未出烦躁消毒平，已出烦躁凉血宁，浆清发烦保元治，靥后虚烦匹物灵。
[注]
痘证始终以安宁为吉，但有烦躁必生他变，
盖烦者，心愤也，躁者，身扰也，皆由毒火太盛，神不能静也，
痘末出而烦躁，是为表郁，以消毒饮主之，
痘已出而烦躁，是为血热，以凉血解毒汤主之，
若养浆时顶平清稀而烦躁，是气虚也，以加味保元汤主之，
收靥后而烦躁，是血虚也，以加减四物汤主之。

(加味保元汤) 人参 黄耆制 甘草炙 当归酒洗 白芍药炒 麦冬去心
枣仁炒研 水煎服

毒热炽盛犯心经，错语妄言神不清，妄见妄闻志昏愦，黄连解毒服即宁。

谵妄者，由毒热炽盛，上干心气，
致使错语妄言，神气不清，妄见妄闻，志气昏愦，以黄连解毒汤主之。

痘疹未出时的烦躁，用消毒饮使其平复；痘疹已出后的烦躁，用凉血解毒汤使其安宁；浆液清稀、痘顶平坦而发烦的，用保元汤治疗；结痂脱落后出现虚性烦躁的，用加减四物汤（匹物灵）调理。
[注]
痘证始终以神志安宁为吉兆，但一旦出现烦躁，必然产生其他变证。
所谓“烦”，是指心中愤懑不安；所谓“躁”，是指身体扰动不宁。这些症状都是由毒火过盛，导致心神不能宁静所致。
痘疹尚未发出而出现烦躁，这是表邪郁闭，以消毒饮为主治方剂；
痘疹已经发出而出现烦躁，这是血分有热，以凉血解毒汤为主治方剂；
若在养浆阶段，痘顶平坦、浆液清稀且伴有烦躁，这是气虚所致，以加味保元汤为主治方剂；
在收靥（结痂脱落）之后出现烦躁，这是血虚所致，以加减四物汤为主治方剂。

毒热炽盛侵犯心经，导致言语错乱、胡言乱语、神志不清，出现幻觉（妄见妄闻）、意志昏沉，服用黄连解毒汤即可安宁。

致使患者言语错乱、胡言乱语，神气不清，出现幻听幻视，意志昏沉，以黄连解毒汤为主治方剂。

For restlessness (Fan Zao) occurring before the eruption of variola (smallpox/variolae), use Xiaodu Yin (Detoxification Decoction) to calm it; for restlessness after eruption, use Liangxue Jiedu Tang (Blood-Cooling and Detoxifying Decoction) to pacify it; if the pustules are flat and clear with scanty fluid accompanied by vexation, treat with Baoyuan Tang (Protect the Origin Decoction); for deficiency-type restlessness after the scabs fall off, use Jiashi Siwu Tang (Modified Four Substances Decoction).
[Note]
In variola cases, tranquility is always auspicious, but the emergence of restlessness inevitably leads to other complications.
"Fan" (vexation/restlessness) refers to mental agitation and indignation; "Zao" (fidgeting/restlessness) refers to physical tossing and turning. These symptoms arise from excessive toxic fire that prevents the Shen (Spirit/Mind) from remaining tranquil.
Restlessness appearing before the eruption indicates exterior constraint; the primary formula is Xiaodu Yin (Detoxification Decoction).
Restlessness appearing after the eruption indicates heat in the blood level; the primary formula is Liangxue Jiedu Tang (Blood-Cooling and Detoxifying Decoction).
If during the stage of pus formation, the pustule tops are flat and the fluid is clear and thin, accompanied by restlessness, this indicates Qi deficiency; the primary formula is Jiawei Baoyuan Tang (Modified Protect-the-Origin Decoction).
Restlessness occurring after the scabs have fallen off (shou ye) indicates Blood deficiency; the primary formula is Jiashan Siwu Tang (Modified Four Substances Decoction).

Jiawei Baoyuan treats deficiency vexation,
Ren Shen, Huang Qi, and Gan Cao gather together;
Wine-washed Dang Gui and stir-baked Bai Shao Yao,
Mai Dong and Suan Zao Ren decocted as one.

Delirium (Chan Wang)
When toxic heat blazes and invades the Heart Meridian, it causes confused speech, delirious talking, and unclear spirit; with hallucinations (seeing/hearing things that are not there) and mental confusion, taking Huanglian Jiedu Tang (Coptis Decoction for Resolving Toxins) brings immediate peace.

Delirium arises from blazing toxic heat disturbing the Qi of the Heart,
