For cases where fire therapy causes adverse reactions, followed by purgation, and subsequently agitation and restlessness due to burning needle therapy, Guizhi Gancao Longgu Muli Tang (Cinnamon Twig, Licorice, Fossilized Bone, and Oyster Shell Decoction) is the primary treatment.
[Annotation]
"Fire adverse reaction" refers to any condition where qi mechanism becomes disordered due to forced sweating via fire methods. Here, it specifically refers to that caused by fire needles. Using burning needles to force sweat, followed by erroneous purgation, although the pathogenic factors from fire adverse reaction are somewhat reduced by purgation, the symptom of "agitation and restlessness" alone persists. This is because sweating and purging severely damage body fluids. Therefore, Guizhi and Gancao are used to rescue the exterior qi, while Longgu and Muli are used to secure the middle jiao. By not directly treating the agitation, it resolves spontaneously.
[Collected Annotations]
Yu Chang stated: This condition results from misdiagnosis compounded by further error. Although there are no severe changes such as fright or mania, agitation and restlessness indicate that external pathogens remain unresolved and signal the impending loss of true yang.
Cheng Yingmao stated: Fire adverse reaction combined with purgation leaves the interior qi deficient. If this deficiency is not treated, and burning needles are applied additionally, it naturally leads to the collapse of yang. The presence of only agitation and restlessness, without the extreme restlessness and inability to lie still seen in the previous clause, indicates a difference in the severity and urgency of the heat pathogen. Thus, the modifications to the previous formula differ slightly. Ultimately, one must not allow the false appearance of agitation to be confused with the true pathology.
Guizhi (Cinnamon Twig) 1 liang, Gancao (Licorice Root, prepared with honey) 2 liang, Longgu (Fossilized Bone) 2 liang, Muli (Oyster Shell, calcined) 2 liang.
The above four herbs are ground into coarse powder. Add 5 sheng of water, decoct until 2.5 sheng remains, strain the dregs, and take 8 ge of the warm liquid per dose, three times daily.
[Collected Exegesis]
Wang Hu stated: This formula is essentially Guizhi Qushaoyao Jia Shuqi Longgu Muli Jiuniu Tang (Cinnamon Twig Decoction without Paeonia, plus Pyrrosiae Herba, Fossilized Bone, and Oyster Shell for Rescue), but administered in a reduced dosage. When fire pathogen forces inward, it generates agitation and restlessness. Although agitation seems to carry exterior pathogen characteristics, dispersing it with the acrid and hot nature of Guizhi is inappropriate. Furthermore, since the fire adverse reaction has already undergone purgation, yin blood is injured. Compared to Jiuniu Tang, adding Shaoyao (White Peony Root) might seem appropriate.
