Yu XX from Tianjin, aged over forty, contracted a warm disease (Wenbing) in early summer.
Etiology: After an angry dispute with someone, sweat on his head and face was exposed to wind pathogen, leading to the warm disease.
Symptoms: Fever present both in the exterior and interior; chest and diaphragm fullness resembling Jie Xiong (chest binding syndrome); severe difficulty in breathing; inability to sleep at night. The pulse is floating, wiry, and forceful on both left and right sides. The tongue coating is white and thick. No bowel movement for three days.
Diagnosis: This disease involves the Taiyang channel extending to the Yangming and Shaoyang channels. Because the pathogen is in Taiyang, the pulse is floating; because it extends to Yangming, it feels forceful upon pressure; because it further involves Shaoyang, the pulse is floating, forceful, and wiry. The chest fullness and breathing difficulty are due to anger stagnating in the chest, carrying wind pathogen and phlegm-fluids to condense in the Taiyang region. The appropriate strategy is to release the Taiyang exterior, clear the Yangming heat internally, harmonize the Shaoyang, and open up the chest diaphragm to purge turbidity. This constitutes a complete approach.
Prescription: Sheng Shigao (Raw Gypsum Fibrosum, 2 liang, crushed fine), Lou Ren (Trichosanthis Semen, 2 liang, stir-fried and crushed), Sheng Laifuzi (Radish Seed, 8 qian, crushed), Tianhuafen (Trichosanthis Radix, 6 qian), Suzi (Perilla Fructus, 3 qian, stir-fried and crushed), Lianqiao (Forsythiae Fructus, 3 qian), Bohe Ye (Menthae Haplocalycis Folium, 2 qian), Yichen (Artemisiae Scopariae Herba, 2 qian), Longdancao (Gentianae Radix et Rhizoma, 2 qian), Gancao (Glycyrrhizae Radix et Rhizoma, 2 qian).
Decoct together in a large cup of water. Take warm, then cover with blankets to induce slight sweating.
Outcome: One hour after taking the medicine, the whole body sweated, the chest felt relieved, fever completely subsided, and he slept peacefully at night. The pulse returned to normal. However, the bowels remained unopened. He was given Xi Yao (Sennae Folium, 1 qian) soaked in boiling water and taken twice, after which the bowels opened.
Zhao XX from the northern border of the county, aged nearly thirty, contracted Wind-Warmth (Fengwen) in early autumn.
Etiology: In late early autumn, farmers were busy with harvest. Excessive sweating from physical labor followed by excessive cooling in the shade of trees led to Wind-Warmth.
Symptoms: Stomach heat with rebellious Qi, causing frequent vomiting after taking medicine. Multiple doctors were consulted over ten days without effect. Upon my examination, he had high fever throughout the body and intense heat in the heart/chest. He had not eaten or drunk anything for five to six days, and no bowel movement for several days. When asked why he did not eat, he said he sometimes desired food, but all foods smelled abnormally foul to him. Forcing himself to eat caused vomiting, hence he could not eat. His pulse was wiry, long, and forceful, with the right side slightly showing a flooding quality, beating five times per breath.
Diagnosis: Based on these symptoms and pulse, the Yangming fu-organ heat is solid/excess, accompanied by Chong Qi (Rebellious Qi from the Chong vessel) rushing upward. This prevents eating and causes vomiting even when taking medicine. Treatment should focus primarily on clearing stomach heat, assisted by herbs that descend rebellious Qi. Once Chong Qi descends, Stomach Qi will follow, stopping vomiting and allowing intake of medicine and food.
Prescription: Sheng Shigao (Raw Gypsum Fibrosum, 3 liang, crushed fine), Sheng Zheshi (Hematitum, 1 liang, crushed fine), Zhimu (Anemarrhenae Rhizoma, 8 qian), Lu Dangshen (Codonopsis Radix, 4 qian), Jingmi (Oryza Sativa, 3 qian), Gancao (Glycyrrhizae Radix et Rhizoma, 2 qian).
Decoct together in a large bowl. Take warm, divided into three doses.
Formula Analysis: This formula is Bai Hu Jia Ren Shen Tang (White Tiger Decoction plus Ginseng) added with Zhesi. Bai Hu Tang is used because of the solid heat in the stomach fu organ. Ren Shen is added because of prolonged vomiting. Zhesi is added to subdue the rebellious Qi of the Chong and Stomach vessels.
Outcome: After finishing the three doses, vomiting stopped. The next day, Zhesi was removed, and another dose was taken. Bowels opened, and fever reduced by more than half. On the third day, Shigao was reduced by one liang, and Xuanshen (Scrophulariae Radix, 6 qian) was added. After one dose, the pulse became calm, and the body cooled, yet he still could not eat and disliked food odors as before. I told his family: "The disease is cured; no more medicine is needed. The reason he cannot eat is that Stomach Qi has not yet opened. Nothing opens the Stomach better than radish. Use fresh radish, cut into shreds, stir-fry semi-cooked in sesame oil, make soup with scallions and soybean paste, do not overcook, and mix in a little mung bean flour for him to drink." When the soup was ready, he refused initially. Forced to taste a little, he found it fragrant and delicious, drinking two bowls quickly. From then on, his diet normalized. He told his family: "I took over ten doses of medicine before without improvement. Today, thanks to the radish soup, I am suddenly cured. If I had known earlier, would my illness not have healed sooner?" His family laughed involuntarily.
Appendix: I recall when I was young, a neighbor suffered from external contraction complicated by phlegm and wheezing. An old local physician, Pi XX, prescribed Xiao Qing Long Tang (Minor Blue Dragon Decoction). One dose cured the wheezing, but the patient felt mist-like obstruction in the chest and could not eat. Pi XX said: "This is dampness abundance causing Stomach Qi stagnation. We need an appetite-opening agent." He prescribed Ling Gui Zhu Gan Tang (Poria, Cinnamon Twig, Atractylodes Macrocephala, and Licorice Decoction) from *Jin Gui Yao Lue*. Less than half a ke (approx. 30 minutes) after ingestion, the chest obstruction cleared instantly, and he could eat. Observers marveled at the miraculous efficacy. Mr. Pi truly deserved the title of famous doctor. I am further amazed by the profound efficacy of classical formulas (Jing Fang), which are indeed beyond ordinary comprehension. Here, one case used radish, the other a classical formula, both opening the Stomach Qi within moments. Hence, I record this.
