Gao XX from Shanxi, aged 28, residing temporarily in Tianjin, contracted Warm Disease (Wen Bing) during the mid-autumn season.
Etiology: Invited by a friend to drink alcohol, he consumed excessive amounts of wine and subsequently drank large quantities of hot tea. This led to profuse sweating over the entire body. Upon going outside, he was exposed to wind pathogen.
Symptoms: Pain in all bone joints; body temperature of 39.4°C; heat sensation in the heart region accompanied by thirst; thin, slightly yellow tongue coating. Dry stool, scanty dark urine; occasional dry cough; extreme physical soreness and weakness; dizziness upon movement; pulse rapid exceeding five beats per cycle (approx. >100 bpm), floating, wiry, and forceless. From the onset of illness to this point, forty days had passed, yet repeated medical consultations and medication proved ineffective.
Diagnosis: This condition is merely a slight external contraction and is not difficult to treat. Due to improper treatment, the exterior pathogen has remained unresolved for over a month, making it impossible to clear the internal heat. The patient’s lack of appetite due to illness, combined with the prolonged heat consuming Yin fluids, has transformed the initial excess heat of external contraction into deficiency heat of internal injury. With both types of heat present, the condition becomes increasingly difficult to treat. Therefore, the primary approach should be using substantial doses of herbs that nourish True Yin, assisted by herbs that release the exterior and purge heat.
Prescription: Raw Huai Yam (Sheng Huai Shan Yao) 30g, Raw Huai Rehmannia Root (Sheng Huai Di Huang) 30g, Scrophularia Root (Xuan Shen) 30g, Glehnia Root (Sha Shen) 18g, Raw Hangzhou White Peony Root (Sheng Hang Shao) 18g, Large Sweet Goji Berry (Da Gan Gou Qi Zi) 15g, Asparagus Tuber (Tian Dong) 15g, Trichosanthes Root (Tian Hua Fen) 15g, Talcum (Hua Shi) 9g, Licorice Root (Gan Cao) 9g.
Decoct together to make one large bowl of soup, divided into three doses taken warm. For the first dose, administer half a grain of Aspirin dissolved in white sugar water orally before taking the herbal decoction.
Follow-up Visit: After the first dose, the patient sweated profusely, and joint pain ceased to be noticeable. After completing the second and third doses, the fever subsided by more than half, urination became unobstructed, and the pulse was no longer floating and wiry, showing slightly increased strength. Consequently, the original prescription was slightly modified for continued use.
Prescription: Raw Huai Yam (Sheng Huai Shan Yao) 30g, Raw Huai Rehmannia Root (Sheng Huai Di Huang) 24g, Scrophularia Root (Xuan Shen) 18g, Glehnia Root (Sha Shen) 18g, Large Sweet Goji Berry (Da Gan Gou Qi Zi) 18g, Asparagus Tuber (Tian Dong) 18g, Talcum (Hua Shi) 9g, Licorice Root (Gan Cao) 6g, Donkey-hide Gelatin (Zhen E Jiao) 9g (crushed).
Nine ingredients in total. First, decoct the preceding eight ingredients in two large cups of water, strain, and dissolve the crushed Donkey-hide Gelatin into the liquid. Divide into two doses taken warm. For the first dose, still administer one-third of a grain of Aspirin dissolved in white sugar water orally beforehand. The reason for adding Donkey-hide Gelatin in this formula is its dual efficacy in nourishing Yin and moistening dryness to relieve constipation.
Outcome: After the first dose, the patient sweated slightly again. After taking the remaining two doses, thirst ceased, and bowel movements occurred on the same day. Immediately after defecation, the patient felt mentally refreshed, and the burning heat completely disappeared. The disease was thus cured.
Explanation: The formula heavily employs cooling and moistening herbs to nourish True Yin, thereby retreating deficiency heat. Simultaneously, Aspirin is used to release the muscle layer, and Talcum promotes diuresis, thereby providing an exit route for excess heat. Regarding the sweating observed after administering half a grain of Aspirin: individuals with deficient constitutions sweat easily; furthermore, those with internal dry-heat often sweat spontaneously when receiving cooling and moistening medications that provide hydration and relief.
