Hoarseness with Throat Carbuncle
This condition arises from severe exposure to cold, causing obstruction of the Lung organ (Lung), resulting in hoarseness, difficulty in swallowing liquids or solids, and possibly the appearance of ulcerated spots. The pulse presents as deep and choppy at the Lung position, and surging and large at the Spleen and Stomach positions, accompanied by coldness in the back and fever in the body. Treatment utilizes Liuwei Tang (Six-Ingredient Decoction) added with Qianghuo (Notopterygii Rhizoma et Radix, 2 qian), Gegen (Puerariae Lobatae Radix), and Ziyè (Perillae Folium, 1 qian each). The decoction is used for gargling; after one dose, if the voice returns to normal, remove the first three herbs and substitute with Huafen (Trichosanthis Radix, 1 qian), Ruxiang (Olibanum, 5 fen), Gegen (Puerariae Lobatae Radix), Huangqin (Scutellariae Radix, wine-fried), Guiwei (Angelicae Sinensis Radix caulis), Chishao (Paeoniae Radix Rubra), Shanjia (Manis Squama), and Jiaoci (Gleditsiae Spina, 2 qian each). Administer again. Concurrently administer Baxian San (Eight-Immortal Powder) and Yushu Dan (Jade Pivot Pill). Two doses will lead to complete recovery.
Single Throat Carbuncle
A single throat carbuncle occurs either on the left or right side. Symptoms include fever in the body and coldness in the back, indicating pathology involving the Spleen and Lung organs. The presence of red spots indicates Wind-Fire; absence of red spots indicates Wind-Cold. The pulse presentation is as described previously. Treatment uses Liuwei Tang (Six-Ingredient Decoction) added with Ziyè (Perillae Folium) and Qianghuo (Notopterygii Rhizoma et Radix, 2 qian each), used as a gargle for one dose. On the following day, add Chishao (Paeoniae Radix Rubra), Guiwei (Angelicae Sinensis Radix caulis), and Dogen Shanzhi (Gardeniae Fructus with root bark, 1.5 qian each). One dose results in cure.
External Symptom Throat Carbuncle
This condition manifests below the mandible and above the Tiantu acupoint (CV22), with swelling both internally and externally, hindering diet. Initially, there is no phlegm-saliva, and no visible signs internally; this is classified as Wind-Toxin Throat Carbuncle. Treatment utilizes Liuwei Tang (Six-Ingredient Decoction) added with Huangqi (Astragali Radix), Jiaoci (Gleditsiae Spina), Shanjia (Manis Squama), Guiwei (Angelicae Sinensis Radix caulis), Chishao (Paeoniae Radix Rubra), Heche (Placenta Hominis), Honghua (Carthami Flos), Gegen (Puerariae Lobatae Radix, 2 qian each for the first five, 1 qian for the latter two), and Ruxiang (Olibanum, 5 fen). Administer continuously for three doses until the swelling subsides. If suppuration has occurred, a fistula will inevitably form. Use Shiquan Dabu Tang (Decoction for Ten-fold Tonification) to conclude treatment.
Cheek-Hugging Throat Carbuncle
This carbuncle forms below the cheek, named Xuan Yong (Hanging Carbuncle), caused by the accumulation of cold qi. Externally, apply Gongzhi moxibustion (Palace Moxibustion method) for two moxa cones. Internally, use Liuwei Tang (Six-Ingredient Decoction) added with Shanjia (Manis Squama), Guiwei (Angelicae Sinensis Radix caulis), Jiaoci (Gleditsiae Spina), Chuanxiong (Ligustici Chuanxiong Rhizoma), Baizhi (Angelicae Dahuricae Radix, 1 qian each), Shengma (Cimicifugae Rhizoma, 3 fen), Honghua (Carthami Flos), and Ruxiang (Olibanum, 5 qian each). Treat until the swelling resolves. If pus forms, puncture immediately. If a fistula develops, frequently use Renshen (Ginseng Radix et Rhizoma) for internal support; recovery may be achieved. Do not neglect such conditions.
