Adults and children, if the glans penis is covered by skin, leaving only a small aperture, making urination difficult to dribble out. Insert a bone needle into the aperture and gradually expand it. If the skin opening is slightly larger, use scissors; lift the skin beside the Mako (urethral meatus) with forceps, cut it open as needed, and stop the bleeding quickly. Alternatively, pass a medicinal thread through the skin beside the Mako using a fine needle for a width of several fen, then tie a slipknot on the medicinal thread and gradually tighten it. After seven days, the skin will naturally split, thereby enlarging the Mako.
When an abscess or carbuncle first swells, it becomes hard as stone, or gradually enlarges, or remains unchanged in size. Mistakenly using knives or needles causes unceasing blood flow or leakage of clear fluid, with the affected wound everted and protruding, making it difficult to treat. Administering the method of soothing the Liver and tonifying the Spleen may still prolong life.
Treating ulcerated abscesses that fail to heal after a long time. If there is much old pus inside the hole and dead bones have formed, one must use Jiang Dan Tiao (Red Sublimate Paste Strips) to extract the pus and remove necrotic tissue before the wound can close. In cases where excessive pus and blood loss lead to Qi and Blood deficiency preventing healing, or when the wound has just closed but the skin and flesh are still tender, premature labor causes recurrence of swelling and ulceration with discharge of clear pus or foul-smelling turbid water. These are all due to damage and failure of the Spleen Earth, requiring tonification of the Spleen and moderation of labor and desire for recovery. There are cases of long-standing ulcers penetrating through, forming fistulas, which are incurable.
Abscesses and carbuncles swollen like cotton, with normal skin color, large as a cup or plate-like, with ceaseless pain and ineffective treatment, indicate Qi deficiency and exhaustion of Blood, causing obstruction in the collaterals and pain. Puncture with knives or needles is contraindicated. If upon ulceration, green-yellow fluid flows or black blood is discharged, with persistent swelling and pain, or the wound is everted, or swelling subsides but the wound fails to close, prolonged duration leads to reduced appetite and emaciation; these are beyond treatment.
During the onset of disease and after recovery, the Spleen and Stomach are extremely deficient. If food intake is not regulated, stagnation occurs in the Middle Jiao, making digestion extremely difficult. This suppresses latent heat, generating dampness and phlegm. The Spleen and Stomach are the officials of the granaries; when phlegm accumulates and dampness steams, it resembles fermenting yeast, unable to excrete outward onto the skin and muscles, causing yellowing of the face, eyes, and limbs. The method of resolving stagnation and leaching out dampness should be applied.
Previous illnesses, although temporarily receded, often recur as usual or worsen one or two days before solar term changes. This is because heaven and earth change solar terms, causing all things to alter their appearance. The internal organs of the human body also react to climatic changes, triggering old injuries and diseases. Treatment is more difficult to cure than at the initial stage, and there are cases of bedridden patients who cannot be saved.
