One prescription: For "being struck by a ghost" (sudden severe fright or acute sudden illness), use Fulonggan (Caulis Longani, specifically Caoshixin Tu / Hearth Earth) ground into powder, mixed with fresh cold water and taken orally, 2 qian.
Another prescription: To treat shiyan (nightmare/sleep paralysis where the patient appears dead but pulse remains), use Zaotu Mei (Soot from the chimney, also known as Baicaoshuang) in a pill size of a large marble, mixed with rice water and drunk.
Another prescription: To treat frail individuals experiencing trance-like visions of ghosts and madness, use Pingwei San (Peaceful Stomach Powder) added with Chen Sha (Cinnabar) powder, mixed with Jujube decoction and taken orally, 2 qian per dose, twice daily. Stop when cured.
The Classics state: When Yang Qi and Yin Qi struggle against each other, and Water and Fire are antagonistic, one experiences startled fear. When Yin Qi is deficient and Yang Qi enters inwardly, the struggle between Yin and Yang leads to terror.
It is also stated: Pain, soreness, and shock all belong to Fire. Furthermore, when the Shaoyin and Yangming meridians are affected, the primary manifestations are shock/back pain. If one hears the sound of bells and drums without fear, but is startled by the sound of wood, it indicates Yangming disease. The Yangming corresponds to the Stomach Earth. When Stomach Earth is deficient, it fears Wood and becomes startled.
The human body is governed by the Heart, and the Heart governs Blood. Once Heart Blood is deficient, Shen (Spirit) fails to reside within, which is the origin of Jingji (palpitations/fright). Jing refers to fear, while Ji refers to trembling palpitations. When Blood is insufficient, Shen does not reside; thus, symptoms of fright, fear, and palpitations arise.
Zhengtong syndrome is characterized by a constant feeling of inner trembling and restlessness, unable to find peace, occurring at any time. Jingji is characterized by sudden jumping/startling, immediate fright upon hearing sounds, or palpitations triggered by events, occurring intermittently. Although the clinical presentations differ slightly, the treatment principles are largely the same: both result from overwork or injury to the Heart, Liver, and Gallbladder.
The *Neijing · Daqi Lun* states: A rapid and forceful Liver pulse indicates shock due to fright. A rapid pulse can cause sudden fright. A tense Yangming pulse causes fright. A Cun Kou pulse that is moving and weak indicates that "moving" signifies fright and "weak" signifies palpitations. A tight Cun Kou pulse combined with a floating Duyang pulse indicates deficient Stomach Qi, leading to palpitations.
Treatment of Jingji must first employ formulas to nourish the Heart and calm the Spirit, followed by resolving phlegm, or using emesis methods. If there is constipation and a full/excess pulse, Zhusha Guntan Wan (Cinnabar Rolling Phlegm Pill) should be used to purge. If one dose is ineffective, administer another; cure is assured.
Danxi stated: Jingji mostly belongs to Blood deficiency; movement occurs with thought, indicating deficiency. Intermittent onset and remission indicate phlegm stirred by fire. Thin people mostly have little Blood; fat people mostly have much Phlegm. Frequent heart palpitations also indicate Blood deficiency. Zhengtong occurs continuously, whereas Jingji occurs intermittently.
