An elderly surnamed Li from Tianjin, over sixty years of age, contracted Huoluan (cholera-like syndrome) in midsummer.
Etiology: Due to the hot weather, she went out for some business and was exposed to summer-heat on the road. Upon returning home, she cooked herself, inhaling excessive charcoal smoke, which led to the onset of Huoluan.
Symptoms: Nausea, vomiting, abdominal pain, and diarrhea. Within ten hours of onset, vomiting and diarrhea occurred more than ten times. Her hands and feet were cold; hand coldness extended to the elbows, and foot coldness extended to the knees. She felt heat in the heart region. Her pulse was deep, fine, and nearly imperceptible, with fewer than four beats per respiration.
Diagnosis: This is caused by Huoluan pathogens entering the Zang-Fu organs along with damp-summer-heat. The heart is paralyzed by the pathogens, causing the mechanism of heartbeat to nearly cease, resulting in a deep, fine, and slow pulse. The blood vessels lack the strength to circulate effectively to the limbs, causing coldness in the hands and feet. The pathogens invade the intestines and stomach, disrupting their Qi transformation functions. Additionally, the healthy Qi of the Zang-Fu organs repels the invading pathogenic Qi, leading to nausea, abdominal pain, and alternating vomiting and diarrhea. The sensation of heat in the heart region is indeed due to the presence of summer-heat; however, even in Yang-type Huoluan without summer-heat, heat in the heart region is common. Treatment should employ formulas that detoxify and clear heat.
Prescription: Weisheng Fangyi Baodan (160 pills), Lizhong Dan (4 qian), Yiyuan San (4 qian).
First, administer Weisheng Fangyi Baodan in three doses, taken with warm water, approximately every half hour. After completing the three doses, the nausea and abdominal pain should resolve, and vomiting and diarrhea will likely subside accordingly. If heat in the heart region persists after recovery, mix the latter two medicines evenly and also divide them into three doses to be taken with warm water. Take once every hour; if the heat subsides, there is no need to finish the remaining doses.
Outcome: After taking Weisheng Fangyi Baodan in three doses, the nausea, vomiting, abdominal pain, and diarrhea all resolved. However, the heat in the heart region did not diminish. Subsequently, Lizhong Dan and Yiyuan San were mixed evenly and taken in three doses. The heat then disappeared, and the patient made a full recovery.
