A patient suffered from mania (madness) for a long time, speaking arrogantly and refusing to take medicine. I pondered that stubborn phlegm was knotted and obstructed, so medicines could not enter the body. How could it be removed? Unless the phlegm was expelled through severe vomiting, life could not be preserved. Therefore, I mixed tung oil with flour, added brown sugar to make cakes, baked them until fragrant, and fed them to the patient. After eating several cakes, the patient vomited a basin full of sticky, gum-like stubborn phlegm shortly thereafter, fell into a coma for one day, and then revived. The critical condition was stabilized. Subsequently, medicines to nourish and regulate Stomach Qi were used, and within ten days, the patient recovered.
Cold is a killing and fierce Qi that causes water to congeal and Qi to knot, making all things hide deeply. It is prevalent in winter. Those who are skilled in health preservation protect their bodies meticulously and thus avoid this pathogen. However, for individuals with weak constitutions, improper care leads to invasion by Cold, which suddenly attacks the Five Zang organs. The *Neijing* states: "When Yin pathogens are received, they enter the Zang." The symptoms include sudden collapse and loss of consciousness, locked jaw (mouth closure), aphonia, cold and rigid limbs, shivering aversion to cold, slight fever, severe pain around the navel and abdomen, vomiting and regurgitation of cold Qi, profuse cold sweat, and deep, slow pulses in all six positions. These are all signs of this condition. If there is cyanosis of the fingernails, retraction of the tongue, or retraction of the testes, the prognosis is poor. This is because the Kidney is the root of the human body; the Kidney belongs to Water and Yin, and its nature is Cold. Only Cold can injure the Kidney. Once Kidney Qi is deficient, external pathogens attack mutually, leading to tongue retraction and nail cyanosis that cannot be easily corrected, making treatment extremely difficult. If nourishment is insufficient, not only does Cold invade during frosty seasons, but even in summer months, lying on the ground facing the wind allows Cold to enter from the skin into the meridians, and from the meridians into the Zang-Fu organs, causing disease. Thus, it is said that the Kidney is the root of the human body. If symptoms such as locked jaw, acute pain, syncope, and coma occur, it is called "Zhong Han" (Mid-Cold). Treatment should employ warming methods using Lizhong Tang (Regulate the Middle Decoction) and Sini Tang (Frigid Extremities Decoction). If headache, aversion to cold, stiffness, and mild abdominal pain occur, it is called "Gan Han" (Sensation of Cold). Treatment should employ dispersing methods using Wuji San (Five Accumulations Powder) and Zhengqi San (Righteous Qi Powder). In cases where lips are blue, limbs are cold, and the condition is critical, moxibustion can be rapidly applied to the Dantian acupoint below the navel until the limbs warm up. Alas, treating such conditions requires assessing the severity and adapting accordingly.
Dai Yun stated that Zhong Han (Mid-Cold) occurs when the body is exposed to killing atmospheric influences or consumes raw/cold foods, causing severe deficiency of the Stomach and sparse pores, leading to immediate illness upon sudden invasion. Symptoms include cold extremities, fatigue, weak respiration, heart and abdominal pain, and deep, fine pulses. Even if there is heat, there is no thirst, and the patient is reluctant to speak or move. Urgent warming is required; delay renders it incurable. This differs fundamentally from Shanghan (Cold Damage) and febrile diseases, like the difference between water and fire; they cannot be treated identically, and misdiagnosis can be fatal.
Cold injuring the human body involves both mild sensation (Gan) and severe mid-strike (Zhong). Sensation is mild, while mid-strike is severe. Mid-strike refers to Cold entering the Five Zang organs, causing severe colic, cold extremities, deep and slow pulses, a body feeling as if beaten by a club, vomiting, diarrhea, and cold sweat. This is the Yin syndrome of Zhong Han. Cases with cyanotic nails, retracted tongue, and retracted testes are difficult to treat. Symptoms such as headache, abdominal pain, shivering aversion to cold, slight fever, vomiting, and cold sweat constitute Gan Han (Sensation of Cold).
