To treat hematemesis (vomiting blood) and epistaxis (nosebleed), where the patient suffers from deficiency and damage in the lower yuan (kidney and mingmen), and exhaustion of middle qi (spleen and stomach qi). The chong qi (penetrating vessel qi) and stomach qi, due to deficiency, rebel upwards. The pulse is wiry, hard, and rapid, appearing seemingly forceful upon movement.

Appendix:
My friend Mao XX once treated a young man suffering from hematemesis. This patient had previously been cured by another physician but relapsed repeatedly. Upon palpating his wiry and seemingly forceful pulse, Mao XX determined that it was caused by the rebellion of chong qi and stomach qi upwards. Consequently, in his formula for treating hematemesis, he heavily used Pinellia Tuber (Banxia) and Hematite (Heshi) to direct rebellious qi downwards, White Peony Root (Baishao) and Oyster Shell (Muli, calcined) to consolidate chong qi and drain heat, and added Ginseng Radix et Rhizoma (Renshen) to tonify middle qi, thereby strengthening middle qi to facilitate the success of the other herbs. A physician who had previously cured this patient was present and greatly doubted the use of Pinellia Tuber, but Mao XX strongly advocated for its administration. After one dose, the bleeding stopped; after two doses, the pulse became harmonious. The physician was astonished, considering it a miraculous event. Mao XX explained: "This condition stems from deficiency and damage in the lower yuan, causing chong qi to rebel upwards due to deficiency, which in turn forces stomach qi to rebel. The pulse appears forceful but is not truly so. As stated in Li Shicai's 'Four-Character Pulse Manual': 'Straight up and straight down, the chong vessel is evident,' refers precisely to this situation. If one mistakenly identifies this pulse as excess heat and recklessly uses bitter-cold drugs to cool the blood level, the blood may coagulate due to coldness, stopping the bleeding temporarily. However, blood stasis may later become a pathology, often evolving into lao zhai (consumptive disease with tuberculosis-like symptoms). In the current formula, Hematite and Pinellia Tuber are used to suppress chong qi, allowing it to remain in its original residence, while Baishao and Muli are used to consolidate and secure it, preventing upward rebellion forever. Blood is the companion of qi, and qi is the master of blood; when qi is tranquil, blood naturally becomes tranquil. This is why we do not directly treat the hematemesis, yet it stops spontaneously. Moreover, the powerful Ginseng Radix et Rhizoma assists the other herbs, enabling the descending and consolidating actions of the formula to succeed through reliance."
