An elderly woman surnamed Xu in Tianjin, nearing fifty years of age, contracted malaria (Malaria) in late summer.
Etiology: She managed the household frugally and diligently, performing many domestic chores personally. Additionally, a flour mill was located beside her residence, and the family's meals were supplied from there. Due to labor combined with exposure to summer-heat (Summer-heat), she developed malaria.
Symptoms: The disease manifested every other day, beginning with chills followed by fever. The chills were mild, while the fever was severe. She experienced nausea, anorexia (loss of appetite), frequent sensations of heat in the heart region, and a craving for cold foods. Her pulse was wiry and hard on the left side, and surging and full on the right side. Her stool was dry, and her urine was scanty, red, and painful. Multiple courses of medication proved ineffective.
Diagnosis: This condition involves latent malarial pathogens (Malarial pathogen) in the Liver and Gallbladder, and stagnant summer-heat (Summer-heat) in the Stomach. The combination of summer-heat and malarial pathogens results in alternating chills and fever. However, since the chills are few and the fever is abundant, this corresponds to what medical texts describe as "Yangming Heat Malaria" (Heat-type Malaria involving the Yangming channel). It is appropriate to expel the pathogens from the Liver and Gallbladder while simultaneously clearing the heat from the Stomach.
Prescription: Raw Gypsum (Shi Gao, 30g, finely ground)
Divided equally into three packets. On days when malaria does not attack, take one packet with decoction of Bupleurum Radix (Chai Hu, 6g) in the morning; half a day later, take another packet with plain boiled water; five hours before the expected onset of malaria on the following day, take the final packet with decoction of Fresh Ginger (Sheng Jiang, 9g).
Outcome: After completing the scheduled dosage, the malaria was cured, and the sensations of heat in the heart region and anorexia also resolved. Gypsum excels at clearing Stomach heat and can also clear Liver and Gallbladder heat. The initial administration with Bupleurum Radix decoction was intended to harmonize the Shaoyang (Lesser Yang) channel and resolve its pathogens. The subsequent administration with Fresh Ginger decoction was to prevent the impending malaria from combining with the Taiyang (Greater Yang) channel, which would generate chills.
Mr. Liu XX from Tianjin, aged thirty-two, contracted malaria accompanied by dysentery in late autumn.
Etiology: Urgently needing a cart, he had to rush repairs to a broken wheel, often staying awake all night during busy periods. Excessive fatigue and hardship led to dysentery, followed by malaria.
Symptoms: His dysentery consisted of mixed red and white pus and blood, occurring more than ten times per day, accompanied by a sensation of rectal tenesmus (bearing down) and abdominal pain. His malaria occurred every other day, with mild chills and heavy fever. His pulse showed a wiry quality in both left and right positions, but the left Guan position was distinctly wiry and forceful.
Diagnosis: A wiry pulse in both left and right positions is typical for malaria. The distinct wiriness and forcefulness at the left Guan position indicate that the root of the disease lies in the Liver and Gallbladder. The wiry pulse arises because the Liver and Gallbladder have been invaded by external pathogens. Since the pathogen causing the malaria is an externally contracted heat pathogen, the left Guan pulse is wiry and forceful. The heat forces downward into the intestines, resulting in dysentery. The proposed treatment is to clear the heat from the Liver and Gallbladder and disperse the externally contracted pathogens, so that both malaria and dysentery may be cured simultaneously.
Prescription: Raw Hang Peony Root (Sheng Hang Shao, 30g) Sliced Hawthorn Fruit (Shan Zha Pian, 9g) Virgate Wormwood Herb (Yin Chen, 6g) Raw Barley Sprout (Sheng Mai Ya, 6g)
Bupleurum Radix (Chai Hu, 4.5g) Rhizoma Dichroae (Chang Shan, 4.5g, wine-fried) Rhizoma Tsaoko (Cao Guo, 4.5g, crushed) Scutellaria Root (Huang Qin, 4.5g) Licorice Root (Gan Cao, 6g) Fresh Ginger (Sheng Jiang, 3 slices)
Decoct in a large bowl of water. Take the decoction in the evening on non-malaria days, and re-decoct the dregs the next morning for a second dose.
Outcome: Following the prescribed method, both malaria and dysentery were cured. Subsequently, a prescription was provided containing Raw Huai Yam (Sheng Huai Shan Yao, 30g), Raw Hang Peony Root (Sheng Hang Shao, 9g), and Yellow Raw Chicken Gizzard Lining (Huang Se Sheng Ji Nei Jin, 3g), to be decocted and taken as one dose daily. This regimen nourishes Yin, tonifies Qi, and resolves stasis, administered continuously for several days to consolidate recovery.
