

Ten doses resulted in cure.
(22) Vertigo and Collapse
Male, 40 years old
Involved in litigation, imprisoned over a year, photophobia, unable to distinguish colors, collapsed. Pulse fluctuating, four major pulses indistinguishable. Body smooth, cool, resembling Yin-Yang exhaustion. Pupils absent, sclera yellow inside blue outside. Diagnosed as Yang Deficiency with Yin encroachment, Clear Yang not ascending, Yin-Dampness obscuring vision, causing vertigo and collapse. Prescribed Renshen Baihu Tang (Ginseng White Tiger Decoction) with heavy Cang Zhu (Atractylodis Rhizoma).
One dose taken, vertigo and photophobia reduced, but aversion to food odor, brown tongue, pulse with "fetal breath" quality, rate still unclear. Believed previous prescription unsuitable. Switched to Dampness-Overcoming and Yang-Recovering method.
Cang Zhu (Atractylodis Rhizoma) 4 qian, Gui Zhi (Cinnamomi Ramulus) 1.5 qian, Duan Shi Gao (Gypsum Fibrosum calcined) 3 qian, Bai Zhu (Atractylodis Macrocephalae Rhizoma) 3 qian, Fu Zi (Aconiti Lateralis Radix Preparata) 2 qian, Si Gua Luo (Luffae Fructus Retinervus) 2 qian, Qin Jiao (Gentianae Macrophyllae Radix) 2 qian, Yin Chen (Artemisiae Scopariae Herba) 3 qian, Wei Jiang (Roasted Ginger) 1 qian, Hong Zao (Jujubae Fructus) 3 pieces, decocted. Instructed to buy 1 jin of Cang Zhu, smoke under bed for patient to inhale; if sweating occurs, remove Cang Zhu, ventilate, then resume medication.
Two days later, patient sat up, no collapse, severe nasal congestion, one sneeze. Switched to Qianghuo Shengshi Tang (Notopterygii et Ligustici Decoction for Overcoming Dampness):
Chuan Qiang (Notopterygii Rhizoma et Radix) 2 qian, Cang Zhu (Atractylodis Rhizoma) 2 qian, Xiang Fu (Cyperi Rhizoma) 3 qian, Tai Wu (Illicii Fructus) 1.5 qian, Kou Ren (Amomi Fructus) 2 qian, Fu Zi (Aconiti Lateralis Radix Preparata) 2 qian, Yi Yi Ren (Coicis Semen) 3 qian, Wei Ling Xian (Salviae Miltiorrhizae Radix et Rhizoma - *Note: Context suggests Saposhnikoviae Radix usually, but text says Wei Ling Xian*) 2 qian, Si Gua Luo (Luffae Fructus Retinervus) 2 qian, Sheng Jiang (Fresh Ginger), Da Zao (Jujube Fruit) each 2 pieces.
Two doses taken, symptoms reduced. Patient refused further visits. Later heard he liked turtle and eel, developed Yellow Puffy Disease (Huang Pan Bing).
(23) Wind-like Syndrome ■
Male, 36 years old
No pain or itch, unable to turn or move, no sweat, eats while sleeping, requires carrying for toiletries, robust voice, peach-blossom complexion. Wife asked for treatment; I remained silent due to previous failures. Soon, a herbal gatherer came buying Red Niuxi, heard of the disease, offered treatment. Desperate, patient agreed. Cost: "Any amount, pay after cure." Next day, sent one packet of herbs, decoct as tea daily. Taken for over ten days, patient felt lighter. Herbalist returned twice with fresh herbs, stripped leaves, boiled down to paste, added small amount of powder. Paste ready, instructed to mix with wine, one cup daily until finished. After 20+ days, patient could turn, felt sweating, legs gained mobility. After finishing paste (~40 days), could get out of bed. Herbalist returned with miscellaneous herbs, one dose, decocted for full cure. Effective.
Story spread, skeptics visited. Herbalist visited me, revealed effective herb was Qin Jiao (Gentianae Macrophyllae Radix), claiming it searches out Wind. Usage same, preparation different. Local vs mountain Qin Jiao differs; decoction searches Wind/Damp, paste unblocks joints. Must harvest at flowering, dry under eaves; wind-drying ensures pliability. Without wind-drying, dry and brittle, lacking horizontal movement properties. Paste making requires specific length ("Yuan Cun") for connection. Later found in Feng's Brocade Bag single-use Qin Jiao cases with miraculous effects.
(24) Dreaming of Sexual Intercourse with Spirits
Female, 22 years old
Persistent fever, claims ghosts are present, family panicked. Attempts included soul redemption, Tai Sui appeasement, spirit medium invocation, hired six Taoists for rituals. Parents bewildered. I arrived: crowded with mediums, Seven Star lamps under bed, tables full of candles and Buddhas, hot air rising, hard to sit. Asked some to leave.
Patient face bright red, tongue red and moist, lips slightly red, six pulses floating and rapid, consistent throughout. Appearance not emaciated, crumbs in mouth. Bedside full of food, knowing intake exists but appetite lacking. Mother weeping, stated girl hadn't swallowed rice gruel for 7-8 days. During profuse sweating, cried "Someone! I'm afraid!" hiding in my arms. Extremely heat-intolerant, required cooling fan to settle. Sleep-wake cycle disrupted, unable to rest peacefully.
