Qian Yi stated: Vomiting and diarrhea occurring ten days after the Summer Solstice, accompanied by high fever, is a heat syndrome. This is because in children, nine parts out of ten of their internal organs are characterized by heat. If caused by heat injury and indigestion of milk or food, resulting in dark yellow stools, Yulu San (Dew-Drop Powder) is the primary treatment. After the Great Heat solar term, vomiting and diarrhea occur with a warm but slightly hot body temperature; the internal organs exhibit six parts heat and four parts cold. For vomiting with less milk intake before feeding, Yihuang San (Yellow Benefit Powder) should be administered; for more milk intake after feeding, Yulu San is used. Seven days after the Beginning of Autumn, vomiting and diarrhea present with a warm body temperature; the internal organs show three parts heat and seven parts cold. The child cannot feed on milk, sleeps excessively, feels distressed and chaotic, experiences hiccupping, sighs deeply, has eyes that partially open (exposing the whites), white lips, frequent hiccups, desires to defecate but feels no thirst, and symptoms are more prominent before feeding (treat with Yihuang San) or less prominent after feeding (treat with Yulu San). After the Autumn Equinox, vomiting and diarrhea accompanied by cold body indicate a lack of Yang energy. Inability to feed, dry heaving, and passage of green watery stools require spleen tonification, primarily treated with Yihuang San.
Zheng stated: Pediatric vomiting and diarrhea result from external wind-cold invasion, internal injury from milk or food, or feeding while the infant is still crying and breathing unregulated. Qi reversing upward stagnates in the chest and diaphragm, causing vomiting; Qi reversing downward damages the spleen and stomach, causing diarrhea; when both upper and lower Qi reverse, vomiting and diarrhea occur simultaneously. Cases where infants only vomit without diarrhea are considered adverse prognoses; such vomiting involves phlegm, and if convulsions develop, none survive. Cases with only diarrhea without vomiting, or simultaneous vomiting and diarrhea that persist for a long time, may transform into Yin epilepsy. Treatment requires temporarily stopping breast milk; for mild cases, one day; for severe cases, three days. Frequent administration of thin rice gruel is recommended. Medication acts quickly, with an eight or nine out of ten cure rate. Some do not believe this advice, arguing that infants rely on milk for life and refuse temporary cessation. However, while milk cannot be completely stopped, it is unknown that if the illness was caused by milk injury, continuing to feed the offending milk is like carrying firewood to save a fire. For acute vomiting and diarrhea due to dual deficiency of spleen and stomach, Shenzhu San (Ginseng and Atractylodes Powder) with ginger and pitted dates decocted is used to regulate stomach Qi.
