[Prescription Verse] Shaofu Ju (Abscess of the Lower Abdomen) arises at the edge below the umbilicus. The syndrome is caused by Fire stagnation entangled by Seven Emotions (Joy, Anger, Worry, Thought, Sorrow, Fear, Fright). If the swelling is high and prominent, red and vibrant, with pain radiating to the back, and it easily suppurates with thick pus, it is easy to treat. If the swelling is diffuse and hard, sunken and rotting, with thin watery pus, it is difficult to treat.
[Note] This syndrome arises from a Fire carbuncle generated by internal injury from the Seven Emotions. It frequently occurs at the Qihai (Sea of Qi), Dantian (Elixir Field), and Guanyuan (Gate of Origin) acupoints. Qihai is located 1.5 cun below the umbilicus, Dantian is 2 cun below the umbilicus, and Guanyuan is 3 cun below the umbilicus; all belong to the Ren Mai (Conception Vessel). Swelling occurring in any one of these three acupoints constitutes Shaofu Ju. If the swelling is high and prominent, red and vibrant, with pain radiating to the back, easily suppurating with thick pus, it is easy to treat. If the swelling is diffuse and hard, slowly suppurating and rotting, with thin watery pus, it is difficult to treat. Whenever this syndrome appears initially, urgently apply moxibustion to the top of the swelling, seven to twenty-one moxa cones (zhuang), until a sensation of pain or itching penetrates thoroughly. It is appropriate to administer Xianfang Huoming Yin (Immortal Life Elixir Decoction). For individuals with robust constitution presenting with intense thirst and constipation, Neishu Huanglian Tang (Internal Purging Coptis Decoction) should be administered to promote unblocking and purgation. For elderly or weak individuals, Neibu Shixuan San (Internal Tonification Ten Thousand-Expelling Powder) should be administered to facilitate rapid suppuration; if suppuration is delayed, there is a risk of penetrating the inner membrane. External treatment methods are the same as those described in the chapters on Carbuncles and Ulcers.
Formulas: Xianfang Huoming Yin, Neishu Huanglian Tang (both see Chapter on Carbuncles and Ulcers)
Neibu Shixuan San (see Chest [its] Lichong)
[Prescription Verse] Fupi Yong (Carbuncle of the Abdominal Skin) arises within the abdominal skin, swelling and dull pain in the space between the skin and fascia. Unremitting abdominal pain indicates that pus has formed. The syndrome is caused by Stagnant Fire generated from rich and greasy diet.
[Note] This syndrome arises in the space inside the abdominal skin and outside the fascia. Regardless of whether it is on the left or right, prolonged dull pain followed by the emergence of a carbuncle on the external skin characterizes it. A deep and rapid pulse at the Right Guan position accompanied by severe abdominal pain is its sign, resulting from Fire stagnation due to rich and greasy diet. In the initial stage, for robust patients, Shuangjie Guijin Wan (Double-Resolving Noble Metal Pill) should be used for purgation; for weak patients, use half the dose. If ineffective, administer another half dose. After purgation, if abdominal pain persists, it indicates that pus is about to form; urgently administer Tuoli Toung Tang (Supporting Interior and Penetrating Pus Decoction). Post-suppuration treatment is identical to that in the chapter on Carbuncle and Ulcer. Do not overuse harsh attacking agents. If one seeks resolution but excessively damages Stomach Qi, the swelling will fail to suppurate, and once suppurated, it will fail to heal, leading immediately to critical danger.
Formulas: Shuangjie Guijin Wan (see Chapter on Carbuncles and Ulcers)
