Select large river snails from the fields, in any quantity. Raise them in fresh water, collect the mud they excrete, dry it in the shade, and process it into pills. Take 30 pills per dose, administered with Huoxiang Decoction (Agastache Decoction). This formula is also effective for Fanwei (Reversal Vomiting) caused by accumulated heat.
Snails possess a very cold nature, and snail mud possesses an extremely cold nature; thus, they can be used to clear Stomach heat. Swallowing them with Huoxiang Decoction utilizes its acrid and aromatic properties to open the stomach appetite.
All conditions involving rebellious Qi rushing upward belong to Fire. Therefore, Dahuang (Rhei Radix et Rhizoma) is used, moistened with wine and subjected to nine cycles of steaming and sun-drying, to obtain its property of clearing Fire without damaging Stomach Qi. This is the "General" (a nickname for Dahuang) that has undergone prolonged processing.
Fuzi (Aconiti Lateralis Radix Praeparata, one piece)
Moisten seven times with decoction made from Ganjiang (Zingiberis Rhizoma), then grind into powder. Take 3 qian per dose.
This formula is indicated for Fanwei (Reversal Vomiting) caused by Cold Phlegm.
Cold Phlegm above the diaphragm obstructs the Qi mechanism of the Middle and Lower Jiao. Yin-Cold suppresses Yang; when accumulation reaches its extreme and suddenly unblocks, it causes Fanwei (Reversal Vomiting). Fuzi is acrid and hot, capable of resolving Cold Phlegm. Once Cold Phlegm is resolved, the Qi pathways become unobstructed, eliminating the rebellious Yang resulting from extreme accumulation; thus, Fanwei is instantly cured.
Gansui (Kansui Radix), Yuanhua (Genkwa Flos), Daji (Euphorbiae Pekinensis Radix) (stir-fried with warm medium, each 0.5 liang), Heiqiu (Strychni Semen, 2 liang, using only the finest powder), Dahuang (Rhei Radix et Rhizoma, 1 liang), Qingfen (Calomelas, 1 qian)
Grind all into powder and make water-habituated pills. Start with 5 pills per dose, gradually increasing by 5 pills, until free and rapid bowel movement is achieved.
This formula is indicated for cases where accumulated Phlegm fills the Stomach, causing immediate vomiting after eating.
If the Stomach contains pure Phlegm turbidity, it obstructs the Shaoyang Minister Fire of the Lower Jiao. When accumulation reaches its extreme and suddenly unblocks, Fanwei (Reversal Vomiting) inevitably occurs due to the disease progression. Treating this with mild agents yields no effect over years. Therefore, potent and harsh substances such as Gansui, Yuanhua, Daji, Heiqiu, and Dahuang are gathered to purge the Phlegm accumulation. The formula contains Qingfen, which serves two purposes: expelling Wind-Phlegm and mitigating the acrid and harsh toxicity of Gansui, Yuanhua, Daji, and Heiqiu. This is a pill composed of highly toxic substances, classified as a Mingxuan agent (a remedy that induces severe reactions to cure disease). It can only be administered to patients with a heavy voice and a forceful pulse. Do not administer lightly to those with a faint voice and a weak pulse.
Preface states: In recent medical texts, Fanwei (Reversal Vomiting) and Yige (Dysphagia and Obstruction) are conflated into a single syndrome. However, examination of Han and Tang era texts reveals distinct entities: Fanwei, Ye (Ye), and Ge (Ge). Fundamentally, Fanwei is distinct, whereas Ye and Ge may be treated together. Eight formulas are examined here for discerning physicians to differentiate.
