There is a condition where inappropriate aggressive treatment by incompetent physicians causes severe damage to Stomach Yin. In such cases, a concentrated decoction of Liuwei Dihuang Yin (Six-Ingredient Rehmannia Decoction) should be administered. If deficiency heat is present, it should be combined with Shengmai San (Generate the Pulse Powder). If Middle Qi is deficient, one may select from Liujunzi Tang (Six-Gentlemen Decoction), Lizhong Tang (Regulate the Middle Decoction), Jianzhong Tang (Establish the Middle Decoction), or Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) as appropriate.
There is another condition characterized by constipation and an inability to eat. Incompetent physicians invariably advocate purging to eliminate pathogenic factors, thereby clearing the chest and diaphragm to restore appetite. I, however, argue otherwise. One must nourish the Stomach to support Healthy Qi (Zheng Qi), and support Healthy Qi to eliminate pathogens. If the Stomach has not yet been sufficiently nourished, pathogens will not be immediately expelled; thus, the inability to eat is not a concern at this stage.
There is a condition where there is constipation but the patient can still eat. As long as old waste is eliminated and new nutrients are generated, and the Granary (Spleen/Stomach transport function) is full and overflowing, bowel movements will naturally become smooth and unobstructed. There is no need to worry about constipation in this scenario.
If the patient experiences mental restlessness and irritability, body pain as if bound tightly, cold feet, deafness, cough, or vomiting, these are signs indicating the impending eruption of macules/papules (often referring to measles or similar exanthems). Shengma Gegen Tang (Cimicifuga and Pueraria Decoction) should be used. (If the pulse is weak, Ginseng may be added; if food intake is low and stools are loose, double the dosage of Atractylodis Rhizoma/Bai Zhu.) If the condition is due to Blood Heat, Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction) is indicated. Both formulas can be modified with herbs such as Scutellariae Radix processed with wine (Huang Qin), Coptidis Rhizoma (Huang Lian), Platycodonis Radix (Jie Geng), Forsythiae Fructus (Lian Qiao), Scrophulariae Radix (Xuan Shen), Menthae Haplocalycis Herba (Bo He), and Trichosanthis Radix (Tian Hua Fen). If the heat is severe (manifesting as dry mouth, dry tongue, and yellow coating), combine with Baihu Tang (White Tiger Decoction). The treatment should continue until the rash has fully erupted. If the pulse is hidden/faint and there is irritability, this is termed "about to erupt." When the irritability ceases and the spirit becomes calm, and there are no longer any faintly visible rashes under the skin, it signifies that the eruption is complete. If after the eruption is complete, the patient still has a dry mouth and a flooding pulse, Zhuye Shigao Tang (Leaf of Bambusa and Gypsum Decoction) should be used for modification. Alternatively, one or two cooling herbs such as Rehmanniae Radix (Sheng Di) and Moutan Cortex (Dan Pi) may be added. If the pulse is flooding but forceless, accompanied by physical weakness, irritability, and thirst, add Ginseng, Ophiopogonis Radix (Mai Dong), and Anemarrhenae Rhizoma (Zhi Mu) to the base formula, allowing spontaneous sweating to lead to recovery. For those with vomiting, Banxia Houpo Tang or Banxia Tang (Pinellia Decoction) should be modified. If the pulse is flooding, rapid, and forceful, with hardness and pain in the epigastrium, dry mouth, and a tongue coating gradually turning yellow-black, this indicates pathology caused by dry fecal matter (dry stool). Da Chengqi Tang (Major Order the Qi Decoction) or Da Chaihu Tang (Major Bupleurum Decoction) should be used to purge, depending on the severity. Recovery is indicated when bowel movements occur and the tongue becomes moist. If purging is not yet indicated, but tidal fever and irritability/thirst are present, temporarily use Xiao Chaihu Tang (Minor Bupleurum Decoction) with Pinelliae Rhizoma (Ban Xia) removed, combined with Jiedu Tang (Detoxification Decoction), and add Trichosanthis Radix (Gua Lou Gen) as the main ingredient. Adding Qing Dai (Orpiment/Azurite) is also effective. (Qing Dai is bitter and extremely cold; it clears heat-toxin from the chest and heart and treats red rashes, irritability, and pain caused by Cold Damage.)
If there are symptoms of internal injury with insufficient Yuan Qi (Original Qi), mistakenly treated as an external contraction, floating deficiency fire may manifest externally, causing a rash-like eruption. However, the pulse will be deficient and large, accompanied by fatigue, reluctance to speak or move, and spontaneous sweating, which distinguishes it from exterior patterns. (Due to Qi and Blood deficiency, body pain, irritability, and fever may also occur. Treating this as an excess pattern will lead to immediate danger. Rapidly administer Buzhong Yiqi Tang; recovery occurs when the fever subsides during deep sleep.) In cases of internal injury causing eruptions, Stomach Qi is extremely deficient, and the fire of the whole body floats outward. It is appropriate to tonify to subdue the fire using Dajianzhong Tang (Great Establish the Middle Decoction). If latent Yin-cold exists internally, or if cooling medicines were mistakenly taken, forcing deficient Yang to float outward, resulting in a "Yin Rash," the pulse may appear flooding and large but feels forceless upon pressure. Or, there may be cold extremities extending beyond the elbows and knees. In such cases, first use Paoyang Jiang Lizhong Tang (Baked Ginger Regulate the Middle Decoction) to restore Yang, followed by symptomatic treatment.
