Epidemic diseases are caused by the qi of heavenly pestilence occurring in all four seasons, affecting people of all ages and sizes similarly. Pang Anchang stated: The outbreak of epidemic qi varies in scale; it may spread throughout the world, then to a region, then to a village, and finally to a single household. The elderly and young suffer similarly, all due to the influence of cosmic cycles (Yunqi). Generally, individuals have constitutions that are either robust or frail, and the pathogenic factors they contract vary in severity. Therefore, these conditions should not be treated as Taiyang Cold Damage (Shanghan) using drastic diaphoretic or purgative methods.
Danxi stated: When an epidemic affects everyone similarly, there are three treatment principles: tonification, dispersal, and descending. Tao Shangwen treated epidemics located in the Shaoyang channel using Xiao Chaihu Tang (Minor Bupleurum Decoction) added with Fangfeng (Saposhnikoviae Radix) and Qianghuo (Notopterygii Rhizoma et Radix) to disperse the pathogen. For diseases in the Taiyang channel presenting with headache, body pain, and fever, Ren Shen Bai Du San (Ginseng Powder to Overcome Miasma) is indicated. For diseases in the Yangming channel presenting with fever, eye pain, and dry mouth, Chai Ge Tang (Bupleurum and Kudzu Decoction) and Qing Re Jie Ji Tang (Clear Heat and Release Muscle Decoction) are indicated. For epidemics presenting with fever, thirst, and diarrhea, Yin Chen Wu Ling San (Artemisia Capillaris Five-Ingredient Powder with Poria) added with Chaihu (Bupleuri Radix) and Zhizi (Gardeniae Fructus) is indicated.
